TITLE: [The prevalence of idiopathic proctocolitis]

During screening of colorectal cancer by the Haemoccult test (Röhm Pharma Co.) the authors investigated the incidence of idiopathic proctocolitis in the population of the Bruntál district. The group of tested subjects comprises 16.642 of the population aged 45 to 86 years. After examination of 380 positive subjects (2.28%) twelve cases of idiopathic proctocolitis were detected. Of these twelve seven were identified previously and a new diagnosis was made in five subjects. At twelve cases were identified from a population of 16.642, the assumed prevalence in 1989 in the given age group is 72/100.000. This suggests considerable underestimation of the prevalence of idiopathic proctocolitis in current epidemiological surveys.

The medical hazards of obesity are discussed. Risks include insulin resistance, diabetes mellitus, hypertriglyceridemia, decreased levels of high-density lipoprotein cholesterol, and increased levels of low-density lipoprotein cholesterol. Obesity is also associated with gallbladder disease and some forms of cancer as well as sleep apnea, chronic hypoxia and hypercapnia, and degenerative joint disease. Obesity is an independent risk factor for death from coronary heart disease. A central distribution of body fat enhances the risk for most of these conditions.

TITLE: [A clinical study on 65 cases with tracheobronchial tuberculosis]

We recently diagnosed 65 cases with tracheobronchial tuberculosis using fiberbronchoscope. All the patients presented TB symptoms and were smear positive, some of them had been misdiagnosed for chronic bronchitis or lung cancer. Among them, 16 cases were simple tracheobronchial tuberculosis (24.6%) without TB lesions in the lungs. The results show that fiberbronchoscopic examination is a good method to diagnose tracheobronchial tuberculosis and distinguish lung cancer by direct observation of tracheobronchial lostons or brush biopsy for TB.

Several clinical trials of prostate cancer gene therapy are underway. The therapeutic genes being evaluated by these and other, forthcoming, trials include immunogenes, cell death-inducing genes, antioncogenes, and tumor suppressor genes. Constraints of current gene transfer vectors limit, to local-regional targeting, those gene therapy strategies that do not rely on an intervening immune response. Preclinical models predict modest, if any, therapeutic effects with current forms of prostate cancer gene therapy. Using biological surrogate endpoints and applying further preclinical advances will allow clinical prostate cancer gene therapy to attain its full potential.

A case of adenoid cystic carcinoma (ACC) of the breast in a 71-year-old woman is reported. This neoplasm accounts for about 0.1% of all breast cancers. The mammography showed a well-delineated mass without calcifications. Cytologic examination of percutaneous fine-needle aspiration (FNA) material gave the diagnostic of ACC, which was confirmed by histologic examination. This report emphasizes the utility of immunocytochemical study with collagen IV antibody on FNA material of this uncommon tumor.

TITLE: Video-assisted right lower lobectomy for a lung cancer with mini-thoracotomy.

We report a case of T1N0M0 lung cancer in which we successfully performed video-assisted right lower lobectomy and mediastinal lymph node dissection. The lobectomy was done on a 64-year-old woman who had a 2-cm adenocarcinoma in the right lower lobe. The procedure could be done safely and adequately using three trocars and a 5-cm mini-thoracotomy. The patient had an uncomplicated postoperative course and was satisfied with minimal postoperative pain, quick recovery, and minute skin scars. Video-assisted lobectomy may be a reasonable approach in selected patients of primary lung carcinoma.

Three pivotal phase III trials conducted in North America and Europe served as the basis for the application for approval of toremifene (Fareston) by the FDA. These trials demonstrated that 60 mg/d of toremifene is safe and effective in the treatment of advanced breast cancer in postmenopausal women. The studies also indicated that, on the basis of antitumor efficacy, as well as safety, toremifene is at least equivalent to tamoxifen and may have some long-term advantages.

We present a retrospective analysis of survival in 48 elderly (older than 65 years) and 70 young patients with small-cell lung cancer. Thirty-four elderly and 62 young patients had combination chemotherapy. Overall probability of survival with combination chemotherapy was similar for old and young patients (chi 2 = 0.79, d.f. = 1, p = 0.374). Peripheral neuropathy secondary to vincristine was more common with elderly patients. We believe that decisions about chemotherapy in elderly patients with small-cell lung cancer should be based on performance status, extent of disease and clinical condition rather than age.

For time to event data with many potential failure types, one cannot uniquely determine the distribution of time to a specific event type, or marginal survival distribution, in the case where event types are mutually exclusive. In this paper we discuss several methods for estimating functions that bound the non-identifiable marginal survival distribution in the competing risks problem. We compute and compare bounds for data simulated from two bivariate survival distributions. Results show that the methods provide a suitable estimate of the marginal survival probability when one has specified dependence correctly. Data from a large clinical trial for breast cancer illustrate the methods.
