TITLE: Elevated serum levels of unsaturated folate binding protein: clinical correlates in a general hospital population.

We measured the capactiy of unsaturated folate binding protein to bind 3H-pteroylglutamic acid (UFBC) in serums from 489 general hospital patients. Of the patients 20% had mild-to-moderate elevations in UFBC (250 to 500 pg/ml), but only 6% had marked elevations (greater than 650 pg/ml), which correlated with metastatic cancer, active granulomatous disease, and especially with liver disease. The poor correlation of serum UFBC with folate status of the patient suggests that in many instances the elevated serum UFBC is neither the consequence nor the cause of folate deficiency. Although the role of folate binding protein remains unknown, it may be primarily an intracellular protein that is released into the serum as an index of activity of liver cells, granulocytes, and perhaps certain cancer cells.

The purpose of this study was to determine if measures of speaking fundamental frequency and its perturbation could be useful in differentiating talkers with no known vocal pathology and talkers with cancer of the larynx. Ten male subjects, five with a diagnosed malignancy of the larynx and five with normal voice, produced speech samples from which five voice production measures were obtained: the average speaking fundamental frequency (SFF), SFF variability during the reading of a sentence, the fo of a sustained vowel and a percent and magnitude jitter value. The perturbation factors, both directional and magnitudinal, during sustained vowels were found to be significant in discriminating normal talkers from those with laryngeal cancer. The speaking fundamental frequency and its variability during the reading of a sentence improved the discriminant function.

TITLE: Plasma prolactin levels and breast cancer: relation to parity, weight and height, and age at first birth.

Plasma prolactin has been measured in over 3,500 women volunteers from a normal population. In premenopausal women there was a significant decrease in prolactin levels with increasing parity. However, this effect was transitory since plasma prolactin concentration rose with increasing time after the birth of the last child. There were no significant differences in prolactin levels with respect to height and weight, although overweight compared to underweight women had approximately 15% more plasma prolactin. If prolactin is a carcinogen, then these results are in keeping with the epidemiological findings that multiparity affords protection and that age at last delivery is a risk factor in the development of breast cancer.

The level of lactate dehydrogenase has been reported to increase in a human breast cancer cell line as a result of estrogen action [4]. This finding suggests that lactate dehydrogenase may be useful in clinical human breast cancer as a marker for intactness of the estrogen receptor pathway and for probability of response to hormonal therapy. In 97 hunan breast cancer specimens submitted for estrogen receptor analysis, the level of lactate dehydrogenase varied from 0.19 to 3.3 I.U. per mg of extractable protein and bore no relation with either the presence of estrogen receptor or its level. It thus appears that lactate dehydrogenase is not a useful marker for endocrine responsiveness in clinical breast tumors.

TITLE: Results of a phase II study of maytansine in patients with breast carcinoma and melanoma.

During the phase I study of maytansine at our institution, some activity was observed against breast carcinoma and melanoma. A phase II study was thus initiated to more thoroughly investigate the activity of this drug against these two tumors. In 33 evaluable patients with melanoma, no complete or partial responses were observed. Twenty-one evaluable patients with breast cancer were entered and only one response (partial) was seen. The toxicity was similar to that observed in the phase I study and consisted mainly of diarrhea, paresthesias, phlebitis, and flu-like symptoms. Myelosuppression was infrequent and was short-lived when it occurred.

Ten preoperative cases of choledochal cyst and five patients who had had complications after surgical treatment for their choledochal cysts were studied to evaluate the accuracy of computed tomography (CT) in the diagnosis of this malformation and its complications. Choledochal cysts were accurately diagnosed by CT in seven and suggested in three of the 10 preoperative patients. CT provided the correct diagnosis in all six patients with intrahepatic involvement. The CT pattern of congenital dilatation of the intrahepatic ducts was strikingly different from that of an acquired obstructive one. CT was also effective in the postoperative follow-up patients. In addition, within this group of 15 patients, three choledochal cyst cancers were detected.

TITLE: Modification of muscle-preserving radical mastectomy.

A technical improvement of the muscle-preserving radical mastectomy for breast cancer is presented. In this procedure, the Sulcus interpectoralis, located between clavicular and sternocostal parts of the pectoralis major muscle, is split bluntly and spread apart. Then, the pectoralis minor muscle is severed near its attachment to the coracoid process and an axillary dissection is thereby easily and thoroughly accomplished. The effectiveness of lymph node dissection by this method was ascertained when the number of the lymph nodes removed by this procedure was compared with that removed by the conventional muscle preserving mastectomy (Madden's operation) and the radical mastectomy. Ninety-three patients treated by this operation have shown a satisfactory cosmetic appearance and a good prognosis when compared with patients treated by the standard radical mastectomy.

TITLE: Estrogen receptor concentrations in 269 cases of histologically classified human breast cancer.

This report provides a histological review of 264 female primary breast cancers analyzed for estrogen receptor protein (ER). We also describe 5 cases of male breast cancer all of which bound estradiol specifically. Generally the ER concentrations were lower in the tumors of premenopausal women than in those of postmenopausal women. Three types of cancer with specific morphological features were shown to have ER concentrations that differed significantly from the other types of cancer. Medullary and comedo carcinoma had very low and papillary carcinoma very high levels of ER. No obvious trend in ER concentration was found in unspecified ductal carcinoma, colloid carcinoma, nor in lobular carcinoma. A positive correlation was demonstrated between ER content and degree of differentiation in ductal carcinoma. Cancer with lymphoid infiltration generally showed low ER levels.

TITLE: The Brompton mixture versus morphine solution given orally: effects on pain.

The Brompton mixture is widely used as an effective method for controlling pain in cancer patients. In a double-blind crossover trial a standard Brompton mixture containing morphine, cocaine, ethyl alcohol, syrup BP and chloroform water was compared with morphine alone in a flavoured aqueous solution; both were administered orally. Pain was measured by means of the pain intensity index of the McGill Pain Questionnaire. Ratings of confusion, nausea and drowsiness were obtained from both the patients and their nurses and relatives. The data showed that there was no significant difference between the Brompton mixture and morphine administered orally for any of the variables. Both relieved pain effectively in about 85% of the patients.

Esophagorespiratory communication developed in 46 patients among 570 with esophageal cancer. Therapy was basically palliative and aimed at mechanical interruption of the fistula, restoration of esophageal continuity, and avoidance of external tubes and appliances. Supportive therapy, gastrostomy, tracheostomy, and esophageal exclusion and diversion procedures resulted in little prolongation of life and poor palliation of the patient. Permanent endoesophageal intubation with tubes of the Celestin variety resulted in best palliation with minimal operative risk for most terminal patients. Colon bypass and occasional resection can accomplish the same goal and possibly provide long-term survival in good-risk, young patients with small tumors.

TITLE: Familial influences on breast parenchymal patterns.

Some studies accomplished thus far have indicated that mammographic patterns may serve as risk indices for breast cancer. The present investigation was undertaken to determine whether patterns are familial. Mammograms from 110 mothers and their daughters and 122 sister pairs and a like number of unrelated controls matched for patient age, reproductive history, and personal and family history of breast cancer and drawn from the files of the Radiology Department of Hutzel Hospital, Detroit, Michigan, were compared. A familial influence was indicated by the finding that pattern similarities were significantly stronger in test cases than in unrelated controls. This influence appeared to be manifested by premature age changes in daughters and younger sisters and consisted, primarily, of a decrease in DY and an increase in P2 patterns. The data suggest that performing mammography on first degree relatives of women with high risk P2 and DY patterns should be considered.

TITLE: Defective monocyte killing in patients with malignancies and restoration of function during chemotherapy.

Spontaneous monocyte-mediated cytotoxicity (SMMC) in 34 patients treated with a chemotherapeutic regimen including cisplatin for various malignancies was depressed (median 7% 51Cr release) compared with SMMC in 31 normal controls (median 43%; p < 0.0001). Cytotoxicity in 7 patients with stage III or IV ovarian carcinoma was then followed during six cycles of chemotherapy with a cisplatin-containing regimen. All patients had initial cytotoxic values of < 8%, and in all SMMC increased at least three-fold between the third and fifth cycle of chemotherapy. This study lends support to the concept that cancer patients have depressed monocyte function. Cisplatin-containing chemotherapy seems to enhance monocyte function in vitro and in vivo.

TITLE: Bronchial cancer in Hong Kong, 1976-77: a preliminary report.

A case-control study of the relationship of bronchial cancer to smoking was conducted during 1976-77 in Hong Kong, in which we took particular note of the histologic type of the tumor. A total of 132 male and 111 female patients were interviewed. The association with smoking was more evident in males than it was in females, and squamous and small-cell type carcinomas, as a group, appeared to be more closely associated with smoking than did adenocarcinomas and large cell tumors. Of the women with bronchial carcinoma, 44% were nonsmokers; 79% of their tumors were adenocarcinomas or large cell carcinomas. For the women in this group, no association could be detected with place of residence, occupation, or use of kerosene for cooking.

The number of colony-forming units-spleen (CFU-S) and colony-forming units-culture in the femoral marrow of mice bearing an AKR transplantable leukemia remained relatively unchanged up to the death of the host despite the almost complete displacement of the normal differentiated hematopoietic cells by infiltrating leukemia cells. The proliferative state of the CFU-S was not changed significantly by the presence of the leukemia cells as determined by using 5-fluorouracil as a proliferative probe. The implications of these findings are discussed with regard to possible control mechanisms governing the interaction of leukemia cells with normal marrow cells and their potential importance for cancer chemotherapy.

The accuracy of diagnosis of 455 thyroid nodules evaluated by cytological and histological specimens obtained by fine- and large-needle biopsy, respectively, was compared with that for 1,094 nodules previously evaluated by clinical methods. The use of cytohistological data halved the number of patients with suspected cancer and doubled the number of patients to be observed. Cancers identified at operation for high and intermediate cancer-risk patients increased 75%, and operation for diagnosis of benign disease decreased 70%. Forty-two of 47 excised cancers were included in the cytohistological probable cancer group. Only 29 of these cancers were so classified clinically. Two cancers not diagnosed cytologically were suspected histologically and vice versa for one cancer. All 51 excised cytohistologically benign nodules were benign. There were more false-positive findings with cytological than with histological specimens.
