TITLE: [The prevalence of idiopathic proctocolitis]

During screening of colorectal cancer by the Haemoccult test (Röhm Pharma Co.) the authors investigated the incidence of idiopathic proctocolitis in the population of the Bruntál district. The group of tested subjects comprises 16.642 of the population aged 45 to 86 years. After examination of 380 positive subjects (2.28%) twelve cases of idiopathic proctocolitis were detected. Of these twelve seven were identified previously and a new diagnosis was made in five subjects. At twelve cases were identified from a population of 16.642, the assumed prevalence in 1989 in the given age group is 72/100.000. This suggests considerable underestimation of the prevalence of idiopathic proctocolitis in current epidemiological surveys.

The medical hazards of obesity are discussed. Risks include insulin resistance, diabetes mellitus, hypertriglyceridemia, decreased levels of high-density lipoprotein cholesterol, and increased levels of low-density lipoprotein cholesterol. Obesity is also associated with gallbladder disease and some forms of cancer as well as sleep apnea, chronic hypoxia and hypercapnia, and degenerative joint disease. Obesity is an independent risk factor for death from coronary heart disease. A central distribution of body fat enhances the risk for most of these conditions.

We recently diagnosed 65 cases with tracheobronchial tuberculosis using fiberbronchoscope. All the patients presented TB symptoms and were smear positive, some of them had been misdiagnosed for chronic bronchitis or lung cancer. Among them, 16 cases were simple tracheobronchial tuberculosis (24.6%) without TB lesions in the lungs. The results show that fiberbronchoscopic examination is a good method to diagnose tracheobronchial tuberculosis and distinguish lung cancer by direct observation of tracheobronchial lostons or brush biopsy for TB.

TITLE: Small-cell lung cancer in elderly patients: the case for chemotherapy.

We present a retrospective analysis of survival in 48 elderly (older than 65 years) and 70 young patients with small-cell lung cancer. Thirty-four elderly and 62 young patients had combination chemotherapy. Overall probability of survival with combination chemotherapy was similar for old and young patients (chi 2 = 0.79, d.f. = 1, p = 0.374). Peripheral neuropathy secondary to vincristine was more common with elderly patients. We believe that decisions about chemotherapy in elderly patients with small-cell lung cancer should be based on performance status, extent of disease and clinical condition rather than age.

Total pelvis radiation therapy as refined over the past 40 years has impacted positively on gynecologic cancer management. Improved overall results on a worldwide basis reflect a broader application of contemporary radiation treatment plans. Individual clinical stages of disease and subsets have not seen improvement in several decades. Currently available techniques for the safe application of extended field radiation therapy strongly support its routine use. Concomitant continuous infusion chemotherapy (CCIC) with radiation therapy (RT) should be strongly considered. Primary radical surgery is employed in our hands prior to extended RT and CCIC for most Stage I and II cases.

The polypeptide hormone, gastrin, is known to promote both the in vitro and in vivo growth of human gastric cancer. This proliferative activity has been shown to be mediated by high affinity, membrane-associated receptors. This has led to the development of agents with the ability to antagonise gastrin receptor binding, which have been evaluated for their potential clinical value. Other anti-gastrin therapies have been investigated. As gastrin may act as an autocrine mediator of gastric tumor cell proliferation, anti-secretory agents have been evaluated, as have agents which induce the production of neutralising anti-gastrin antibodies in situ.

The main human cancers are associated with complex life-style related causative, enhancing, and inhibiting factors. Tobacco smoking or chewing exposes humans to genotoxic carcinogens and to promoting substances. Likewise, Western dietary traditions involve certain carcinogens and promoters, whereas Oriental traditions implicate other carcinogens and promoters. Importantly, in virtually all situations regular intake of fruits and vegetables appreciably lowers the risk of cancer. This paper reviews the causes of the main human cancers and analyzes the mechanisms of the protective effects of fruits and vegetables. Prevention of human cancer requires the definition of optimal levels of recommended daily allowances of micronutrients.

Trifluoperazine (TFP) could inhibit the proliferation of A549 lung cancer cells and human embryo lung (HEL) cells. The inhibitory effects were dose dependent. The inhibitory effects were stronger in A549 lung cancer cells than in HEL cells at the same TFP dose. Cytoskeleton in A549 lung cancer cells treated with TFP was increased as observed electron microscopically. The role of calmodulin in the proliferation and the changes in cytoskeleton of A549 lung cancer cells is discussed.

A new method for drainage of ascites is presented: ultrasound guided intraperitoneal catheterization. Forty-six drainages were performed in 17 patients. The duration of drainage was from one to 12 days. Two patients had slight complications during drainage. Puncture was easily done even in cases of difficult or impossible conventional puncture. The method provided very effective drainage. Further, we present ultrasound guided intraperitoneal catheterization for chemotherapy in ovarian cancer. In eight patients 51 catheterizations were performed. Three punctures were primarily unsuccessful, but successful on the second attempt. There were no complications in these or in other cases.

We present a rare case of multiple gastric cancer where both tumors were advanced and represented different types of neoplasia under Lauren's classification. Both tumors where diagnosed preoperatively.
