TITLE: Biological principles and clinical development of prostate cancer gene therapy.
PUBDATE: 19970201
Several clinical trials of prostate cancer gene therapy are underway. The therapeutic genes being evaluated by these and other, forthcoming, trials include immunogenes, cell death-inducing genes, antioncogenes, and tumor suppressor genes. Constraints of current gene transfer vectors limit, to local-regional targeting, those gene therapy strategies that do not rely on an intervening immune response. Preclinical models predict modest, if any, therapeutic effects with current forms of prostate cancer gene therapy. Using biological surrogate endpoints and applying further preclinical advances will allow clinical prostate cancer gene therapy to attain its full potential.

PUBDATE: 19970501
A case of adenoid cystic carcinoma (ACC) of the breast in a 71-year-old woman is reported. This neoplasm accounts for about 0.1% of all breast cancers. The mammography showed a well-delineated mass without calcifications. Cytologic examination of percutaneous fine-needle aspiration (FNA) material gave the diagnostic of ACC, which was confirmed by histologic examination. This report emphasizes the utility of immunocytochemical study with collagen IV antibody on FNA material of this uncommon tumor.

This review focuses on ways to ameliorate diagnosis and treatment of cancer-related pain with currently available knowledge and methods. The first part indicates how to improve pain assessment and diagnosis in the cancer patient. The second part evaluates current views for adequate pain management based on evidence of double-blind analgesic trials in cancer-related pain and deals with misconceptions in established symptomatic therapy.

PUBDATE: 19950201
We report a case of T1N0M0 lung cancer in which we successfully performed video-assisted right lower lobectomy and mediastinal lymph node dissection. The lobectomy was done on a 64-year-old woman who had a 2-cm adenocarcinoma in the right lower lobe. The procedure could be done safely and adequately using three trocars and a 5-cm mini-thoracotomy. The patient had an uncomplicated postoperative course and was satisfied with minimal postoperative pain, quick recovery, and minute skin scars. Video-assisted lobectomy may be a reasonable approach in selected patients of primary lung carcinoma.

PUBDATE: 19970501
Three pivotal phase III trials conducted in North America and Europe served as the basis for the application for approval of toremifene (Fareston) by the FDA. These trials demonstrated that 60 mg/d of toremifene is safe and effective in the treatment of advanced breast cancer in postmenopausal women. The studies also indicated that, on the basis of antitumor efficacy, as well as safety, toremifene is at least equivalent to tamoxifen and may have some long-term advantages.

It is believed that over one-half of people diagnosed with cancer will at some point use an unconventional therapy (Hopkins & Brigden, 1991). In light of this, nurses have a professional responsibility to learn about unconventional therapies (Fletcher, 1992) so they can help their patients make informed choices. This article focuses on Essiac, an unconventional therapy that has historically and is presently being used by some cancer patients. While reasons patients might choose unconventional therapies are identified, the majority of this article focuses on the history of Essiac, where it stands today, and the need for sound scientific research.

PUBDATE: 19960201
Integrins comprise a large family of heterodimeric proteins that mediate cell-cell and cell-extracellular-matrix adhesive connections. There is an extensive literature on their importance in neural development and cancer, but evidence for the existence of integrins in the adult CNS has emerged only recently. With growing immunohistochemical and molecular biological evidence for the presence of integrins in the adult CNS, a variety of functions from microglial migration to synaptic rearrangements can be considered for these adhesive proteins.

TITLE: Methods for bounding the marginal survival distribution.
PUBDATE: 19950901
For time to event data with many potential failure types, one cannot uniquely determine the distribution of time to a specific event type, or marginal survival distribution, in the case where event types are mutually exclusive. In this paper we discuss several methods for estimating functions that bound the non-identifiable marginal survival distribution in the competing risks problem. We compute and compare bounds for data simulated from two bivariate survival distributions. Results show that the methods provide a suitable estimate of the marginal survival probability when one has specified dependence correctly. Data from a large clinical trial for breast cancer illustrate the methods.

The connections between cancer and the basic machinery of the cell cycle have taken a surprisingly long time to become apparent. However, the past 2 years has seen a dramatic increase in the number of cell cycles regulators that have been implicated as either protooncogenes or as tumour suppressor genes. In this review I will attempt to show how perturbations in the known cell cycle regulators may play a part in the process of oncogenesis.

Few diagnoses are feared more by patients than cancer. In addition to the life-threatening nature of many cancers, the spectre of very difficult the treatment provides patients with little or no level of comfort. However, recent advances in the treatment of the side effects of cancer chemotherapy are helping to change these attitudes. On April 4, 1996, a panel of experts was convened to discuss these issues and how they affect MCOs.

In a prospective study including 20 laparoscopic cholecystectomies, markers of the humoral (immunoglobuline) and cell mediated (lymphocytes and sub population T4/T8) have been assessed on 1st, 3nd and 8th post-operative days. There is no significant variation (except total lymphocyte count) within the limits of normality. This study suggests that laparoscopic surgery dont affect immunity. Laparoscopy seems to be preferable for immunodepressed patients (AIDS, cancer).

Up to stage B1 or prostate cancer with N0-1 positive lymph node may be curable by monotherapy of either radical prostatectomy or irradiation. Localized but more advanced stages (B2 and C), however, are difficult to cure. To further improve the prognosis in these stages, a new treatment modality is proposed; pelvic lymphadenectomy, followed by irradiation with neoadjuvant and adjuvant endocrine therapy. An average 2-year followup is favorable. Further studies is awaited to make conclusion.

PUBDATE: 19970801
Treatment of patients with unresectable stage IIIA and IIIB non-small cell lung cancer with conventional radiation therapy has resulted in poor local control, a high rate of systemic failure, and 5-year survival of less than 10%. Recent modification in radiation treatment planning, dose delivery, and fractionization has demonstrated significantly improved median and 2-year survival, comparable in some trials to that achieved with induction chemotherapy followed by radiation. Current trials focus on combining systemic therapy with dose-intense radiation therapy to improve with local and systemic control.

PUBDATE: 19940501
The authors made an exact investigation of the treatment of patients suffering from cervical cancer during one year period. Out of the data of 101 gynecological wards 1700 cases were evaluated. The authors evaluated the different ways of treatment with respect to the international requirements on the base of hystological results. They elaborated the operative methods in every stage of cervical cancers, with more details on radical hysterectomy and regional lymphadenectomy. The authors suggest that patients suffering from cervical cancer should be treated by standardized principles based on international scientific experiences.
