TITLE: Stage I vulvar cancer: criteria for microinvasion.

Thirty-eight women with stage I epidermoid carcinoma of the vulva were studied retrospectively in an attempt to define the criteria for conservative therapy in early invasive vulvar cancer. Among the 38 women, 23 met the 6 criteria established for early invasive carcinoma of the vulva. In those 23 women, there were no nodal metastases. Depth of stromal invasion correlated strongly with the degree of tumor differentiation and the presence of carcinoma in situ (CIS). Stromal invasion less than 3 mm and presence of CIS were predictive of no involvement of the lymph nodes or endothelial-like space.

In this work we intended: a) to confirm the correlation between the increase of serum levels of alpha 1-antitrypsin (ATT) and neoplastic disease; b) to verify in what tumoural diseases the increase of ATT has a specific significance. Therefore we have examined 164 patients: 60 represented the control group and 104 were suffering from neoplastic diseases. We have subdivided the nest group according to histological type and tumour location. The result of this work has demonstrated that the increase of ATT is really determined by the existence of neoplastic disease, more than histological type or location of cancer. The AAT represents a diagnostic index of neoplastic diseases, highly sensitive but little specific.

The effects of carbon tetrachloride-induced liver cirrhosis and xerostomia on oral carcinogenesis were studied in rats given the lipid-soluble carcinogen 7,12-dimethylbenz-[a]anthracene (DMBA). The first carcinoma was detected after only 1 month of DMBA applications. After a further 8 months 85% of the animals had developed one or more squamous cell carcinomas. The latency period for DMBA-induced oral cancer in cirrhotic rats with xerostomia was markedly reduced in comparison with previous findings from xerostomic rats without liver cirrhosis. The results support earlier epidemiologic studies indicating a relationship between liver cirrhosis and oral cancer.

The cosmetic and functional disability associated with mandibular resection has been a major problem to the patient with direct invasion of the mandible by oral cancer. Marginal resections with combined postoperative radiation therapy have frequently been substituted for the more preferred segmental resections and resultant deformities. Presented are 15 cases of oral cavity cancer involving resection of the mandible, immediate radiation to 10,000 rad, and primary reconstruction as irradiated mandibular autografts. The longest following is 4 years and 3 months, with a success rate of 66%. Morbidity is minimal as compared to autogenous bone grafting. Tumor size, previous radiation, or use of regional flaps have not been a factor in the success of this method in reconstruction of the mandible primarily.

TITLE: Death from unsuspected colorectal cancer.

An analysis of all subjects undergoing autopsy in the Plymouth Health District over a 10-year period revealed 61 cases of unsuspected colorectal cancer. The carcinoma was the primary cause of death in 57 cases and surgical treatment for cure might have been possible in over one-half. The 61 subjects accounted for 4.1% of all cases of fatal colorectal cancer in this area. Examination by means of rectal digital palpation, rigid sigmoidoscopy and flexible sigmoidoscopy might have diagnosed 10, 24 and 41 carcinomas respectively. This study has demonstrated that a significant number of colorectal cancers remain undetected until after death. These cases should be included in future studies of colorectal cancer.

The study of the family of transmethylases, critical to normal cellular function and often altered in cancer, can be facilitated by the availability of a high specific-activity S-adenosylhomocysteine. We report the two-step preparation of [35S]adenosylhomocysteine from [35S]methionine at a specific activity of 1420 Ci/mmol in an overall yield of 24% by a procedure involving demethylation of the [35S]methionine to [35S]homocysteine followed by condensation with 5'-chloro-5'-deoxyadenosine. The ease of the reactions, ready availability and low cost of the reagents and high specific-activity and stability of the product make the procedure an attractive one with many uses, and superior to current methodology.

TITLE: [Consequences of after-care in colorectal cancer]

Results of a computer supported follow-up-program on patients with colorectal cancer are presented. Between 1978 and 1985 1024 patients underwent these program, the drop-out-rate was 18%. 231 recurrences in 137 patients were discovered (40% local recurrence, 30% liver metastases and 30% others). 54% of patients with local recurrence and 43% with liver metastases were free of symptoms. Radical surgery was performed in 47% of local recurrences and in 24% of liver metastases. The three year survival rate after radical surgery for recurrence amounted to 28% for local recurrences and to 31% for liver metastases.

Fifty-nine postmenopausal women with advanced breast cancer were treated with tamoxifen (antiestrogen), 20 mg orally twice a day for at least 2 months. They had been previously treated with other types of hormonal therapy or intensive chemotherapies, or both. Nineteen of the 59 patients (32%) had either a complete response (seven patients) or partial response (12 patients). The median duration of response was 9+ months. Tumors containing estrogen receptors and those that responded to previous hormonal manipulation tended to respond to tamoxifen (60% and 69%, respectively). Patients with receptor-negative tumor or with a history of failure of previous hormonal treatments did not respond to tamoxifen therapy. Tamoxifen is effective against advanced breast cancer. Side effects of the treatment were mild.

A retrospective analysis of the survival and mean life rates of 107 patients with breast cancer metastases to the skeletal bones was performed. In non-randomized groups there was no significant correlation of long-term therapeutic results and the type of fractionation, a total radiation dose value, chemo- or hormonotherapy. Prognosis was better in patients in whom solitary metastases were detected 5 and more years after primary treatment. In such cases radiation therapy was indicated at doses sufficient for eradication or stable stabilization of a focus using the most sparing dose fractionation regimens.

Studies were made on the effect of cancer chemotherapeutic drugs on in vitro differentiation of a clone (R4) of mouse myeloid leukemic cells (MI) that is resistant to inducers. Treatment of the cells with 50% ascitic fluid (an inducer) plus 0.25 microgram/ml of adriamycin or 0.3 microgram/ml of daunomycin induced phagocytic activity and suppressed cell growth, but had little effect on cell viability; treatment with ascitic fluid or the drugs alone had no effect. In combination with ascitic fluid, mitomycin-C, hydroxyurea, 5-fluorouracil, or bleomycin also induced phagocytic activity, but 6-mercaptopurine, amethopterin, or aminopterin did not. These drugs also induced other differentiation-associated properties, lysozyme activity, and locomotive activity. The present results indicate that some cancer chemotherapeutic drugs sensitive resistant leukemic cells to an inducer of cell differentiation.

Postdischarge progress in 11 types of patients was analyzed with regard to eating, dressing, transfers, bladder management, ambulation, cognition, language comprehension and speech, placement (living arrangement), and mortality rates. Of 273 patients admitted, 263 were available for foillow-up, which disclosed that patients with lower bladder management and cognition scores are more likely to be directed into restrictive discharge placements (p < 0.05), and that CVA patients with right and left cerebral hemisphere involvement, cancer patients, and patients with brain stem infarctions make significant postdischarge gains in ADL (p < 0.05). The majority of patients showed continual improvement after discharge, some making significant gains in functional competency. Such program evaluation offers a systematic method for forecasting post-discharge outcome.

Preoperative transcatheter arterial chemo-embolization (TAC-E) was used for 19 patients with locally advanced breast cancer, to eradicate the extensive loco-regional areas prior to surgery and hopefully to reduce the postoperative recurrence rate. The results were as follows: (1) Each primary tumor showed marked regression within 2 weeks after TAC-E. (2) Surgicopathology confirmed the excellent efficacy of TAC-E against both primary tumors and metastatic lymph node tumors. (3) Side effects due to this treatment were few, compared with other forms of cancer chemotherapy. (4) The prognosis of each patient was good. We conclude that preoperative TAC-E has a great potential for the treatment of locally advanced breast cancer.

Changes in the pattern of disease managed by general surgeons in New Zealand, were assessed for the period 1940-80, using the public hospital admission and cancer incidence data of a representative selection of diseases. During the forty-year period, there was an increase in the incidence of four of the five cancers studied, and an increase in admission rate for non-specific abdominal pain, head injury (indicated by skull fracture), peripheral arterial disease (indicated by aortic aneurysm), gall-stone disease and pancreatitis, large bowel disorders and breast diseases. There was a decline in admission rate for appendicitis, stomach disorders and goitre.

The incidence of allergic reactions to bovine TSH and its relation to dose have been studied in 42 patients with thyroid cancer who had total ablation of functioning thyroidal tissue and have been followed prospectively for up to 25 yr. Of these patients, 43% showed an allergic reaction to bovine TSH. The remaining 57% have shown no reaction, even though they have received cumulative TSH doses larger than those who did show a reaction. The reaction rate is linear up to a cumulative dose of 150 units and is not dose-related therafter.
