A 30-year follow-up study of the mortality of 500 women employed in manufacturing gas masks during the second world war showed a pronounced excess of deaths from mesothelioma and lung cancer. Although these women were subject only to short periods of exposure, greater excesses were found for those exposed for between one and five years than for those exposed for under one year. Even in the latter group, however, five deaths from lung cancer and four deaths from mesothelioma were recorded compared with 1.5 deaths and 0.1 deaths expected (p approximately equal to 0.02). An excess of deaths from cancer of the ovary was also found, and this appeared to be related to exposure to asbestos.

TITLE: Absence of the COOH-terminal peptide on ectopic human chorionic gonadotropin beta-subunit (hCG beta).

A comparison of the physical properties and immunoreactivity of ectopic hCG beta from cultured cervical cancer cells and standard hCG beta was performed. The heterogeneity of the preparations was examined by ion-exchange chromatography, and the molecular size by analytical gel filtration. Two distinct forms of ectopic hCG beta were identified. One form was indistinguishable from standard hCG beta, while the other, although larger, lacked the characteristic COOH-terminal peptide (CTP). This was shown by the failure of antisera specific for determinants on the cTP to recognize this molecule, and by the apparent absence of the 0-linked oligosaccharides and thermolysin cleavage site normally found in this region.

TITLE: Role of computed tomography in the evaluation and management of carcinoma of the prostate.

Between January 1978 to March 1980, 25 patients with biopsy-proven prostate carcinoma were evaluated by computerized tomography (CT). CT differed from clinical stage in 7 of 25 patients (28%). In 6 of the 7 patients, change in stage resulted because of demonstration of extracapsular extension and/or pelvic lymph node involvement. Twelve of the 25 patients (48%) underwent surgery with histological confirmation of CT findings. Ct identified nodal involvement accurately in 10 of 12 patients (83%). We recommend use of CT for initial staging, treatment planning and assessment of response in the management of prostate cancer.

A new monoclonal antibody (IB9) was prepared by hybridoma technique directed specifically to sialosyl alpha 2 leads to 6 galactosyl residue. With this reagent, accumulation of two major gangliosides in human colonic and liver adenocarcinoma has been detected, and these gangliosides were isolated and characterized as structures A and B (below). Another ganglioside with a ceramide nonasaccharide structure, which reacted to anti-X-hapten antibody after desialylation, was also isolated and partially characterized. (formula; see text) These gangliosides were absent or present in very small quantity in normal tissue and may represent human cancer-associated markers.

Cryptorchism was diagnosed in all 3 male members of 2 pairs of dizygotic twins in a sibship. Two of these brothers developed seminomas at age 31 and 33 years. Studies revealed a nephew with an atrophic testicle, but no additional instances of twinning, cryptorchism or testis cancer. Cryptorchism has been reported previously in 3 of 8 sets of twins with testis cancer, but rarely in familial testis cancer affecting other relatives. Hormonal factors may be involved in the association of twinning, cryptorchism and testis cancer in this family.

Twelve patients with recurrent breast cancer were treated with Peplomycin monotherapy. Peplomycin was given intermittently with a dose of 10 mg intramuscularly twice a week. As side effects of Peplomycin, fever elevation in 75% (9/12), malaise in 67%, nausea and vomiting in 42%, anorexia in 42%, pulmonary toxicity in 8%, and loss of hair in 8%, were observed. Out of 8 evaluable cases, CR was obtained in 1, PR in 1, NC in 3, and PD in 3 cases, respectively.

Of these, 6 were classified as early esophageal cancer, and 3 of these 6 had intraepithelial cancers. In efforts to detect early esophageal cancers, especially at the intraepithelial cancer stage, panendoscopic examination seems to be highly useful.

Adenocarcinoma of the rectum accounts for approximately 5 percent of new cancer cases and for an estimated 31 percent of new cases of colorectal cancer. Although surgical resection remains the treatment of choice for almost all cases of this disease, a significant number of patients will develop locoregional recurrence after surgery if no other treatment is given. Relatively modest doses of radiation may improve long-term survival and can prevent locoregional relapse after surgery in many high-risk patients.

In a case-control study of 36 men with cancer of the ureter and 307 male controls with the same age distribution tobacco smoking conferred a relative risk for this cancer of 2.9, whereas consumption of analgesics of any kind did not increase the risk. The relative risk for cancer of the renal pelvis, determined for 29 men, was 2.4 for tobacco and 6.0 for phenacetin-containing analgesics. Analgesics containing no phenacetin conferred no increased risk for cancer at this site. Data for 13 women with ureteral cancer, 5 of whom had taken phenacetin, are presented but not analyzed.

At a thermoelectric power plant in Turbigo (Milan, Italy), an analysis of the technological process has shown the presence of exposure to several carcinogens: polycyclic aromatic hydrocarbons, asbestos, hydrazine, polychlorinated biphenyls, chromium, nickel, and beryllium. An epidemiologic study was carried out to quantify the effect of the detected exposures on the workers, and the cancer mortality of the cohort was compared with the corresponding rates from the closest town and with cancer register data from a nearby province. Excess mortality was found for workers with 10 or more years of employment; the median latency time was about 20 years.

TITLE: Oophorectomy in advanced breast cancer: clinical radiologic correlations.

The radiograms of the chest and skeleton of 49 patients with advanced breast cancer treated with bilateral ovariectomy were reviewed. The modifications in the secondary localizations and/or their appearance after castration were compared with the findings of the clinical examination. Appearance or progression of intrathoracic lesions, like the appearance or progression of osteolytic lesions, corresponded to a progression of the disease in other sites. Osteoblastic evolution of osteolytic lesions and the appearance of osteoblastic lesions in bones undamaged before ovariectomy were signs of a favorable response to therapy. The response of chest and bone metastases is usually rather early, and the first radiographic survey should be performed about 3 months after ovariectomy.

Plasminogen activating activity (PAA) has been determined in 105 human breast cancer cytosols with a colorimetric assay. PAA has been related to steroid receptors in the same cytosols. A strong positive correlation between PAA and progesterone receptor levels was found (Spearmans coefficient of rank correlation, R - 0.74, P less than 0.001). PAA discriminates between progesterone receptor-positive and -negative tumours, irrespective of oestradiol receptor content. It is suggested that PAA is a marker of functional oestradiol receptor and that fibrinolytic activity may be a causative factor in the low metastatic potential of progesterone receptor-positive tumours.

TITLE: Phase II study of mitolactol in chemotherapy-refractory metastatic breast cancer.

Thirty-eight evaluable patients with metastatic breast cancer refractory to hormonal therapy and multiple chemotherapy regimens were treated with mitolactol at a dose of 130 mg/m2/day orally for 10 days every 6 weeks. Only one patient, with nodal and chest wall metastases, had a sustained complete regression; two patients had stable disease; and 35 patients had disease progression. The toxicity, which was primarily hematologic, was acceptable.
