TITLE: Ductular carcinoma of the breast: serum antibodies to tumor-associated antigens.

Serum antibodies to tumor-associated antigens of breast carcinoma have been studied by indirect immunofluorescence in 109 patients with breast carcinoma and 125 controls, including age/sex matched normal individuals, patients with nonmalignant disease, and patients with malignant disease other than breast cancer. We report here that sera of a large proportion of patients with ductular carcinoma of the breast have antibodies to cell surface and/or intracellular antigens of autologous tumor cells and include evidence that the antigens are absent from a considerable range of normal and other types of malignant tissues. In addition to testing of control sera, specificity of the reacting antibodies was investigated further by testing of sera with normal breast tissue and the absorption of sera from breast cancer patients with various normal tissues and cancer cells. The significance of the findings in breast cancer is discussed.

TITLE: Establishment of a cell line of human skin squamous cell carcinoma in vitro.

A cell line of a squamous cell carcinoma of human skin (HSC-I) has been established in vitro, and successfully maintained proliferative in continuous tissue culture for about 2 1/2 years since March 1978. Another cell line (HSC-Ib) has been established in vitro from the recurrence of the same cancer. It has been maintained for about 8 months since December 1979. 3T3 feeders were used in the initial cultures in both cases. The cells grow in a monolayer in vitro and are epithelioid with anaplastic features. Chromosome analysis of HSC-I, frozen and thawed HSC-I, the transplanted tumour in a nude mouse and HSC-Ib all showed hypotetraploidy with the modal number 80, which reconfirms that HSC-I derives from the original skin tumour. The cell line is available to other investigators.

TITLE: Identifying the primary site in metastatic cancer of unknown origin. Inadequacy of roentgenographic procedures.

Two hundred sixty-six patients with metastatic nonsquamous carcinoma of unknown origin underwent upper and lower gastrointestinal series, intravenous pyelograms, and chest roentgenograms (CR) for location of a primary cancer site. Of 129 identified cancer sites, only 22 were verified antemortem, whereas necropsy disclosed 25 cases with false-positive examination results. The CR patterns thought typical for lung cancer (eg, single mass lesion and hilar or mediastinal adenopathy) were often shown (43%) to represent metastatic lesions. Because contrast roentgenographic studies are costly, uncomfortable, of low yield, and often misleading, they should be limited to cases with specific organ dysfunction.

TITLE: Prospective evaluation of endoscopic retrograde cholangiopancreatography in the diagnosis of periampullary cancers.

In a prospective study of 112 patients suspected of periampullary cancer endoscopic retrograde cholangiopancreatography (ERCP) was successfully performed in 87 patients (78%). Technical failures were due to gastric outlet obstruction in four patients and inability to cannulate the ampulla of Vater in 21 patients. Successfully performed ERCP had both a high sensitivity (92%) and specificity (90%) for periampullary cancer. The few errors in pancreatogram interpretation were due to juxta-ductal cancers and difficulty in differentiating duct changes of cancer from those of chronic pancreatitis. Pancreatic cytology, performed in 21 patients, was reliable, diagnosed two pancreatic cancers when the pancreatogram failed and, if used routinely, assists interpretation of the pancreatogram. Retrograde cholangiography provided a correct diagnosis in six jaundiced patients with normal pancreatograms. Three complications of ERCP occurred in this series.

TITLE: Early diagnosis of metachronous colorectal carcinoma.

The frequency of metachronous colorectal tumours in a prospective 20-year series of patients with colorectal cancer is reported. Thirty-one patients, that is 2.1% of the patients operated on for cure, had metachronous carcinomas, and 5.4% had metachronous polyps. One-third of the metachronous carcinomas were diagnosed at scheduled postoperative control examinations, and the others because of their symptoms or at autopsy. The reported metachronous carcinomas are considered to represent failures of follow-up only and not to indicate their true incidence, since their natural course has been arrested by removal of adenomas whenever found. Several of the metachronous tumours were judged to be overlooked synchronous ones, and therefore a thorough postoperative examination of the remaining large bowel is advocated. The finding of subsequent tumours in this series indicates that continuous follow-up is worthwhile.

TITLE: 5-Di-(2'-tetrahydropyranyl)secalonic acid D as a new antibiotic derivative with anticancer activity.

A pyranyl derivative was chemically synthesized from secalonic acid D, an antibiotic obtained from culture filtrates of Penicillium oxalium, and its anticancer activity towards a highly antigenic rat bladder cancer, BC-47, implanted into inbred ACI/N rat was studied. The anticancer effect of the drug was similar to that of adriamycin. Delayed initiation of treatment, starting 5 days after the cancer implantation, was more effective than treatment starting from day 1. In addition, it was less effective in an immunodeficient host subsequently implanted with BC-47. This compound is thought to retard the cancer cell growth until the appearance of tumor immunity in the host.

22 patients with advanced prostatic cancer underwent transnasal-transsphenoidal hypophysectomy. 2 patients died immediately after the operation, when this method was introduced. Of 19 patients, who were followed at short time intervalls, 13 = 68,5% showed a good remission, which lasted on an average of 5.5 (3-8) months. The survival time of these patients with a good remission was 11.5 months, whereas in patients without any remission it was 4.8 months. We recommend the hypophysectomy in individual cases as a possible form of treatment, which might lead to relief of pain and to objective tumor remission.

Urinary hydroxyproline measurements were performed in a group of health volunteers as well as patients with cancer and myelofibrosis. Patients in whom there was no metastatic involvement of bone marrow excreted an amount of hydroxyproline not different from that of the control group. Those who had marrow metastasis produced elevated levels of hydroxyproline; the highest excretions were observed when marrow fibrosis was associated with metastasis. These results contrasted with those observed in agnogenic myeloid metaplasia patients whose excretions were equivalent to the control group. The result suggests differences in the pathogenesis of myelofibrosis and a technique potentially useful for distinguishing between patients who may otherwise be diagnostic problems.

TITLE: Physicochemical properties and isoenzyme composition of hexokinase from normal and malignant human tissues.

The activity of hexokinase was studied in several normal and malignant human tissues. The enzyme activity in tumors was significantly higher. Isoenzyme studies on normal gastric mucosa and stomach cancer extracts showed that malignancy is accompanied by a "simplification" of the hexokinase isoenzyme pattern due to "deletion" of the slowest isoenzyme. Preparative polyacrylamide gel electrophoreis was used to isolate hexokinase isoenzymes from normal and malignant tissues. Tumor hexokinase isoenzymes displayed an increased affinity to glucose when compared to the corresponding normal prototypes (Km/glucose, 10(-6) M and 10(-5) M, respectively; Km = Michaelis constant). The molecular weights, subunit composition, and peptide patterns were identical for corresponding isoenzyme pairs from normal and tumor tissues.

TITLE: Inhibition of skin carcinogenesis in vivo by caffeine and other agents.

The induction of skin cancer in mice of the Swiss (Carshalton) strain, by repeated irradiation with UV-light, was strikingly reduced by the local application of caffeine prior to each exposure. Theophylline displayed the same activity. These two substances have been selected as probable inhibitors of error-prone, postreplicative DNA repair. Conversely, reductone and chloroquine, which are considered as inhibitors of the error-free, prereplicative excision repair, did not modify the incidence of the tumors. Special emphasis has been given to the histologic behavior of radiolabeled caffeine in the normal and UV-irradiated epidermis of the mouse in vivo and to the ability of mouse epidermal cells in vitro to repair DNA after UV irradiation.

The primary objective of this study was to determine whether there was an increased mortality, especially with respect to cancer of the lung, among spray painters in the automobile manufacturing industry. The study was carried out at ten assembly plants from five participating companies and utilized both proportionate mortality ratio (PMR) and case-control analyses. Among workers with spray-painting experience, there were no statistically significant PMRs for lung cancer, either for all companies combined or for any company individually. In addition, the case-control analyses did not demonstrate a statistically significant excess risk for lung cancer mortality, adjusting for length of employment and duration in spray painting. The case-control analysis was large enough to detect approximately a two-fold increase if, in fact, an increase that large actually existed.

Because of the persistently high mortality from sepsis in cancer patients, a retrospective study was designed to identify the causative organisms and to determine the factors affecting the outcome of sepsis. A total of 84 episodes of septicemia in 61 children with cancer were studied. The more frequently isolated organisms were: Staphylococcus aureus (21.4%); Escherichia coli (18%); Klebsiella (7.1%); Pseudomonas (6%); and Bacteroides fragilis (6%). Other isolates included Proteus, Serratia, Acinetobacter, hemolytic Streptococcus, and Bacillus cereus. In 10.7% of septic episodes, mixed bacterial infections were documented. Twenty-four (28.6%) resulted in death; in 13 (54%) death occurred within 24 hours after admission. The fatality rate was high in cases associated with absolute polymorphonuclear leukocyte counts of less than 100/cu mm, in neoplastic relapse, and when infection with gram-negative, anaerobic and mixed bacterial flora occurred.

90 out of 202 lung cancer patients hospitalized for preoperative investigations were scheduled for radical surgery. 15 patients were not operated on because of insufficient lung function data. The operation was performed in 75 patients. The complication rate was 11% and mortality 8%. 7 out of 8 patients who subsequently developed severe complications had fulfilled the criteria for lung resection only after intensive antibronchitic treatment. Nearly all the survivors showed the predicted preoperative minimal lung function values after the operation. The limit of 1000 ml for postoperative FEV1 is justified, since patients with these or larger functional reserves nearly all had a favourable early and late postoperative course, and also because lung function values continuously deteriorate in chronic obstructive lung disease.

TITLE: Reliability of histo-pathological diagnosis of squamous epithelial changes of the uterine cervix.

The reliability of histological diagnosis of squamous epithelial changes was tested by letting 13 pathologists read 1,001 consecutive cervical biopsies twice. Intra-observer and inter-observer agreement, variance, and deviation of diagnosis were determined. The diagnostic ability showed great individual variation and no significant correlation to experience in pathology. The diagnosis of invasive cancer had a high diagnostic specificity, and the diagnostic sensitivity of the diagnosis of no significant epithelial changes was high too. The reliability of the diagnosis of dysplasia and carcinoma in situ proved unsatisfactory.

Chronic pain may be considered a disease and its treatment a necessity. Neurosurgical treatment of chronic pain is justified in cases where conservative treatment is no longer effective or causes excessive side effects. The new percutaneous methods involve no stress, minimal risk and short hospitalization. Destructive neurosurgical procedures are mainly used for cancer pain, with the exception of trigeminal neuralgia. Non-destructive neurostimulating methods to control pain are well on the way to achieving their optimum clinical potential and preserve the integrity of the nervous system.
