TITLE: [Topometric preparation of patients with cervix cancer for intracavitary irradiation on the ANET-B apparatus]

The authors worked out methods of topometric investigation of cervical cancer patients during preparation for intracavitary irradiation using 252Cf fast neutron sources. The use of balloon catheters made it possible to determine a coefficient of projection enlargement in the point of interest, to determine accurately the distance between endostats, sources and contrasted organs of interest: the bladder and rectum. The results of the studies were used for individual design of intracavitary treatment as well as for the determination of radiation exposure of the pelvic organs in 38 patients. The patients received a total of 138 investigations.

The effects of Gn-RH agonists in advanced breast cancer patients have been examined. In both pre- and postmenopausal women they produce pituitary gland desensitisation and a fall in circulating concentrations of LH and FSH. In premenopausal patients plasma progesterone and oestradiol levels fall to the castrate or postmenopausal range within 3-4 weeks. Tumour remissions have been observed in approximately 30% of premenopausal women and approximately 10% of postmenopausal patients. The mechanism of action of Gn-RH agonists is discussed and their use projected to other tumour types.

TITLE: The enhanced 32P labeling of CDP-diacylglycerol in c-myc gene expressed human kidney cancer cells.

Correlation between c-myc gene expression and phosphatidylinositol (PI) metabolism was studied using seven human kidney cancer cell lines. We found that the exceptional incorporation of 32P into CDP-diacylglycerol (CDP-DG) was observed in only two cell lines, in which c-myc expression was increased among seven cell lines tested. In these two cell lines, PI was also labeled. The other five cell lines indicated the accumulation of radioactive PI, whereas CDP-DG was unlabeled.

Pelvic lymphocysts occur infrequently after radical surgery for cervical cancer. The problem is one of recognition. In a retrospective analysis of patients seen over 10 years in this department, the incidence was 25.3%. All were diagnosed by bimanual, clinical examination only. The only factor that appears to relate to the formation of lymphocysts is the presence of poorly differentiated tumors; 70.8% of patients developing lymphocysts had poorly differentiated tumors. All the lymphocysts in this series resolved spontaneously and did not produce any major complications.

Radiation-induced osteoradionecrosis of the ribs and brachial plexus lesions following irradiation of breast cancer patients are well known phenomena and have been reported several times in the literature. Damage to the shoulder joint after irradiation is uncommon. The case of a 75-year-old woman with breast cancer is described; she first developed a brachial plexopathy and then necrosis of the head of humerus after therapy with combined ortho-voltage and high-voltage radiation. The differential diagnosis and possible pathogenetic mechanisms are discussed.

Primary liver cancer has been induced experimentally in female Wistar rats by diaminoazobenzene intoxication or subcapsular liver implantation of Novikoff cell suspensions. The ultrasonic B-scan image of these malignant tumours are described via statistical texture parameters in respect of image brightness, microtexture and macrotexture. A discrimination of the tumour texture profiles is possible with an accuracy of about 80%. There are correlations between ultrasonic image parameters and histomorphological tissue components but there is no specific link between the histological tumour-type and these B-scan data.

In order to clarify the cause of arterial changes after intra-arterial infusion of anti-cancer drugs (AI-AD) experiments were made on the arteries of rabbits. Histopathologic sections, 7 days after AI-AD revealed endothelial damage characterized by pyknosis, hyalinization with edematous change of the intimal layer. Proliferation of the endothelial cells was also observed. The cause of narrowing or occlusion of the arteries was considered to occur with thrombus formation surrounded by the proliferated endothelial cells and this suggested that thrombolytic agents would be effective to prevent these arterial changes.

The role of thymidine in the treatment of cancer has been under clinical investigation for nearly a decade. Clinical trials have demonstrated that it lacks antitumor activity in its own right. In this review, the mechanism of action and rationale for the use of thymidine as a biochemical modulator of standard agents such as 5-fluorouracil, 1-beta-D-arabinofuranosylcytosine, and methotrexate are summarized. With this background, the clinical trials which have been conducted with thymidine, either alone or in combination, are described. We suggest a number of further studies of the role of thymidine in the biochemical modulation of antimetabolites.

Recent evidence indicates that carboplatin may be useful in the treatment of various forms of testicular cancer. The results of one study showed that carboplatin was highly effective and well tolerated in patients with previously untreated disseminated seminoma. However, conventional doses of carboplatin appear to be inactive in patients with refractory nonseminoma. Of particular interest are recent results showing that an intensive regimen of carboplatin in combination with etoposide with autologous bone marrow rescue will induce a few complete remissions in heavily pretreated germ cell tumor patients.

Chemotherapy prolongs survival of patients with small-cell lung cancer, but very few are cured, and the treatment is unpleasant. Thirty patients, 28 with advanced disease, were treated with etoposide 250 mg/m2 orally, daily for 5 days, plus vincristine 2 mg intravenously on the first day, the cycle being repeated 3-weekly, to a maximum of 6. There was a response rate of 70%, a median survival of 249 days, and an 11% 2-year survival. Symptomatic side-effects were less pronounced than with most other regimens of comparable efficacy.

TITLE: Results of combined radiotherapy and multidrug chemotherapy in advanced squamous cell cancer of the head and neck.

Treatment results of combined radiotherapy and polychemotherapy in a group of 38 patients with advanced squamous cell of the head and neck cancer are presented. The radio-chemotherapy schedule included 60Co irradiation with 20 Gy in 10 fractions over two weeks followed by multidrug chemotherapy with vincristine, bleomycin, methotrexate and leucovorin. Three series of radio-chemotherapy were given. In 14 patients the total dose of irradiation was increased to 70-105 Gy. The probabilities of overall and symptom-free survival at 2 years were 32% and 20%, respectively. Treatment results correlated with stage of the disease. Patients who received higher doses of radiation seemed to have better chances of symptom-free survival.

Benefits of improved glycemic control and lowered blood lipids continue as patients follow high-fiber, high-carbohydrate diets over time. In our experience, body weight, insulin requirements, glycemic control, and serum lipids are well managed by such diets for up to 10 years of follow-up. Besides minimizing the need for drugs, we found high-fiber, high-carbohydrate diets were well tolerated by diabetic patients. Other family members can consume the diet as well, since it closely approximates the dietary recommendations of the American the dietary recommendations of the American Heart Association and the National Cancer Institute.

TITLE: Adenocarcinoma in Meckel's diverticulum: case report and literature review.

A case of adenocarcinoma arising in Meckel's diverticulum in a 55 year old man is reported, and a brief review of the literature is presented. The patient developed low abdominal pain and showed elevation of serum carcinoembryonic antigen (CEA) level. The tumour was located in the apical portion of the diverticulum and extended into the mesenterium. Histologically, the tumour was a well-differentiated adenocarcinoma arising in the Meckel's diverticulum. Immunohistochemical study showed that malignant cells were positive for CEA. The noteworthy feature of this case is the pre-operative elevation of serum CEA level and the immunohistochemical demonstration of CEA in the cancer cells.

TITLE: [A case of lung cancer with bullous pemphigoid]

A 79-year-old man was admitted to our hospital for radiation therapy for lung cancer on July 24, 1984. In the course of radiation therapy, many bullas were recognized on his trunk. The lesion was diagnosed as bullous pemphigoid. To date, about 27 cases of malignant tumor with bullous pemphigoid have been reported in Japan. At present, the causal relation between malignant tumor and bullous pemphigoid has not been establishes. In our case, the bullous lesions disappeared during the course of radiation therapy, and the patient has been in good general condition for nine months without recurrence of metastasis of lung cancer.
