Rapid advances are being made in our understanding of the interaction of airborne fibers with biological tissues. It is possible to reduce human exposures and potential risk to lung cancer and fibrosis if current science and models are fully utilized. However, the criteria used by IARC and NTP in their cancer classifications have not kept pace with current science and, in fact, remain entrenched in the scientific vagaries present in the 1960's. As long as public policy cannot keep pace with contemporary scientific understanding, we are doomed to a world of exaggerated risks, inappropriate regulations, misguided priorities, inadequate funding for social and environmental programs, mass anxiety, and skepticism about scientific research.

Thirty patients with invasive carcinoma of the cervix, along with 15 age-matched healthy females as controls, were studied to examine the frequency of spontaneous SCEs, chromosomal aberrations (CAs), and cell cycle progression in lymphocytes. The frequency of SCEs and TCAs was statistically significant in the patients over the control group. The correlation of SCEs and TCAs with the stage of cancer, age, and number of pregnancies was also made. The cell cycle of lymphocytes showed its prolongation in the patients, as is evident from the higher proportion of cells at M1 (metaphase of first cell division after 72 hours).

The use of cytokines in head and neck cancer is increasingly under investigation. Clinical trials have tested the interferons, interleukin-2 and other cytokines as single agents and in various combinations. The addition of interferon to the cisplatin and 5-fluorouracil (5-FU) regimens (with and without leucovorin) has been explored. A randomized international trial comparing cisplatin and 5-FU with cisplatin, 5-FU and interferon-alpha 2b is in progress.

There is increasing evidence that certain morphological features and biological markers found in breast tumors may provide prognostic information by predicting the risk of recurrence and metastasis in early breast cancer. This information may also be important in choosing therapeutic options in patients with advanced disease. Prognostic testing is commonly performed on surgically excised lesions. However, there are clinical conditions in which a surgical specimen may not be suitable or available for such analysis. In these circumstances, fine-needle aspiration biopsy and imprint preparation provide an attractive sample for prognostic testings. This review summarizes the current approach to the use of cytologic preparation for assessment of established and newly recognized prognostic factors.

We evaluated the effectiveness of three-dimensional (3D) CT for pleural indentation or invasion by peripheral lung cancer adjacent to pleura. Pleural indentation was shown in three of nine cases on conventional CT, and six cases were demonstrated three-dimensionally. 3D-CT was superior to conventional CT in demonstrating pleural indentation. Pleural invasion could not be assessed on 3D-CT because the pleura adjacent to the tumor was not visualized.

It is well documented that proteoglycans are involved in a wide range of pathological conditions. Recently published results in international journals provide new information on the role of proteoglycans in such conditions. A mutation in the gene encoding for a cell surface proteoglycan has been demonstrated in overgrowth syndromes. A proteoglycan has been isolated from urine and shown to induce cachexia in cancer patients. Furthermore, in both achondrogenesis and colon cancer, the reduced ability to sulphate proteoglycans is due to genetic defects in cellular sulfate transporters. Finally, fibrosis has been inhibited in glomerulonephritic mice by transferring the gene for decorin, a transforming growth factor beta-1 binding proteoglycan, into muscle tissue.

Malignancy potential of colorectal polyps increases with size. The growth rate and destiny of each polyp is virtually unknown. It was recently shown that polyps smaller than 10 mm left in situ may partly regress or partly increase in size, whereas one quarter of polyps are unchanged after 3 years. Polyps smaller than 5 mm show a mean increase in size, whereas polyps measuring 5 to 9 mm show a mean decrease in size. Methodologic problems with the measurement of polyps and cancer are discussed. More studies of polyp growth related to risk factors are warranted.

A 63-year-old woman with dermatomyositis (DM) developed multiple punched-out ulcers of the skin. She had not only a past history of gastric cancer 10 years prior to the onset of DM, but also a familial history of cancer. A skin biopsy from one of the ulcers revealed both vasculitis and membrano-cystic lesion in the same specimen. She died of interstitial pneumonitis one year after the onset of DM. Vasculitis and membrano-cystic lesion may be an important marker of a poor prognosis in DM.

Advances in the understanding of Ras oncoprotein function suggest novel points for anti-tumor intervention. First, upstream-acting guanine nucleotide exchange factors and SH2/SH3 domain-containing adaptor proteins that link Ras with growth factor receptor tyrosine kinases have recently been characterized. Second, work on downstream-acting Ras effector functions including the Ras GTPase-activating protein (p120GAP) and the Raf kinase has revealed direct biochemical interactions that are functionally required for oncogenic Ras signalling. We summarize progress in these areas and discuss the potential for novel applications to anti-cancer chemotherapy.

Breast cancer is the second leading cause of cancer deaths in women. Approximately 90% of breast cancer is curable if it is diagnosed and treated early. Increasingly, women are participating in mammographic screenings, which result in earlier detection of nonpalpable lesions. In the past, surgical removal was not an option for small lesions, and frequent mammographic monitoring was required until patients' lesions were large enough for needle core biopsies or needle localization for open surgical biopsies. Technology now has combined stereotactic imaging with a minimally invasive biopsy system for removal of these nonpalpable lesions. This capability for accurate early diagnosis and intervention is essential in improving patients' survival rates.
