Transvaginal US is often the initial imaging examination for women with dysfunctional (postmenopausal or intermenstrual) uterine bleeding. However, once the diagnosis of endometrial cancer has been made, contrast-enhanced MRI should be performed in patients who require multifactorial assessment (eg, depth of myometrial invasion, cervical involvement, lymph node metastasis). The results of contrast-enhanced MRI help distinguish patients who need more aggressive therapy and referral to a gynecologic oncologist from those who will do well treated by a community gynecologist.

Five epidemiological studies of brain, cranial meninge, and salivary gland tumor sites reviewed implicate prior dental radiography with increased risk of subtentorial intracranial meningiomas and tumors of the parotid gland. Diagnostic radiography (medical and dental) was the most important risk factor identified for parotid gland cancers; among Los Angeles County residents, about 85% of the cumulative parotid dose from diagnostic radiography came from dental examinations. Irradiation of patients during dental radiography declined substantially from 1920 to 1988; it is uncertain whether current low doses increase risk. Recommendations for exposure reduction are made.

Paraneoplastic syndromes affect the nervous system in a very small percentage of patients with cancer. In those patients, portions of the nervous system are damaged or destroyed, usually in association with a small and often occult neoplasm elsewhere in the body. Current evidence suggests that these syndromes occur when the body mounts an immune response to an antigen shared between the tumor and the nervous system. The immune attack is then misdirected against that portion of the nervous system that possesses the "onconeural" antigen.

Resveratrol analogs were newly synthesized and evaluated for cytotoxicity in cultured human lung and colon cancer cells. 3,5,4-Trimethoxy-trans-stilbene and 3,5,2',4'-tetramethoxy-trans-stilbene were found to be more potent rather than resveratrol. 3,4,5-Trimethoxy-4'-bromo-cis-stilbene was the most active among the test compounds.

Rapid advances are being made in our understanding of the interaction of airborne fibers with biological tissues. It is possible to reduce human exposures and potential risk to lung cancer and fibrosis if current science and models are fully utilized. However, the criteria used by IARC and NTP in their cancer classifications have not kept pace with current science and, in fact, remain entrenched in the scientific vagaries present in the 1960's. As long as public policy cannot keep pace with contemporary scientific understanding, we are doomed to a world of exaggerated risks, inappropriate regulations, misguided priorities, inadequate funding for social and environmental programs, mass anxiety, and skepticism about scientific research.

There is a concern that antioxidants might reduce oxidizing free radicals created by radiotherapy and some forms of chemotherapy, and thereby decrease the effectiveness of the therapy. The question has arisen whether concurrent administration of oral antioxidants is contraindicated during cancer therapeutics. Evidence reviewed here demonstrates exogenous antioxidants alone produce beneficial effects in various cancers, and except for a few specific cases, animal and human studies demonstrate no reduction of efficacy of chemotherapy or radiation when given with antioxidants. In fact, considerable data exists showing increased effectiveness of many cancer therapeutic agents, as well as a decrease in adverse effects, when given concurrently with antioxidants.

Therapy for Hodgkin's disease has been associated with a significant increase in risk for second cancers. To begin an investigation of the association of therapy-induced genetic damage with this risk, somatic mutations at the hypoxanthine phosphoribosyltransferase locus were measured in lymphocytes from patients previously treated for Hodgkin's disease. The results demonstrate that a subset of patients have persistently elevated mutation frequencies, perhaps suggesting that these individuals are among those at significant risk of second cancer development.

Thirty patients with invasive carcinoma of the cervix, along with 15 age-matched healthy females as controls, were studied to examine the frequency of spontaneous SCEs, chromosomal aberrations (CAs), and cell cycle progression in lymphocytes. The frequency of SCEs and TCAs was statistically significant in the patients over the control group. The correlation of SCEs and TCAs with the stage of cancer, age, and number of pregnancies was also made. The cell cycle of lymphocytes showed its prolongation in the patients, as is evident from the higher proportion of cells at M1 (metaphase of first cell division after 72 hours).

The experimental feasibility of using polarization transfer techniques at 2.0 T was demonstrated with a phantom and an intact chicken egg. The potential clinical applications of CMRI/S with polarization transfer include the assessment of prostate cancer. Preliminary results using human prostate specimens are presented.

Estrogen and androgen play essential roles in the development of breast cancer and prostate cancer, respectively. However, only 30% of breast cancer mass and 70% of prostate cancer mass are found to be hormone-dependent when they have been treated clinically. These findings show that hormone dependency of cancer disappears rather easily. Recently, it has been found that the growth of hormone-dependent cancer is mediated through sex steroid-induced growth factors in an autocrine manner. Future studies on sex steroid-induced growth factor(s) will play important roles in order to understand the molecular mechanisms of the development and growth of breast cancer and prostate cancer.

There is increasing evidence that certain morphological features and biological markers found in breast tumors may provide prognostic information by predicting the risk of recurrence and metastasis in early breast cancer. This information may also be important in choosing therapeutic options in patients with advanced disease. Prognostic testing is commonly performed on surgically excised lesions. However, there are clinical conditions in which a surgical specimen may not be suitable or available for such analysis. In these circumstances, fine-needle aspiration biopsy and imprint preparation provide an attractive sample for prognostic testings. This review summarizes the current approach to the use of cytologic preparation for assessment of established and newly recognized prognostic factors.
