Infective pneumonia occurring in immunocompromised patients is characterized by its multiple causes which create diagnostic problems and by the need for a prompt treatment. The principal criteria pointing to a specific organism are the causes of immunodeficiency (now usually due to treatment of blood diseases or cancers, organ transplantation and AIDS) and the radiological features of the pneumonia. Confirming the diagnosis frequently requires features of the pneumonia. Confirming the diagnosis frequently requires invasive explorations which in any case are limited by the patient's fragility. Consequently, empirical therapeutic measures are initiated, at least initially, taking into account all likely assumptions. The severity and frequency of these lung infections make it desirable to develop preventive measures applying to the patient himself (antibiotic prophylaxis) or to his environment.

Superficial bladder tumors--cancer in situ (CIS) and papillary transitional carcinoma of the bladder--are the most frequent presentations of bladder cancer, the second most common disease of the urethelial tract. These superficial tumors demonstrate a tendency to recur, either at the same stage and grade or as deeply invasive tumors. To some extent, the likelihood of tumor recurrence can be predicted, based on certain risk factors. Furthermore, the probability of recurrence can be altered through appropriate clinical interventions. A review of the literature, much of which is preliminary, suggests that interferon therapy for bladder cancer may modify the biologic behavior of this disease and provide clinical benefit for some patients.

Epidemiological studies demonstrate that cancer patients are at increased risk of suicide. While affective illness and alcoholism are the most important determinants of suicide in the physically healthy population, vulnerability to suicide in cancer patients is influenced by a number of other factors including psychosocial and psychosomatic effects of advanced illness, pain, organic mental syndromes and preexisting psychopathology. Ways of influencing these risk factors are discussed, together with their use in preventive care and management of suicidal cancer patients.

The low risk of colon cancer among the Japanese suggests a high intake of dietary fiber. Composite diets for 1959, 1970, and 1979 were prepared using food consumption data from the National Nutrition Survey in Japan and analyzed for non-starch polysaccharides (NSP) at the Dunn Clinical Nutrition Centre in Cambridge. The results showed that average intake of NSP by Japanese in the above years did not exceed 13 g per day, which is as low as the corresponding intake by the Scandinavians and the British, whose risk of colon cancer is known to be high.

TITLE: Pleomorphic adenoma of the human breast with local recurrence.

Pleomorphic adenoma of the human breast is a rare, benign tumor for which the literature is sparse. A local recurrence has been reported only once before. The diagnostic challenge of this cancer-imitating lesion is emphasized in this report. Although local excision of the tumor is the treatment of choice, almost 50% of the patients reported have undergone fairly extensive surgery, i.e., simple or radical mastectomy. A paraffin secretion of the excised tumor is mandatory to confirm the benign nature of the lesion to avoid an unnecessary mastectomy for a benign condition.

Published data on the mutagenicity and genotoxicity of theobromine and caffeine were analysed by the Carcinogen Prediction and Battery Section (CPBS) method. In spite of some positive responses, these analyses did not predict for theobromine a potential for causing cancer by virtue of a genotoxic mechanism. Caffeine, on the other hand, clearly has potential for genotoxic carcinogenicity. The predictive performance of cost-effective batteries consisting of selected combinations of four assays was also evaluated. The predictions were similar to those derived when all the available test results were considered.

Lung cancer is divided into hilar and peripheral types by its site of origin, because of the difference in the structures between bronchi and bronchiolo-alveoli. Accordingly, the definition of early lung cancer differs depending upon the site of tumor origin. At present, however, there is no agreement as to the definition of early lung cancer of both types. Most early cancer of the hilar region are squamous cell type. Squamous cell carcinoma in situ should be differentiated from dysplasia with marked atypia. On the other hand, many early cancer of the peripheral type are adenocarcinoma. Atypical adenomatous hyperplasia or adenoma may necessitate differentiation from very well differentiated adenocarcinoma. Problems on the definition and differential diagnosis are discussed.

Before launching a medication program, each set of symptoms should be assessed by history, physical examination, and laboratory tests. When symptoms can be understood in the context of the disease without correctable, specific causes, the physician can carefully prescribe treatments to alleviate the misery and suffering. The most common symptoms in terminally ill cancer patients are anxiety and depression (80 to 90 per cent); pain (66 per cent); nausea, vomiting, and constipation (50 per cent); and respiratory symptoms (30 to 40 per cent). Medical attention to these discomforts will help the patient and family to cope better with the dying process.

Fifty-three premenopausal patients presenting with advanced breast cancer have been treated with a potent new luteinising hormone-releasing hormone agonist Zoladex (ICI 118630) in a phase I clinical trial. On progression of disease 26 patients have undergone therapeutic oophorectomy. We present the clinical and endocrinological responses to treatment in 45 assessable patients. The response rate to Zoladex in this series was 31% and the ER status of the primary tumour was predictive of a response to the luteinising hormone-releasing hormone.

Sulfamethazine acetylation phenotype was determined in 49 patients with cancer of the colon or rectum, 41 old, and 45 young control subjects. Metabolic clearance of sulfamethazine, plasma ratio of N-acetylsulfamethazine:N-acetylsulfamethazine plus sulfamethazine and urinary ratio of N-acetylsulfamethazine:N-acetylsulfamethazine plus sulfamethazine were used to classify subjects into slow and fast acetylation phenotypes. All three measures gave similar results. The proportions of slow and fast acetylators were similar in both control groups and there were significantly more fast acetylators in the cancer group than in the control groups (chi 2 = 5.0-8.5; P less than 0.05). The data suggest that there may be an association between acetylation phenotype and colorectal carcinoma.

Based on an investigation of micropreparations of resected lungs, findings of test thoracotomies and autopsies the authors have studied regularities of regional lung cancer metastazing in 275 patients of young age. It was shown that the intensity of regional metastazing is higher in young patients with lung cancer than in elderly patients, it grows with low-differentiated forms of tumor, nodular-peribronchial form of growth and increases in parallel with the growing size of the tumor node.

We report the case of a 51-year-old woman presenting t(4;11) acute undifferentiated leukemia 19 months after initiation of adjuvant radio-chemotherapy for a breast cancer. Morphology of the blasts was FAB L2 and cytochemical and immunological studies were in favor of an undifferentiated leukemia. A possible relationship between t(4;11) leukemia and exposure to carcinogens is discussed.

The present paper describes the trend in incidence of oral and pharyngeal carcinomas in Denmark in the period 1943-1985, and provides an illustration of the effect of birth cohort on the overall trend. The results are discussed in relation to changes in consumption of alcoholic beverages and tobacco, and to changes in dietary habits in this century. The incidence of these cancers appear to be increasing rapidly in response to the increasing level of alcohol consumption in Denmark.

The IFN producing activity of RLNL and PBL was studied in cancer patients using K562, PHA-P and OK-432 as inducers. In response to K562, IFN production was significantly greater in the PBL than in the RLNL, and the titer was about equal in the PBL and RLNL when OK-432 was used as an inducer. On the contrary, the amount of IFN produced by PHA-P was significantly higher in the RLNL than in the PBL. There was no definite relationship recognized between tumor progression and IFN productivity in the cases studied.
