TITLE: Radioimmunoassay determinations of prostaglandin E in pleural effusions of varying causes.

The concentrations of prostaglandin E (PgE) were measured in pleural fluid samples from 26 consecutive patients with pleural effusions of varying causes. The PgE concentrations were significantly increased in exudates when compared with transudates and provided a single measurement to separate these 2 classifications of pleural fluid. The highest concentrations of PgE were found in exudates associated with tuberculous pleuritis and metastatic lung cancer. Simultaneous measurement of pleural fluid and plasma PgE concentrations in patients with malignant effusions revealed that pleural fluid concentrations did not reflect plasma concentrations.

A 60-year-old woman presented with diffuse bone pain 12 months after simple mastectomy. During investigation for metastatic disease it was realized that her pain had preceded the mastectomy by 2 years and that the biochemical and radiological findings were inconsistent with metastatic disease. The findings of phosphaturia, hypophosphataemia, normocalcaemia and normal renal function without hyperparathyroidism are discussed in the light of the rarity of such findings in a 60-year-old adult and, in particular, in a patient with cancer of the breast.

The limitations and advantages of the different ultrasonic breast-scanner (manual and automated) were described, likewise experiences with an automated waterpath scanner (Octoson). For breast cancer screening the automated scanner seems to be an acceptable concept, because it manifests the breast with an excellent anatomic view. In addition the mamma is in a stable position and the examination can be properly delegated. The manual examination can be used as an additional examination. Ultrasound scan can be correlated with the manual examination and a cyst can be punctured under ultrasonic control. Essential technological improvements are to be expected by both ultrasonic systems.

A series of 794 patients with laryngeal cancer in Liverpool from 1965 to 1983 were reviewed with particular reference to the results after supraglottic laryngectomy. The results indicated that radiotherapy is to be preferred for T1N0 supraglottic tumours, and supraglottic laryngectomy is indicated only for patients with small primary tumours, and clinically involved lymph nodes. On comparing 274 patients with laryngeal cancer from Auckland, seen over a similar period (1965-1979), it is clear that even fewer patients than in Liverpool, both absolutely and relatively, would be eligible for supraglottic laryngectomy in New Zealand. In view of the reportedly high morbidity and mortality associated with the operation, one may question the wisdom of performing supraglottic laryngectomy in New Zealand, where suitable patients are rare.

TITLE: Serum selenium in patients with ovarian cancer during and after therapy.

Serum concentrations of selenium were determined by atomic absorption spectrophotometry in 40 patients with ovarian cancer in association with and after surgical and cytostatic therapy. Patients with ovarian cancer had significantly (p less than 0.001) lower serum concentrations (mean +/- SE) of selenium (0.93 +/- 0.04 mumol/l) than age-, weight- and place of residence-matched control subjects (1.22 +/- 0.03 mumol/l). In clinical stage IV disease there was a lower serum level of selenium (0.82 +/- 0.07 mumol/l) than in clinical stages I and II combined (1.00 +/- 0.04 mumol/l). Serum selenium concentrations also showed a tendency to follow the outcome of the disease; an increase in patients with remission and a decrease in patients with progressive disease, probably because of nutritional reasons.

Four patients (2 with renal cell carcinoma, 1 with testicular embryonal carcinoma, 1 with testicular seminoma) underwent surgical resection for pulmonary metastases. Three patients except one with renal cell carcinoma are still alive without disease at 1, 2.3 and 2.3 years, respectively. Surgical resection of metastatic pulmonary sarcoma or carcinoma is now the accepted method of treatment. As chemo- and radio-therapy are often ineffective for metastatic genitourinary cancers, our current policy is to employ surgical treatment provided that the operability is assessed carefully.

The characteristics of feline mammary cancer--that is, the rapid growth of primary mammary tumors, the high rate of tumor recurrence, and the poor survival statistics--demonstrate the need for (1) early diagnosis of the primary tumor; (2) immediate, aggressive surgical therapy; and (3) frequent follow-up examinations to detect early clinical signs of recurrent disease. A number of factors that influence prognosis have been described. As more information becomes available concerning the behavior of feline mammary tumors and the results of various forms of treatment, more effective protocols can be developed. Continued etiologic research may play a vital role in determining the direction of therapy.

Zinc and copper levels in erythrocytes, plasma, and whole blood were determined in 35 cancer patients and compared with 24 normal individuals. A decrease in zinc was found in all three blood constituents of the cancer patients. The decrease was significant in plasma and whole blood and nonsignificant in erythrocytes. Copper levels in the cancer group showed a slight and nonsignificant increase in erythrocytes, plasma, and whole blood. The copper to zinc ratio revealed a significant increase only for plasma levels. Further investigations are indicated to determine whether these two elements could serve as indicators for diagnosis or prognosis in cancer patients.

A new endometrial sampling device, the Endopap, was tested in a series of 851 patients. This sampler is of simple design, without moving parts, inexpensive and easy to use. Cellular samples proved adequate in 90% of the cases, usually with very abundant material. Endometrial cancer shed atypical cells in all 20 cases studied. However, only about half the patients with adenomatous hyperplasia were correctly identified by the endometrial sample. This fact seems to reflect the lack of adequate morphologic criteria for the recognition of endometrial hyperplasia; this situation prevails with all types of endometrial cell samplers.

Frequent distributions of different histological diagnosis and stages of lung cancer patients were examined among 4 different age groups consisting of (1) older than 70 years old, (2) 69-65, (3) 64-60 and (4) younger than 60. All patients examined were male and their primary tumor had not been removed. The worst prognosis was found among the age group 69-65 years followed by the older than 70 years. Patients whose age was younger than 60 showed better prognosis. Elderly patients should be irradiated with about 10% smaller radiation field. When chemotherapy is combined with irradiation, radiation dose and field should be lower than with irradiation alone.

TITLE: [Case of histopathological findings of choriocarcinoma in stomach cancer]

A case of gastric cancer which contained a part of choriocarcinoma was reported in this study. The patient was a 64-year-old man, who was operated on with the diagnosis of gastric cancer in the prepyloric region. The postoperative histological examination revealed that typical choriocarcinoma was noted in a part of the adenocarcinoma, which occupied almost the entire portion of the surgical specimen. In addition, the precise localization of human chorionic gonadotropin (hCG) in the portion of choriocarcinoma was proved by the indirect immunoperoxidase stain (PAP method). The patient showed the gynecomastia clinically. The concentration of postoperative serum hCG was elevated and testicular abnormality was not noted clinically.

The efficacy of high doses of radioiodine for total ablation of functional remnants left behind after total surgical thyroidectomy for thyroid cancer decreases with the number of treatments as well with the course of therapy. Most effective are 500 mCi of 131I administered in 5 fractions within 3 years. Up to 1000 mCi of 131I delivered within 5 years are still powerful but much less so. All further treatments with high doses of radioiodine are only of diagnostic value for detection of functional metastases with low radioiodine uptake. In about 45% of all patients even excessively high doses of radioiodine failed to ablate all functional residual thyroid tissue.

Thirteen patients with retinitis pigmentosa were graded in terms of vitreous change and compared by vitreous fluorophotometry to an equal number of normal patients. A highly significant difference was found between groups. A high correlation was also noted between severity of vitreous change and midvitreous fluorophotometry values. Certain types of vitreous detachments appear to produce a characteristic "cancer hump" in the resultant data. A possible explanation for the increased values is discussed.

TITLE: Use of ultrasound scan in prostatic I-125 implantation.

The average dimension of prostate was measured by transabdominal ultrasonography preoperatively to compare with direct intraoperative measurements in 28 patients undergoing suprapubic I-125 seed implantation for the treatment of prostatic cancer. A highly significant correlation (r = 0.932) was found between these two measurements. Transabdominal ultrasonography is a relatively simple and accurate non-invasive method of determining the number of I-125 seeds needed to implant the patient with prostatic malignancy.
