TITLE: Ranking possible carcinogenic hazards.

This review discusses reasons why animal cancer tests cannot be used to predict absolute human risks. Such tests, however, may be used to indicate that some chemicals might be of greater concern than others. Possible hazards to humans from a variety of rodent carcinogens are ranked by an index that relates the potency of each carcinogen in rodents to the exposure in humans. This ranking suggests that carcinogenic hazards from current levels of pesticide residues or water pollution are likely to be of minimal concern relative to the background levels of natural substances, though one cannot say whether these natural exposures are likely to be of major or minor importance.

TITLE: Length polymorphisms of the human Y chromosome in patients with chronic myelogenous leukemia.

The extent of length heteromorphisms of the human Y chromosomes was measured in 50 patients with chronic myelogenous leukemia and was compared with 50 normal controls. No significant difference (p greater than 0.05) in the size of the Y chromosome was noted between these two groups. These findings differ from an earlier investigation, where a longer heterochromatic segment of Y chromosome was noted in malignant tumors and it was stressed that constitutive heterochromatin plays a role in susceptibility of cancer. Present investigation, however, suggests that this is not the case in patients with chronic myelogenous leukemia.

Throughout 1970-1975, 881 patients were treated at Departments I and II of Obstetrics and Gynecology, Helsinki University Central Hospital. They were treated mainly by using surgery and/or radiotherapy combined with adjuvant progestin therapy. The over-all uncorrected 5-year survival rate was 72.2% and corrected rate was 82.1%. In 146 out of 245 cases death was due to endometrial cancer, and this group was analyzed separately.

TITLE: Menstrual influence on surgical cure of breast cancer.

In a retrospective study of 44 premenopausal women who underwent resection of a primary breast cancer and were followed for 5 to 12 years, disease recurrence and metastasis were more frequent and more rapid in women who had been operated upon during the perimenstrual period (days 0-6 and 21-36 of the menstrual cycle). By multivariate analysis, the time of resection in relation to the menstrual cycle is an independent predictor of the likelihood of future metastatic disease. Patients who underwent resection during the perimenstrual period had a more than quadrupled risk of recurrence and death compared with women operated upon during days 7 to 20 of the menstrual cycle.

One of the major problems in cancer chemotherapy is the development of drug resistance during treatment. In the treatment of tumors with antitumor agents, tumor cells can acquire resistance to the drugs. This type of resistance is called as acquired drug resistance and the acquired drug resistances is a major factor for limiting the clinical use of antitumor agents. The elucidation of the resistance mechanisms has progressed well recently owing to the application of cellular and molecular techniques in addition to the usual biological and biochemical techniques. In this article, the mechanisms of cellular resistance, especially those of multidrug resistance at a molecular level, and possible approaches to overcoming drug resistance are described and discussed.

Between 1961 and 1985, 190 retroperitoneal lymph node dissections (RLND) have been performed in the Urological Clinic of the University Hospital of Zurich in patients with low stage (I, IIa, IIb) non seminomatous germ-cell testicular cancer. Indication, operative technic, complications (8,9%, dry ejaculation in 27%) and the results of the operation are pointed out (relapse rate in stage I: 10%, IIa: 35%, IIb: 59% without and 14% with adjuvant chemotherapy). The question is analysed if this primary surgical approach in this primary surgical approach in these stages, simultaneously a diagnostic and therapeutic procedure, is still indicated in the era of efficacious chemotherapy.

Malnutrition is a common accompaniment of gastrointestinal malignancy. The etiology of such malnutrition is multifactorial, but, when present, is a poor prognostic sign. Simple causes of decreased intake such as obstruction are readily overcome by distal enteral or parenteral feeding. Metabolic abnormalities that limit the efficacy of parenteral nutrition in such patients have been identified and, although not uniform, give further insight into the manner in which the host and the tumor partition energy and substrate. Widespread use of adjunctive parenteral nutrition in patients undergoing chemotherapy or radiation therapy for gastrointestinal malignancy has not improved tolerance or efficacy of such treatment. Nutritional support of the malnourished patient with a gastrointestinal cancer, prior to a major operation, has been shown to decrease perioperative morbidity and mortality.

Spontaneous chromosome instability is well established for the dominantly inherited cancer prone condition, familial polyposis coli (FPC), but conflicting results have been obtained regarding sensitivity to mitomycin C (MMC). We have investigated cell survival in fibroblasts and the induction of sister chromatid exchanges and chromosome damage in lymphocytes and fibroblasts after MMC treatment. We can find no evidence for a differential response of FPC cells as measured by any of these parameters, although individual FPC fibroblast cultures did show an enhanced chromosomal response. Overall, the FPC mutation does not appear to result in defective DNA repair in response to MMC.

After diagnostic hormonal treatment of 129 patients, 11 (8.5%) were revealed to have breast cancer. The diagnostic accuracy in the case of a lesion of less than 1.0 cm in diameter was 11.8% for palpation, 18.2% for mammography, 33.3% for ultrasonography; the accuracy of this method is 66.6%. Minimal breast cancer was present in eight out of nine cases. Thus, diagnostic hormonal treatment was found to be useful in the diagnosis of minimal breast cancer that coexists with mastopathy.

The effectiveness of re-infusions of auto-blood after isolated irradiation used for the correction of hemocytopenia developing during cancer chemotherapy was studied in 77 patients. 200 ml of the patient's blood was taken into a flask with a preservative and irradiated at an absorbed dose of 220 Gy on the RUM-17 x-ray apparatus. After irradiation the blood was reinfused to a patient. Posttransfusion reactions in the patients were absent. The efficacy and technical simplicity of the method of re-infusions of auto-blood after isolated irradiation make it applicable in clinical practice for the correction of hemopoietic disorders in cancer patients.

There have been few randomized trials that have shown a significantly longer survival after chemo-endocrine therapy than the chemotherapy or the endocrine therapy alone, although there seem to be rationales for simultaneous combination of the both modalities. Some new strategies that would make the response rate higher as well as obtain a longer survival by means of chemo-endocrine therapy was suggested. At the present time, sequential endocrine therapy to chemotherapy is recommended for the advanced or recurrent breast cancer patients, except for those with larger tumor burden, liver metastasis, rapid growing tumors, and ER-negative tumors.

We now have developed methods by which we can quantitate gut hormone receptors in normal and neoplastic gut mucosa and pancreas. Analysis of gut and pancreatic tumors for gut hormone receptors may thus allow us to select patients with these cancers who would respond to treatment with hormones, antihormones, or hormone ablation in a manner similar to current strategies that are employed successfully for treatment of patients with breast cancer.

The patient underwent left upper division segmentectomy and right S3 segmentectomy. Histological examination showed large cell carcinoma in the left side and small cell carcinoma in the right side. The patient died 5 years and 3 months after operation. If general condition and pulmonary function permit, bilateral thoracotomy for multiple lung cancer is recommended.

IMP dehydrogenase (EC 1.1.1.205), the rate-limiting enzyme of de novo GTP biosynthesis and a promising target for cancer chemotherapy, was purified 4860-fold to homogeneity from rat hepatoma 3924A by a method including affinity chromatography in which IMP is bound to epoxy-activated Sepharose 6B. This affinity gel provided a specific elution of the enzyme with 0.5 mM IMP. The final enzyme preparation gave a single band with a molecular weight of 60,000 +/- 1000 on sodium dodecyl sulfate polyacrylamide gel electrophoresis.
