TITLE: Stage I vulvar cancer: criteria for microinvasion.

Thirty-eight women with stage I epidermoid carcinoma of the vulva were studied retrospectively in an attempt to define the criteria for conservative therapy in early invasive vulvar cancer. Among the 38 women, 23 met the 6 criteria established for early invasive carcinoma of the vulva. In those 23 women, there were no nodal metastases. Depth of stromal invasion correlated strongly with the degree of tumor differentiation and the presence of carcinoma in situ (CIS). Stromal invasion less than 3 mm and presence of CIS were predictive of no involvement of the lymph nodes or endothelial-like space.

In this work we intended: a) to confirm the correlation between the increase of serum levels of alpha 1-antitrypsin (ATT) and neoplastic disease; b) to verify in what tumoural diseases the increase of ATT has a specific significance. Therefore we have examined 164 patients: 60 represented the control group and 104 were suffering from neoplastic diseases. We have subdivided the nest group according to histological type and tumour location. The result of this work has demonstrated that the increase of ATT is really determined by the existence of neoplastic disease, more than histological type or location of cancer. The AAT represents a diagnostic index of neoplastic diseases, highly sensitive but little specific.

The cosmetic and functional disability associated with mandibular resection has been a major problem to the patient with direct invasion of the mandible by oral cancer. Marginal resections with combined postoperative radiation therapy have frequently been substituted for the more preferred segmental resections and resultant deformities. Presented are 15 cases of oral cavity cancer involving resection of the mandible, immediate radiation to 10,000 rad, and primary reconstruction as irradiated mandibular autografts. The longest following is 4 years and 3 months, with a success rate of 66%. Morbidity is minimal as compared to autogenous bone grafting. Tumor size, previous radiation, or use of regional flaps have not been a factor in the success of this method in reconstruction of the mandible primarily.

Preoperative transcatheter arterial chemo-embolization (TAC-E) was used for 19 patients with locally advanced breast cancer, to eradicate the extensive loco-regional areas prior to surgery and hopefully to reduce the postoperative recurrence rate. The results were as follows: (1) Each primary tumor showed marked regression within 2 weeks after TAC-E. (2) Surgicopathology confirmed the excellent efficacy of TAC-E against both primary tumors and metastatic lymph node tumors. (3) Side effects due to this treatment were few, compared with other forms of cancer chemotherapy. (4) The prognosis of each patient was good. We conclude that preoperative TAC-E has a great potential for the treatment of locally advanced breast cancer.

Changes in the pattern of disease managed by general surgeons in New Zealand, were assessed for the period 1940-80, using the public hospital admission and cancer incidence data of a representative selection of diseases. During the forty-year period, there was an increase in the incidence of four of the five cancers studied, and an increase in admission rate for non-specific abdominal pain, head injury (indicated by skull fracture), peripheral arterial disease (indicated by aortic aneurysm), gall-stone disease and pancreatitis, large bowel disorders and breast diseases. There was a decline in admission rate for appendicitis, stomach disorders and goitre.

Pancreatic oncofetal antigen (POA) was purified from fetal pancreas and migrated in beta-region electrophoretically. Its molecular weight was 80 X 10(4) daltons. Enzyme immunoassay for serum POA revealed that elevated levels of POA were found in sera of patients with pancreatic cancer. By a serological screening of 440 out-patients with combined assay of tumor markers including POA, 4 cases of pancreatic cancer were found. POA was demonstrated in the ductal cells of fetal pancreas and cancer cells of duct cell type pancreatic adenocarcinoma immunohistochemically. Elevated levels of serum POA of patients with pancreatic cancer was probably derived from cancer tissue. These findings indicate that serum POA assay is clinically useful.

Bone marrow aspirates and biopsy specimens were examined from seven patients with small cell lung cancer. All patients had received recent combination chemotherapy including VP-16. No correlation between marrow biopsy cellularity and hematologic toxicity could be established. However, an unusual combination of morphologic changes was seen. This included an initial rapid increase in the M:E ratio, interference with cell division, and eventual cell death. There was little evidence of mitotic arrest or megaloblastosis. These changes are consistent with the known mechanism of action of VP-16, the only agent given to all patients. The small sample size and the unknown contributions of the other cytotoxic agents administered allow only limited conclusions.

The human breast cancer cell line (T47D) has specific, high affinity calcitonin receptors and calcitonin-responsive adenylate cyclase. Human, salmon and [Asu1,7]eel calcitonin inhibited cell growth in a dose-related manner with almost equipotency. Analogues of human calcitonin demonstrated slight cell growth inhibition. We found extreme growth inhibition with daily treatment with dibutyryl cyclic AMP (10(-4) M). In contrast to calcitonin 1,25-(OH)2D3 had a biphasic effect on cell growth. Physiological doses (5 X 10(-10) M) of 1,25-(OH)2D3 stimulated growth of T47D, whereas treatment by supraphysiological amounts (2.5 X 10(-7) M) caused significant inhibition of growth. Calcitonin and 1,25-(OH)2D3 appeared to have additive effects.

TITLE: The effect of radiofrequency hyperthermia and chemotherapy upon human neoplasms when used with adjuvant metronidazole.

It appears that metronidazole causes an increase in tumor core temperature above that achieved with RFHT alone, by an as yet undetermined mechanism. Furthermore, radiofrequency hyperthermia, in combination with chemotherapy, can produce objective evidence of tumor repression in 30 to 50 per cent of the patients with metastatic cancer. These results justify proceeding to a randomized prospective trial of RFHT and chemotherapy in specific human neoplasms. Such a study is currently being undertaken at this center.

The radiograms of the chest and skeleton of 49 patients with advanced breast cancer treated with bilateral ovariectomy were reviewed. The modifications in the secondary localizations and/or their appearance after castration were compared with the findings of the clinical examination. Appearance or progression of intrathoracic lesions, like the appearance or progression of osteolytic lesions, corresponded to a progression of the disease in other sites. Osteoblastic evolution of osteolytic lesions and the appearance of osteoblastic lesions in bones undamaged before ovariectomy were signs of a favorable response to therapy. The response of chest and bone metastases is usually rather early, and the first radiographic survey should be performed about 3 months after ovariectomy.

This paper reports the development of a self-report instrument designed to assess pain in cancer and other diseases. It is argued that issues of reliability and validity should be considered for every pain questionnaire. Most research on measures of pain examine reliability to the relative neglect of validity concerns. The Wisconsin Brief Pain Questionnaire (BPQ) is evaluated with regard to both reliability and validity. Data from patients with cancer at 4 primary sites and from patients with rheumatoid arthritis suggest that the BPQ is sufficiently reliable and valid for research purposes. Additional methodological and theoretical issues related to validity are discussed, and the need for continuing evaluation of the BPQ and other measures of clinical pain is stressed.

Chromosome abnormalities were analyzed in 200 consecutive patients with preleukemia and leukemia, and four patients were found with a deletion of 13q14 for an incidence of 2%. Together with data on chromosome aberrations in cancer from the literature, our results indicate clearly that deletion of band 13q14 is a nonrandom chromosome anomaly in premalignant and malignant blood disorders. Deletion of 13q14 appears specifically to constitute a primary event in the initiation of preleukemia. An additional rearrangement involving another chromosome must occur for progression of the preleukemia to acute nonlymphocytic leukemia.

The authors make an analysis of the operative treatment of 525 patients with cancer of the hepato-biliary and pancreatoduodenal zones. The distal, central and proximal obturations of bile ducts were found which determined the indications to formation of various bilio-digestive anastomoses. The operation of choice for distal obturation is a wide areflux cholecystogastrostomy with the use of blanket suture with the chromium-coated catgut. In the central and proximal obturation the precision suture with an atraumatic suture material and magnifying optics are recommended.

A high frequency radiator has been developed for the treatment of prostate cancer by hyperthermia. The applicator produces a deep seated hot spot. The radiator has an outer diameter of 20 mm and an insertable length of about 175 mm. A high frequency cylindrical slot antenna inside the applicator is cooled by water. The frequency used is 433.9 MHz, one of the frequencies allowed for medical therapy in the F.R.G. A control system regulating the power output of the radiator avoids damage to the tissue around the prostate, especially the rectum mucosa and the tissue between rectum and prostate. About 60 experiments with male dogs proved that local heating of the prostate is possible with the help of the developed applicator without any damage in the surrounding tissue.
