TITLE: Induction of oral cancer by 7,12-dimethylbenz[a]anthracene in rats with liver cirrhosis.

The effects of carbon tetrachloride-induced liver cirrhosis and xerostomia on oral carcinogenesis were studied in rats given the lipid-soluble carcinogen 7,12-dimethylbenz-[a]anthracene (DMBA). The first carcinoma was detected after only 1 month of DMBA applications. After a further 8 months 85% of the animals had developed one or more squamous cell carcinomas. The latency period for DMBA-induced oral cancer in cirrhotic rats with xerostomia was markedly reduced in comparison with previous findings from xerostomic rats without liver cirrhosis. The results support earlier epidemiologic studies indicating a relationship between liver cirrhosis and oral cancer.

TITLE: Death from unsuspected colorectal cancer.

An analysis of all subjects undergoing autopsy in the Plymouth Health District over a 10-year period revealed 61 cases of unsuspected colorectal cancer. The carcinoma was the primary cause of death in 57 cases and surgical treatment for cure might have been possible in over one-half. The 61 subjects accounted for 4.1% of all cases of fatal colorectal cancer in this area. Examination by means of rectal digital palpation, rigid sigmoidoscopy and flexible sigmoidoscopy might have diagnosed 10, 24 and 41 carcinomas respectively. This study has demonstrated that a significant number of colorectal cancers remain undetected until after death. These cases should be included in future studies of colorectal cancer.

The study of the family of transmethylases, critical to normal cellular function and often altered in cancer, can be facilitated by the availability of a high specific-activity S-adenosylhomocysteine. We report the two-step preparation of [35S]adenosylhomocysteine from [35S]methionine at a specific activity of 1420 Ci/mmol in an overall yield of 24% by a procedure involving demethylation of the [35S]methionine to [35S]homocysteine followed by condensation with 5'-chloro-5'-deoxyadenosine. The ease of the reactions, ready availability and low cost of the reagents and high specific-activity and stability of the product make the procedure an attractive one with many uses, and superior to current methodology.

TITLE: [Consequences of after-care in colorectal cancer]

Results of a computer supported follow-up-program on patients with colorectal cancer are presented. Between 1978 and 1985 1024 patients underwent these program, the drop-out-rate was 18%. 231 recurrences in 137 patients were discovered (40% local recurrence, 30% liver metastases and 30% others). 54% of patients with local recurrence and 43% with liver metastases were free of symptoms. Radical surgery was performed in 47% of local recurrences and in 24% of liver metastases. The three year survival rate after radical surgery for recurrence amounted to 28% for local recurrences and to 31% for liver metastases.

A retrospective analysis of the survival and mean life rates of 107 patients with breast cancer metastases to the skeletal bones was performed. In non-randomized groups there was no significant correlation of long-term therapeutic results and the type of fractionation, a total radiation dose value, chemo- or hormonotherapy. Prognosis was better in patients in whom solitary metastases were detected 5 and more years after primary treatment. In such cases radiation therapy was indicated at doses sufficient for eradication or stable stabilization of a focus using the most sparing dose fractionation regimens.

TITLE: Observer error in the characterization of hepatic lesions in patients with neoplastic liver disease.

Sixteen patients with known neoplastic liver disease underwent 20 ultrasound examinations by two separate teams to determine the level of agreement in the measurement of lesion size and sonographic characteristics. The intraclass correlation coefficient for the observations on lesion size was r = 0.97 (95% lower confidence limit r = 0.96). We conclude that sonographic estimation of the size of neoplastic liver lesions in highly reproducible and that the measurements obtained may be safely incorporated into the criteria of response for cancer clinical trials.

The pS2 gene is transcriptionally induced by oestrogen in the human breast cancer cell line MCF-7. We demonstrate here that the 5' flanking sequences (-3000 to +10 bp) of the pS2 gene possess the properties of an oestrogen-inducible promoter. Interestingly, this oestrogen induction could not be demonstrated in transient transfection assays in MCF-7 cells, but only in stably transformed MCF-7 cells, which suggests that some factors responsible for oestrogen induction may be present in limiting amounts in these cells and absent in HeLa cells.

Effects of five amiloride analogues on thermal cell killing were examined using Chinese hamster V-79 cells. When cells were exposed to 42 degrees C hyperthermia in the presence of 0.2 mM amiloride or its analogue, cell survival decreased with increasing exposure time, as compared with that for exposure to 42 degrees C alone. The degree of the thermosensitizing effect varied among the test compounds, and three analogues were found to be more potent than amiloride. The results suggest that some of the amiloride analogues may be useful as hyperthermic sensitizers in the clinical treatment of cancer.

During 10 years from 1974 to 1986, 390 patients with breast cancer underwent radical mastectomy in our consisting of 12 local lymph node, 26 bone metastasis and 20 lung metastasis. Of 10,278 encounters with patients recorded by 33 general practitioners in a defined Norwegian population during a two month period, 429 (4.2%) were classified as resulting in a follow up for suspected cancer. The patient's fear of cancer was the most important indicator of suspicion of cancer among the doctors. Patients who consulted for digestive and gynaecological problems were most frequently suspected of having cancer. Comparing the pattern of suspicion of cancer with the incidence of cancer in the area showed a relative oversuspicion in young patients and women.

Sixteen non-pretreated patients with locally advanced cervical cancer (FIGO Stage IIIb) were treated with two 3-week cycles of platinum, bleomycin, and methotrexate (PBM), which were followed by radiation therapy. Response to both modalities was seen in 11 patients (69%), and three patients (19%) had a progression-free survival of more than 2 years. Radiation therapy was proven to be feasible after two cycles of PBM combination chemotherapy.

TITLE: Pancreatic cancer risk following gastric surgery.

To estimate the risk of pancreatic cancer after gastric surgery for benign disease, we followed all 336 residents of Olmsted County, Minnesota, who had surgical treatment for benign peptic ulcer disease during the years 1935 to 1959. These patients were followed for 6549 person-years of observation. Pancreatic cancer developed in only one patient although 1.6 primary pancreatic cancers were expected (relative risk 0.64; 95% confidence interval, 0.02 to 3.53) based on pancreatic cancer incidence rates for the general population of Olmsted County. We conclude that in our study the incidence of pancreatic cancer is not significantly increased after gastric surgery.

Correlation between c-myc gene expression and phosphatidylinositol (PI) metabolism was studied using seven human kidney cancer cell lines. We found that the exceptional incorporation of 32P into CDP-diacylglycerol (CDP-DG) was observed in only two cell lines, in which c-myc expression was increased among seven cell lines tested. In these two cell lines, PI was also labeled. The other five cell lines indicated the accumulation of radioactive PI, whereas CDP-DG was unlabeled.

Potentially useful modalities of pain control in pancreatic cancer include antitumor therapy, pharmacotherapy, celiac plexus block, splanchnic nerve block, intercostal nerve block, and psychological intervention. These modalities are often used concurrently in treating the multiple dimensions that affect pain. Although thorough assessments are lacking, preliminary data suggest that antitumor chemotherapy and radiotherapy and celiac plexus block are especially useful modalities of pain control in these patients. The optimal time in the disease course for intervention with celiac plexus block is not known. Further studies are needed to clarify the nature of pain syndromes involved and the role of the various therapeutic modalities.
