TITLE: [Breast carcinoma in women younger than 30 years]

Breast cancer is rare in females younger than 30 years. In a retrospective study of 25 women the effects of the early age on diagnosis, treatment and prognosis were investigated. In 21 women there was a clinical or radiological suspicion of carcinoma. Nevertheless, a delay in diagnosis was found in 9 of the 21 patients. In 16 patients stage I tumour was found, in 7 stage II, in I stage III and in I stage IV. After an average follow-up period of 68 months, 20 patients had no tumour recurrence. Tumour stage, treatment and prognosis were comparable with the characteristics reported for other age groups.

Treatment in case of a rom prognosis is a combination of supportive care, orthopedic surgery for pathologic fracture or medullar compression, antalgic radiotherapy, embolization, nephrectomy for hematuria, and analgesic treatments.

41 male patients with breast cancer confirmed pathologically were treated. Of these, 35 were treated by radical mastectomy and 6 by modified radical mastectomy. The patients with axillary lymphatic metastasis were treated by postoperative radiotherapy, chemotherapy. The overall 5-, 10- and 15-year survival rates were 67.6%, 53.3% and 55.0%, respectively. The 15 year survival rates of TNM stage I-II and III patients were 55% and 50% respectively. We conclude that radical or modified radical mastectomy is the rational therapy for male breast cancer. The patients in stage III will benefit from postoperative adjuvant therapy in terms of improving prognosis.

Twenty one patients age ranging from 26-70 years, with biopsy proven recurrent/advanced carcinoma of the cervix were treated with bleomycin, cisplatinum and ifosfamide infusion therapy. 88% patients reported subjective improvement. The objective response rate was 66.6% (Complete 38% and partial 28.5%). Side effects were mainly nausea, vomiting, and alopecia. Treatment related fever was common. Hematuria and reversible encephalopathy with ifosfamide were seen in three and two patients respectively. These results indicate that BIP infusion chemotherapy is an active and safe combination for treatment of advanced/recurrent cervical cancer.

Twenty-five patients, ranging from 21 to 61 years of age (median = 45 years), with histologically proven recurrent and advanced cervical cancer were treated with chemotherapy using a combination of bleomycin, ifosfamide, and cis-platinum (BIP). Twenty-one patients were evaluable for response. Ninety percent of patients achieved a subjective response. An objective response was noted in 14 of 21 (66.6%) patients: complete in 4 (19%) and partial in 10 (47.6%). Side effects were mainly nausea/vomiting, alopecia, myelosuppression, reversible encephalopathy, and impaired renal function. One patient died from the toxic effects of chemotherapy. These results indicate that BIP is an active combination in recurrent cervical cancer with acceptable toxicity.

Phosphorus-32, employed as the orthophosphate or polyphosphate, can reduce or relieve the pain of osteoblastic metastases without serious hematologic toxicity, especially if used as a single injection. Uptake of this beta-emitter by osteoblastic-reactive bone and possibly by tumor and other cells can lead to pain reduction and often to cell killing. Efficacy has been demonstrated for the treatment of pain in 84% of 322 breast cancer patients and 77% of 444 prostate cancer patients found in a review of the literature. These results match those of the newer radiopharmaceuticals currently under investigation.

The human cyclin D1 gene, also known as PRAD1 (and previously as D11S287), is a putative proto-oncogene strongly implicated in several types of human tumors, including parathyroid adenomas, B-cell neoplasms (as the 'BCL-1 oncogene'), and breast and squamous cell cancers. The mechanism by which deregulated production of cyclin D1/PRAD1, and perhaps other D-type cyclins, contributes to tumor development is only beginning to be deciphered.

In 1990 we applied to the 14 area health board ethical committees for approval of a national study of childhood cancer. All the committees approved the study, but the process was time consuming and costly. The times taken for the committees to grant approval, ranged from two to 36 weeks. The committees had differing requirements. We recommend that a single ethical committee should take responsibility for dealing with each multicentre study in New Zealand. This would ensure uniformity of approach, and minimise delays and costs.

The chest film is one of the most common methods of detecting lung cancer, which often appears as solitary nodules or hilar masses. Once the diagnosis is established, the most effective treatment can be determined and staging of the tumor can be done. Surgical resection is the only cure for non-small-cell carcinoma, although radiotherapy and chemotherapy may shrink some tumors. Because 90% of all cases are caused by smoking, a preventive approach to the disease is crucial. Screening of high-risk patients (ie, smokers with airflow obstruction) using chest films and sputum cytology may aid this approach.

Medical histories for the years 1978 to 1988 were reviewed to ascertain associations between colorectal cancers and extraintestinal malignancies. 61 out of 838 patients (7.3%) had an extraintestinal malignancy before the colorectal cancer. In males the rate was 5.4% and in females 10.4%. In males a high association was shown between cancers of the large bowel and urogenital cancers. In females colorectal cancer was associated with breast and gynecologic cancers. We conclude that patients with gynecologic and urologic tumors have an increased risk to develop cancers of the large bowel and that these high risk patients should undergo long-term follow-ups.

TITLE: Mechanical ventilation in cancer patients. Analysis of clinical data and outcome.

Between 1984 and 1988, 49 medically treated cancer patients were ventilated in the intensive care unit (ICU) of our hospitals. The charts of these patients were analyzed retrospectively. Sixty-seven percent of patients died during ventilation; 24% were able to leave hospital. The remaining 9% were transferred from the ICU to the ward, but died shortly thereafter in hospital. Assessment of the severity of acute disease prior to intubation by means of the organ failure score or the APACHE-II score was strongly associated with outcome after ventilation.
