TITLE: Sensitization of resistant myeloid leukemia clone cells by anti-cancer drugs to factor-stimulating differentiation.

Studies were made on the effect of cancer chemotherapeutic drugs on in vitro differentiation of a clone (R4) of mouse myeloid leukemic cells (MI) that is resistant to inducers. Treatment of the cells with 50% ascitic fluid (an inducer) plus 0.25 microgram/ml of adriamycin or 0.3 microgram/ml of daunomycin induced phagocytic activity and suppressed cell growth, but had little effect on cell viability; treatment with ascitic fluid or the drugs alone had no effect. In combination with ascitic fluid, mitomycin-C, hydroxyurea, 5-fluorouracil, or bleomycin also induced phagocytic activity, but 6-mercaptopurine, amethopterin, or aminopterin did not. These drugs also induced other differentiation-associated properties, lysozyme activity, and locomotive activity. The present results indicate that some cancer chemotherapeutic drugs sensitive resistant leukemic cells to an inducer of cell differentiation.

Postdischarge progress in 11 types of patients was analyzed with regard to eating, dressing, transfers, bladder management, ambulation, cognition, language comprehension and speech, placement (living arrangement), and mortality rates. Of 273 patients admitted, 263 were available for foillow-up, which disclosed that patients with lower bladder management and cognition scores are more likely to be directed into restrictive discharge placements (p < 0.05), and that CVA patients with right and left cerebral hemisphere involvement, cancer patients, and patients with brain stem infarctions make significant postdischarge gains in ADL (p < 0.05). The majority of patients showed continual improvement after discharge, some making significant gains in functional competency. Such program evaluation offers a systematic method for forecasting post-discharge outcome.

The incidence of allergic reactions to bovine TSH and its relation to dose have been studied in 42 patients with thyroid cancer who had total ablation of functioning thyroidal tissue and have been followed prospectively for up to 25 yr. Of these patients, 43% showed an allergic reaction to bovine TSH. The remaining 57% have shown no reaction, even though they have received cumulative TSH doses larger than those who did show a reaction. The reaction rate is linear up to a cumulative dose of 150 units and is not dose-related therafter.

The minimum requirements of the International Union against Cancer (UICC) to assess the TNM-classification of urothelial bladder cancer include since 1978 the clinical examination, urography, cystoscopy, bimanual palpation under anesthesia and biopsy or transurethral resection of the tumor. The single biopsy of the tumor should be supplemented by multiple biopsies of all bladder walls in combination with the resection of the tumor to diagnose multifocal tumor growth. The exfoliative urinary cytology should be included in the routine diagnostic regimen: cytology is helpful in detection of severe dysplasia and controls the histologically determined grade of the tumor. In selected cases computer tomography improves the accuracy of clinical evaluation of the depth of tumor infiltration. In our hands the lymphangiography can not assess the regional lymph node metastases: therefore, the staging operation is recommended before radical cystectomy is performed.

In the field of research on cancer we studied 294 women of which 142 who had undergone mastectomy, 70 selected for a high risk group, 82 normal. In order to determine some parameters who characterize the three groups we research for each the globulin factor Gc, Gm, and Inv, the proteic fractions and the immunoglobulins, as already pointed out by other investigators. The study of data points out results some times not according to previous reports. Comparing people who had undergone mastectomy and who had been selected for a high risk group, on the bases of GC phenotype together with immunoglobulins we obtained interesting values that characterize and delimit the two groups.

A fibrorectosigmoidoscope, 60 cm in length has been systematically applied in the endoscopy unit to the exploration of the distal colon. Technical parameters have been analysed in 300 patients: --immediate enema preparation was satisfactory (95% of examinations), --tolerance was good (96% of examinations), --mean length of colon explored reached 48,5 cm, --mean length of duration reached 5,15 mn. Efficacy in the detection of colonic lesions was tested in 476 patients (symptomatic or asymptomatic). Tumoral lesions were detected in 105 patients, with a total of 125 adenomas and 31 cancers; 71 lesions were found in the area that could be explored by a rigid rectosigmoidoscope (0-19 cm) while 85 were beyond this limit (20-60 cm). The fibre endoscope explores a larger territory (rectum + sigmoid), resulting in increased efficacy in the detection of colonic lesions. It should therefore replace progressively the rigid instrument in spite of its higher cost.

From 1968 to 1978, 125 patients with cancer of the tonsil have been treated by the Departments of Radiology and Otolaryngology at the University Hospital, Homburg-Saar. Although all patients were unirradiated, many had previously undergone a surgical procedure. Most of the cancers when seen were in advanced stages. The three-year and five-year survival rates were 38.6 and 23.5% respectively. Reticulum cell sarcomas, Schmincke's tumors and non-keratinized squamous cell carcinomas had better prognoses than keratinized squamous cell carcinomas. Our findings indicate that the volume of large tumors should be reduced by surgical means before irradiation since irradiation alone resulted in a higher relapse rate in the advanced cancers. Various methods for radiotherapy either before or after surgery for different tumor stages are discussed. The combination of chemotherapy with radiation to make use of the so-called "partial synchronisation effect" in cases of inoperable extensive primary tumors or relapses is described.

The incidence of cancer was compared in 499 dialysis patients and 121 renal transplant recipients. De novo malignancy developed in 15 patients on chronic dialysis (3 per cent) and in 6 transplant recipients (4.9 per cent), a significant increase over the expected number in the age-matched general population. There was no difference in the incidence of cancer in uremic patients on dialysis or after transplantation. A total of 10 dialysis patients (67 per cent) and 1 transplant patient (16 per cent) died of cancer. Neoplasms in the dialysis patients were the common types of mesenchymal tumors, while superficial skin cancers were seen more frequently in the transplant recipients. The differences in tumor types accounted for the higher mortality rate from cancer in the dialysis patients and may reflect different patterns of immunosuppression in these 2 patient populations.

Gastrointestinal metastases secondary to bronchogenic carcinoma are relatively uncommon and most are found incidentally at autopsy examination in patients with advanced or widely disseminated lung cancer. Occasionally gastrointestinal metastases occurr relatively early in the course of the disease and give rise to a variety of clinical symptoms and radiological abnormalities. Recognition of these abnormalities is important in order that appropriate palliative therapy may be undertaken. The clinical. radiological and pathological findings in 12 patients with symptomatic gastrointestinal metastases secondary to bronchogenic carcinoma were reviewed. Clinical symptoms varied according to the site of metastatic involvement and included dysphagia, epigastric pain, nausea, vomiting, gastrointestinal bleeding, anaemia and signs of intestinal obstruction or perforation. The sites of metastatic involvement were: oesphagogastric junction (2 cases); stomach (2 cases); duodenum (1 case): jejunum (3 cases); ileum (2 cases), colon (2 cases). The radiological findings are discussed and illustrated.

This study was designed to provide a perspective of intraoral cancer, as it occurs in Victoria, by reference to the Central Cancer Registry of Victoria. Intraoral cancer was found to be a disease of males in the 50 to 69 years' age group. There was a low survival rate which appeared to be related to the aggressive nature of the disease and to the delays which often occurred before treatment was commenced. Therefore, a strong recommendation is made that clinicians, both medical and dental, increase their level of suspicion with regard to lesions of the oral mucous membrane in order to detect oral cancer at an early stage, and thereby to avoid the certain distress to patients which accompanies the management of advanced carcinoma of the oral mucous membrane.

Alcohol combined with tobacco smoking is an established risk factor for cancers of the oropharynx, esophagus, and larynx. It should be possible to clarify further the role of alcohol itself, the modifying effects of tobacco, dose-response relationships, and nutritional cofactors. Studies are also needed to delineate the steps by which alcohol consumption leads to liver cancer and to resolve the suggestion that certain beverages may predispose to other cancers including those of the pancreas and rectum. Epidemiological investigations should be combined with experimental work to identify hazardous fractions in alcoholic beverages and to delineate the mechanisms by which alcohol promotes carcinogenesis. Epidemiologists and biometricians may also contribute toward the development of programs aimed at primary prevention and early detection of cancers related to alcohol and tobacco. Incorporation of research questions into data collection systems deserves serious questions into data collection systems deserves serious consideration as a means of obtaining additional valuable information for etiological studies.

Activity was destroyed by some enzymes, and the apparent molecular size of the active material was reduced by others. An active low-mol.-wt fragment has been partially purified from papain digests of several tumours. Peptides with normal tissue and tumour-characteristic activities have been separated chromatographically from tryptic digests of tumour extracts.

Eleven married couples having histologically identical malignant tumors have been encountered: two couples with melanoma; three with carcinoma of the breast; two with carcinoma of the kidney, and one couple each with fibrosarcoma; carcinoma of the floor of the mouth; carcinoma of the colon and rectum, and carcinoma of the nasopharynx. The interval until the spouse of the patient with cancer was diagnosed to have cancer was within five years in all but three instances. The clinical course of the disease tended to be similar in both husband and wife. Generally, the longer the couple had been married, the more successful was their treatment. Those tumors described, as well as those identical cancers of husband and wife previously reported, have been suggested either experimentally or indirectly to have viral relationships.

Radiotherapy provides a high cure rate for patients with stages I and II seminoma but the results for those with stage III disease have been disappointing. We report on 19 patients with high stage seminoma who have been treated with platinum, vinblastine and bleomycin (14 patients) or platinum, vinblastine, bleomycin and doxorubicin hydrochloride (5 patients). Of the 19 patients 12 (63 per cent) had complete remission of the disease and the remaining 7 had partial remission. To date, 11 patients (58 per cent) remain free of disease. Of the 6 patients with no prior irradiation 5 achieved complete remission, while the 1 failure had massive hepatic metastases. The median followup was 19 months (range 12 to 37 months). These results parallel that achieved with the same chemotherapy regimen in patients with stage III nonseminomatous testicular cancer.

Chronic ingestion of modest doses of dietary iodine, radiation, and polyhalogenated biphenyls (PCB's and PBB's) are environmental factors with known or suspected adverse effects on the human thyroid. Iodine consumption in the United States is approaching 1 mg daily for a large segment of the population. Data are reviewed which support the need for concern regarding the long-term adverse effects of dietary iodine on thyroid function, particularly in certain susceptible individuals. Environmental sources of radiation pose a significant risk of thyroid cancer and hypothyroidism under certain circumstances which may be intentional, inadvertent, or accidental. Exposure to polyhalogenated biphenyls during manufacture or as industrial pollutants are hazardous to man and to wildlife in moderate or large quantities and perhaps also in small amounts. The need to investigate the potential harm posed by these factors in the quantities commonly encountered is emphasized.
