TITLE: Gastric-juice enzymes--an aid in the diagnosis of gastric cancer?

Lactic dehydrogenase (LDH) and beta-glucuronidase concentrations were measured in the resting gastric juice of 113 patients presenting with dyspepsia. All patients were investigated by double-contrast barium meal, endoscopy with biopsy, and, when appropriate, by laparotomy. In all patients tested there was a positive correlation between LDH and beta-glucuronidase concentrations. An index derived from the two enzyme activities correctly predicted the presence of gastric carcinoma in 41 out of 42 cases, and identified the only 2 cases of early gastric cancer in the series. There were 13 (11.5%) false-positive results, all in cases with extensive intestinal metaplasia, a change which may be associated with an increased risk of gastric malignancy. The measurement of gastric-juice enzymes is useful in the diagnosis of gastric cancer and may be of value in the identification of high-risk groups. The test is easily performed, inexpensive, and reproducible.

TITLE: Estrogen receptors and responsiveness of advanced breast cancer to chemotherapy.

We reviewed 89 patients with disseminated breast cancer who had at least one valid estrogen receptor (ER) assay and who underwent one or several trials of chemotherapy. The responses were assessed by two independent extramural reviewers. Of the 89 patients, 81 were evaluable; 28 of 36 (77.8) ER-positive (ER+) and 28 of 45 (60.7%) ER-negative (ER-) tumors reached at least one remission. Of patients who had received Adriamycin-containing therapy, 13/20 (65%) ER+ and 12/34 (35.3%) ER-tumors experienced a remission . ER+ breast cancer tends to respond better to Adriamycin-containing combinations than ER-negative tumors. This study does not support the thesis that lack of estrogen receptor in breast cancer predicts favorable for response to chemotherapy.

TITLE: The initiation, progress, and diagnosis of dog bladder cancer induced by 4-aminobiphenyl.

Twenty-four beagles received 4-aminobiphenyl orally 5 days a week for 3 years. The dogs were examined cytoscopically at frequent intervals. Biopsy and surgery were performed according to predetermined criteria. Three basic patterns of carcinogen responses were seen. Negative or minimal disease was seen in four dogs. Of these, two remained completely free of neoplasia. Eleven dogs progressed slowly in their development of neoplasia with minor fluctuations in development. A rapidly progressive pattern was observed in the remaining nine dogs. A large variance in the time of onset of the first positive diagnosis of neoplasia was seen. Eleven dogs underwent surgery ranging from tumor sampling to cystectomy plus ureterosigmoidostomy. Histologic types and grades and patterns of metastasis as well as several other characteristics seemed to parallel closely human bladder cancer.

TITLE: Time and place for sputum cytology in the diagnosis of lung cancer.

Despite ready access to medical care 91% of patients dying of lung cancer derived from a population of just under 400 000 are shown to have presented too late for other than palliative treatment. As the incidence and death rates from lung cancer in the district were very similar, few survivors can be expected. Morphological confirmation of the diagnosis was available in 96% of cases. The first positive morphological diagnosis was obtained from sputum cytology taken on admission in 69% of all cases and in 77% of those in whom there had been delay in diagnosis. More extensive use of this investigation on an outpatient basis is urged.

TITLE: Cholecystectomy and right-sided colon cancer: an epidemiological study.

The relation between prior cholecystectomy and right-sided colon cancer was investigated in a case-control study of 150 patients with histologically confirmed adenocarcinoma of the caecum or ascending colon and of two comparison groups. One comparison group consisted of 150 patients, matched for age, sex, and race, who had histologically confirmed adenocarcinoma of the descending or sigmoid colon and the other of 123 neighbourhood controls. Compared with left-sided cancer controls, the right-sided colon cancer cases had a relative risk of 1.87 for colon cancer after cholecystectomy. The relative risk was 1.86 when they were compared with the neighbourhood controls. When compared with both control groups relative risk was 1.77, with 95% confidence limits of 0.95 and 3.3 (p = 0.07). The increased risk of right-sided colon cancer after cholecystectomy may be associated with changes in biliary metabolism occurring after removal of a gall bladder.

Excision repair of damage due to ultraviolet radiation, N-acetoxy-2-acetyl-aminofluorene and a combination of both agents was studied in normal human fibroblasts and various cells from cancer prone patients (ataxia telangiectasia, Fanconi's anemia, Cockayne syndrome and Bloom's syndrome). Three methods giving similar results were used: unscheduled DNA synthesis by radioautography, photolysis of bromodeoxyuridine incorporated into parental DNA during repari, and loss of sites sensitive to an ultraviolet endonuclease. All cell lines were proficient in repair of ultraviolet and acetoxy acetylaminofluorene damage and at saturation doses of both agents repair was additive. We interpret these data as indicating that the rate limiting step in excision repair of ultraviolet and acetoxy acetylaminofluorene is different and that there are different enzyme(s) working on incision of both types of damages.

TITLE: Combined treatment by chemotherapy and intravenous hyperalimentation in Japanese patients with advanced breast cancer.

To treat advanced breast cancer that proved refractory to endocrine therapy or other forms of chemotherapy, we administered intravenous administration of adriamycin every 3--4 weeks, in principle, at a dose of 100 mg per 50 kg body weight (total dose, 300--400 mg). At the same time, 50 mg of cyclophosphamide was administered orally every day. In all cases, concomitant hyperalimentation treatment, which is considered an effective method for ameliorating the toxicity of the chemotherapeutic treatment, became necessary. Of 13 cases so treated, 10 showed significant improvement. It was almost impossible in general to administer such a dosage of adriamycin with cyclophosphamide to Japanese patients with advanced breast cancer, since the treatment brought on severe side effects such as depletion of oral intake and suppression of bone marrow function.

Primary cancer of the lung is the most frequent malignant tumor in Switzerland among males and its frequency is rapidly increasing among females. The rate of failure after curative treatment including surgery and/or radiotherapy is about 80%. A large proportion of lung tumors are already inoperable at the time of diagnosis, a fact which accounts for the importance of chemotherapy as a palliative treatment for lung cancer. Single drug chemotherapies are relatively ineffective, with an overall response rate of 20% and a response rate of up to 50% for small cell tumors. Combination chemotherapies attain a 50 to 90% response rate in small cell tumors while the rate of failure is 50% or more in other cell types. Published results of post-surgical adjuvant chemotherapy of lung cancer are equivocal, possibly due to unwanted differences in the selection of patients and in therapeutic schedule. It is still not demonstrated that adjuvant chemotherapy improves lung cancer treatment.

The concentrations of 11 plasma proteins were determined by electroimmunoassay in 6 female and one male patient before and after treatment for 1-4 months with tamoxifen (30 mg/day) for advanced cancer. The concentrations of transcortin, sex hormone binding globulin, pregnancy zone protein, apolipoprotein AI and thyroxine binding globulin increased significantly. The concentrations of orosomucoid and haptoglobin decreased significantly. The concentration of albumin, transferrin, prealbumin and retinol-binding protein showed no significant change. The changes after treatment with tamoxifen were thus similar to those which occur after the administration of oestrogens or during pregnancy although they were quantitatively less pronounced. There were also differences in the pattern of change between treatment with tamoxifen and during pregnancy. There was no apparent relationship between response to treatment and changes in concentration of plasma proteins.

A method called the "anatomic method" of dissecting lymph nodes in a variety of cancer specimens, is presented. It is found to be comparable to the clearing method in terms of detecting lymph nodes and is unencumbered by the slow turnaround time and economic inconvenience. The anatomic method is described in four steps, illustrated by a schematization of the technique. Its advantages over the clearing method include: 1) it is time saving; 2) it is conducted on the fresh state specimen; 3) it is economical; 4) it reduces the monotony often associated with such dissections; 5) it serves as a teaching exercise. Adoption of the method by pathologists is recommended.

TITLE: Brompton's mixture in alleviating pain of terminal neoplastic disease: preliminary results.

We conducted a one-year preliminary study on 107 patients in a hospice for incurable cancer to assess the efficacy of a modified Brompton's Mixture (an oral analgesic solution) in alleviating intractable pain. The results presented indicate that in patients who can tolerate oral medication, the present formulation can be used in lieu of parenteral narcotics, often with superior results and always with cost effectiveness. With Brompton's Misture, side effects of parenteral narcotics such as sedation, lethargy, and nausea are avoided. As documented by four representative case reports, the patient is pain-free, ambulatory, communicating with his environment, and experiencing an improved nutritional status, thereby improving the quality of life. Brompton's Mixture appears to be a valuable adjunct to conventional therapy, giving the patient an effective alternative to parenteral medication for control of pain.

The operative, postoperative and late complications experienced by 300 consecutive patients who underwent 125iodine implantation and pelvic lymphadenectomy for localized prostatic cancer were analyzed. Of the patients reviewed 68 per cent had clinical stage B lesions, while 32 per cent had clinical stage C lesions. The incidence of intraoperative complications was 6 per cent. There were 2 postoperative deaths and 23 per cent of the patients had postoperative complications. Of 177 patients followed postoperatively for 6 months or more (mean 29.3 months) late morbidity was experienced by 49 (28 per cent). The incidence of impotence in 109 patients who were potent preoperatively and who were followed for a minimum of 15 months was 7 per cent.

Breast tissues from 59 patients were immunohistologically studied for transferrin and transferrin receptors. None of the 8 normal breasts or 22 benign mastopathies, and only 1 of 7 fibroadenomas were reactive for transferrin, but 16 (72.7%) of 22 carcinomas were positive and parallel sections showed histopathologically that the rective areas were confined to tumour-cell membranes. That breast-tumour transferrin is probably receptor-bound in vivo was indicatd by its complete chemical removal in vitro and return to the same cells following reincubation with dilute solutions of transferrin. Similar bindig is found on certain normal or transformed cells and at the maternofetal interface of human placentae. It is suggested that this oncofetal receptor preferentially binds apotransferrin and may offer a novel approach to more specific therapy of certain breast cancers.

One hundred patients with stage II and stage III cancer of the oropharynx and hypopharynx were treated under a protocol in which they were randomly selected for treatment by surgery alone or by combined preoperative radiotherapy and surgery. The schedule of preoperative radiation therapy chosen was 2,000 rads from a cobalt 60 machine delivered in five days. Eighty-six of the patients were evaluable at three years; there was no difference in the outcome of the treatment of the two groups. A similar study is urgently needed to determine the value of postoperative radiotherapy in the management of similar cancers.

Condyloma acuminatum of the anus is an increasingly frequent disease that may require a combination of therapies. Large bulky lesions can be reduced with topical freezing, cautery, or the scapel. Topical chemotherapy with podophyllum resin can be supplemented with cancer chemotherapeutic agents. Many of these drugs probably act, at least in part, by stimulating a strong local cellular immune reaction. Dinitrochlorobenzene was used to eradicate lesions that had been resistant to treatment with podophyllum resin. 5-Flourouracil was used as a control. All 23 patients studied were initially skin tested with both agents and randomized to either treatment group. All but one of the dinitrochlorobenzene-treated patients had excellent regressions. Immunotherapeutic agents, such as dinitrochlorobenzene, warrant further study as adjuvant therapeutic agents for this disease.
