TITLE: Histogenesis of exocrine pancreatic cancer in the hamster model.

There is strong evidence that induced pancreatic adenomas and carcinomas derive from ductal and ductular cells in the pancreas. We base our beliefs on our knowledge of the embryology and histology of the pancreas in Syrian golden hamsters, along with the sequential alterations that occur during exocrine pancreatic tumor formation. This concept also has been supported by much experimental evidence, including autoradiographic, immunologic and in vitro studies. We also present other viewpoints on the origin of pancreatic cancer histogenesis and outline certain areas of disagreement. We report the development of acinar cell lesions under certain experimental dietary conditions in hamsters (the lesions resemble those commonly seen in the rat pancreatic tumor model) and the nature of these lesions.

TITLE: Sunlight suppressing rejection of 280- to 320-nm UV-radiation-induced skin tumors in mice.

Repeated exposure of female C3H/HeNCR- mice to sunlight prevented the normal immunologic rejection of a UV-induced tumor. This systemic immunologic alteration was transferred to syngeneic lethally X-irradiated animals with lymphoid cells from mice exposed to sunlight. The lymphoid cells also were able to suppress the capacity of lymphoid cells from normal animals to reject a UV-induced tumor. The 295- to 320-nm wave band appeared to be responsible for this immunosuppressive effect of sunlight because suppression was prevented by filtration of the radiation through Mylar and by application of a sunscreen containing para-aminobenzoic acid. These observations may have importance in understanding the pathogenesis of sunlight-induced skin cancer in humans.

TITLE: Polyps and diverticula of the large intestine: a necropsy survey in Hong Kong.

The study is based on a prospective survey of 200 necropsies, to determine the prevalence of colonic polyps and diverticula in Hong Kong. Adenomatous polyps were found in 34% of men and 19% of women. The corresponding figures for hyperplastic polyps were 22% and 15%. When compared with European countries having similar rates for colorectal cancer, the polyp pattern by type, prevalence, and distribution is very similar. For diverticula the prevalence rate in this study was only 5%; most of these were situated in the caecum. This is at marked variance to the European pattern.

Should federal agencies use separate, less stringent guidelines for regulating epigenetic or nongenotoxic carcinogens on the assumption that thresholds are likely to exist for these agents? This article reviews recent initiatives by the Environmental Protection Agency that either propose or informally adopt this approach in light of responses from the scientific community and a review of the recent literature. Relevant background is provided by current research concerning the role of chromosomal damage and oncogene activation in carcinogenesis along with findings that classical promoters or "epigenetic" agents can induce both DNA damage and chromosomal rearrangements. The conclusion is that such a revision of cancer policy is not now supported by available scientific data concerning chemical carcinogenesis.

TITLE: A phase II study of bisantrene in advanced refractory breast cancer. An Eastern Cooperative Oncology Group pilot study.

Thirty patients with advanced refractory breast cancer received bisantrene 260 mg/m2 intravenously every 3 weeks. Reversible myelosuppression was the most commonly observed side effect. Four patients (13.3%) achieved objective partial response (90% confidence intervals 3-24%), while two patients (6.6%) had disease improvement with a PR + IMP rate of 19.9%. Seven additional patients (23.3%) had stabilization of disease. This drug has antitumor activity against breast cancer and warrants further study, particularly if problems with drug delivery are overcome.

Pelvic irradiation for gynecologic malignancy is associated with several late neoplastic and nonneoplastic complications in the irradiated field; however, this association has been infrequently recognized in primary care literature. We present two cases of bladder cancer that developed 16 and 19 years, respectively, after pelvic irradiation for cervical cancer. The frequent occurrence in older women of cystitis, hematuria, and nonspecific bowel and bladder symptoms of a benign origin obscures identification of late neoplastic complications after pelvic irradiation. Based on experimental and epidemiologic evidence, we believe cost-effective screening methods should be investigated to allow earlier detection of neoplasms; and, in the interim, a high index of suspicion for bladder cancer is warranted in this growing population of high-risk patients.

A patient is described who initially presented with Stage I breast carcinoma and then with a T2, N0, M0 carcinoma of a kidney with moderately well-differentiated clear-cell features. Five years later, a contralateral breast mass developed which was histologically identical with renal primary tumor. Breast metastasis from renal primary carcinoma has been noted in the literature in 6 other cases, although this is only the third actual case report. This is the first case report of such a bizarre progression of disease. This patient is currently well, without clinically detectable tumor and illustrates the variable nature of metastatic renal cancer. The literature on renal cancer metastasizing to the breast is reviewed.

Currently, surveillance or risk-oriented follow-up of patients in the individual practice setting may be the most efficacious means of reducing the morbidity and mortality associated with selected types of cancer. Intervention into carcinogenesis or alteration of the natural history of cancer requires a clear understanding of the pathogenesis of the disease and patient acceptance of interval surveillance. Because etiologic information and patient compliance are frequently limited, careful scrutiny of recommendations for cancer screening or prevention is essential. It is through taking detailed family, occupational, and carcinogen-exposure histories and doing a thorough physical examination that the primary care physician can best utilize information regarding the risk of cancer and the benefits of available methods of prevention and treatment.

Postoperative pain was controlled in 42 patients with either continuous intravenous (iv) or scheduled intramuscular morphine following surgery for gynecologic cancers. In this double-blind study, no statistical differences were found in pain control or rate of complications between the two methods of administration. Both routes were effective in controlling pain without producing major toxicity. Initial doses were based on the patient's weight and then adjusted every 4 hr. We conclude that both methods are safe and effective but that continuous iv infusion is the preferred route because of the ease of administration, elimination of multiple intramuscular injections, and possibly more even pain control.

To eliminate death by cancer, it would be necessary for us to register newly developed cancer in certain districts. We have registered all gynecologic malignant neoplasms in Shizuoka Prefecture, Japan, since October 1982 using a microcomputer. Analysis for one and a half years was performed as follows. Gynecologic malignancies are as detailed below: cervical cancer 304 cases, corpus cancer 58 cases, ovarian cancer 51 cases, vaginal cancer 4 cases, vulvar cancer 5 cases. A study of the correlation between detected malignancies and gynecologic examinations, particularly concerning cytologic check-up, revealed the importance of cytology in early cancer detection.

A new approach for the treatment of metastatic brachial plexus neuropathy at the axillary level is described. This may be used in patients previously submitted to radical mastectomy followed by radiotherapy for breast cancer. The method consists of external neurolysis of the cords of the plexus, dissection and excision of the pathological axillary contents along with the overlying atrophic cutaneous and subcutaneous tissues, reconstruction of the axilla by the latissimus dorsi musculocutaneous flap, and finally early radiotherapy by high-dose radiation delivered to the entire area. The advantages of the procedure are briefly discussed.

As part of an ongoing cohort study of gastric cancer precursors in Nariño, Colombia, blood levels of ascorbic acid, vitamin E, retinol, pre-albumin, retinol binding protein and carotenoids were measured and correlated with histopathologic findings of gastric biopsies. Carotene levels in both sexes and vitamin E levels in males were significantly lower in subjects with gastric dysplasia than in subjects with normal mucosa and subjects with less advanced gastric lesions (chronic atrophic gastritis and intestinal metaplasia). No other significant correlations were found.

Absolute proof of precise viral oncogenic roles in humans remains unrealized despite definite clinical and epidemiologic arguments; the role is that of a cocarcinogen acting in conjunction with physical, chemical, clinical and immunological factors in an as yet obscure chronological order. For the moment, viral vulvar lesions must be considered as cancer risks and be treated as such.

The basic aspects of the SCM-test are summarized. A derivation of the fluorescence intensity vs time curve is given as well as that of the polarization as a function of time. These curves are found to be in excellent agreement with the experimental results. The test is applied to the lymphocytes of twenty-six healthy individuals and of thirty-three cancer patients. No false negative results were obtained. Seven cancer patients were found to be negative after various intervals from the removal of the tumour. The most important technical difficulties and pitfalls are discussed in order to facilitate the reproduction of the test by other researchers.

Of 160 patients who died of colorectal cancer in the Exeter district in 1979 and 1980, 15 had not been correctly certified as such and of these 10 had died postoperatively. A further 2 had not died of colorectal cancer and in 9 the diagnosis was unsubstantiated. When those not proven to have cancer are considered, colorectal cancer as a cause of death was underestimated by about 5%, which is much more accurate than for other diseases. A total of 35 patients (21.8%) had no operative treatment due to advanced disease, old age or rapid death. The mean inpatient stay was 32.3 days, and 184 operations were performed on 125 patients. Only 20 patients were admitted to the district hospital for terminal care.
