Prostate cancer is a frequent concern of the clinician caring for older male patients. The certainty of arriving at the correct diagnosis is related to the presenting patient's risk for prostate cancer, the results of the digital rectal examination, and the value of the serum prostate-specific antigen (PSA). A case report of a patient with acute urinary retention and an elevated PSA is presented. Possible explanations for the elevated PSA are discussed. The clinician's intuitive thought process is compared with an analytic approach using calculated probabilities. Several factors that complicate the estimation of the likelihood of prostate cancer are discussed.

About 40 percent of newly diagnosed breast cancers occur in women older than 65. Hormonotherapy plays an important role as adjuvant treatment of primary cancers in post-menopausal women because of their great hormone-dependence. The drug of choice is tamoxifen which is very well tolerated. Its beneficial effect appears to be related to the duration of administration. It also seems to be particularly promising in the initial treatment (neoadjuvant) of elderly women and potentially beneficial in the breast cancers said to be of good prognosis when the axillary nodes are not involved.

A 16-year-old girl with acute lymphocytic leukemia in hematologic remission and no evidence of active central nervous system disease had a unilateral hypopyon as her sole site of relapse. She underwent diagnostic paracentesis and was treated with topical corticosteroids, chemotherapy, and radiotherapy to the central nervous system. Ocular involvement of the iris and anterior chamber as the site of primary disease or a relapse is a rare event. The anterior chamber serves as a sanctuary for cancer cells, and treatment protocols, including better anterior chamber prophylaxis, are warranted.

Clinical usefulness of cytokines in cancer treatment has been described. Cytokines presently used in clinical practice are IFNs, IL-2, and hematopoietic growth factors. IFNs and IL-2 used with an expectation of direct antitumor effect of these cytokines have been proven to be specifically effective against certain types of tumors. Hematopoietic growth factors have been used as the adjuvant drugs in cancer treatment and proven to be effective against neutropenia and associated infections after chemotherapy and bone marrow transplantation. Future development of new cytokines and trials on the combination among cytokines and cytokines with chemotherapeutic agents, will promote the usefulness of cytokines in cancer treatment.

Neurologic toxicity occurring in patients treated with cisplatin chemotherapy is limited primarily to peripheral neuropathy and ototoxicity. Lhermitte's sign, electric-like paresthesias precipitated by cervical spine flexion, has recently been described as a self-limited complication in cisplatin-treated patients. We report the development of Lhermitte's sign accompanied by cervical motor neuropathy, dorsal column myelopathy, and sensory neuropathy in a patient treated with cisplatin and etoposide for small cell lung cancer. Persistence of the neurologic deficit suggests that potentially irreversible spinal cord toxicity may complicate treatment with this chemotherapy combination.

The dog heartworm Dirofilaria immitis has been diagnosed by thoracotomy as the etiology of neoplastic-appearing nodules in two patients in the Peoria, Illinois area. This brings the total number of reported cases of human pulmonary dirofilariasis to approximately 81 in the United States. The major concern of this benign disease is that in making the diagnosis the patients undergo the risk of surgery because of the presumed preoperative diagnosis of cancer. Greater awareness of this disease is needed as the geographic distribution of human pulmonary dirofilariasis expands in this country.

Hepatitis B virus (HBV) and certain human papillomaviruses (HPV) have been suspected of playing a role in human malignancy. The long latency period (decades) and the fact that only a small proportion of infected individuals subsequently have cancer suggests that these viruses probably contribute in an indirect manner to the development of cancer. Analysis of viral gene function at the molecular level supports this conclusion. More studies are needed to understand the subtle interactions between host cell and viral functions and the genetic and environmental factors that may contribute to malignancy.

Since r was inferior to p 5% (r = 0.3255; p = 0.4438) the 2 parameters are not correlated and their biological expression must be considered independent.

This study includes 29 patients with synchronous primary malignancies of the female genital tract. These patients constituted 1.7% of all genital malignancies. The most frequently observed synchronous neoplasms were those of the ovary together with the endometrium (51.7%). Most patients had early-stage and low-grade disease. Stage I disease was observed in 68.1% of patients with ovarian cancer. Patients with synchronous ovarian and endometrial cancer had a 73.3% 5-year survival rate, suggesting a favorable prognosis.

TITLE: The practice outcomes of a course in flexible sigmoidoscopy for primary care physicians.

A mail survey of 430 participants in a 2-day course on flexible sigmoidoscopy presented over the past 6 years was conducted to determine the extent to which flexible sigmoidoscopy was subsequently utilized in physician practice. Eighty percent of respondents performed the procedure in their practices on a regular basis. Two-thirds of the patients underwent sigmoidoscopy for screening. The course in flexible sigmoidoscopy appears to potentiate the motivation of primary care physicians to incorporate complete colorectal cancer screening in their practice.

In some families breast cancer aggregates as the predominant site-specific neoplasia and in others in association with defined malignancies. In the case of familial adenocarcinomatous (Lynch II-syndrome) it occurs together mainly with colorectal and endometrial carcinoma whereas in the case of the Li-Fraumeni/SBLA-syndrome it belongs to a wider spectrum including sarcoma, brain tumors, lung and laryngeal cancer, leukaemia and adrenocortical carcinoma. The occurrence of these malignancies is reported as they were observed in the families of 600 women suffering from breast cancer.
