Clinical results of intraoperative radiotherapy (IOR) in carcinoma of the stomach and prostate, and malignant soft tissue tumors are reported. The 5-year survival rate was found to be increased by IOR in stages II-IV gastric cancer. From the analysis of the clinical results of prostatic cancer, a single dose of 3,500 rad was considered to be a potential curative dose for the tumor less than 3 cm in diameter. The local recurrence rate of patients with malignant soft tissue tumors who received a single dose ranging from 3,000 to 4,500 rad was 5.9 and the 5-year survival rate was 64.6%.

In a prospective study of 102 patients with pre-neoplasia and/or manifest squamous-cell carcinoma of the lower lip the synchronous occurrence of intra-oral, pharyngeal or laryngeal leukoplakia and carcinoma was assessed. There was a coincidence rate of almost 15% leukoplakias, including individual carcinomas. This high coincidence is probably due to a combination of various "risk profiles" in patients with threatened or manifest lower-lip cancer. Chronic exposure to sunlight, especially in the presence of a genetic disposition, is the most important risk factor in lower-lip cancer and its preceding stages, while chronic alcoholism and smoking abuse have little etiological significance.

Sixty-four patients with primary adenocarcinomas of the colon proximal to the peritoneal reflection underwent exploratory laparotomy at the time of first recurrence of their cancers. Local recurrence, retroperitoneal lymph node metastases, parenchymal liver metastases, and diffuse peritoneal seeding were identified as frequent components of the overall pattern of recurrence. The significance of these findings has been discussed herein, relative to refinements of operative technique and selection of appropriate adjuvant therapy.

We report two cases of herpes simplex pneumonia in children. One patient had Down syndrome, and the other was immunosuppressed by cancer therapy. Both had interstitial pneumonitis with nonspecific physical, radiographic, and laboratory findings, and both died. The diagnosis of herpes simplex pneumonia was made by isolation of herpes simplex virus from autopsy lung cultures as well as by demonstration of antigen in the tissue with an immunoperoxidase procedure. Inasmuch as herpes simplex pneumonia is a potentially treatable infection, early virologic studies are recommended in immunocompromised children with progressive pneumonitis of undetermined cause.

Laparoscopy is highly effective in diagnosing malignant liver disease, with overall accuracy in the 90% range. Up to 80% of the liver surface can be inspected, and biopsies can be directed with precision. A major advantage over scan guided percutaneous techniques is the ability to detect and biopsy lesions only a few millimeters in size on the liver surface. Laparoscopy is useful in staging the liver for metastatic disease during evaluation for treatment of primary cancers. Small peritoneal metastases may also be discovered and biopsied. Primary liver cancers and isolated metastatic deposits can be assessed for resectability, and diagnostic laparotomy can often be avoided. Laparoscopy is safely performed under local anesthesia and mild sedation.

The authors worked out methods of topometric investigation of cervical cancer patients during preparation for intracavitary irradiation using 252Cf fast neutron sources. The use of balloon catheters made it possible to determine a coefficient of projection enlargement in the point of interest, to determine accurately the distance between endostats, sources and contrasted organs of interest: the bladder and rectum. The results of the studies were used for individual design of intracavitary treatment as well as for the determination of radiation exposure of the pelvic organs in 38 patients. The patients received a total of 138 investigations.

The effects of Gn-RH agonists in advanced breast cancer patients have been examined. In both pre- and postmenopausal women they produce pituitary gland desensitisation and a fall in circulating concentrations of LH and FSH. In premenopausal patients plasma progesterone and oestradiol levels fall to the castrate or postmenopausal range within 3-4 weeks. Tumour remissions have been observed in approximately 30% of premenopausal women and approximately 10% of postmenopausal patients. The mechanism of action of Gn-RH agonists is discussed and their use projected to other tumour types.

One day after sunbathing, a breast cancer patient received intravenous methotrexate, cyclophosphamide and 5-fluorouracil and had a recall of her UV erythema over the following week. Phototesting with UVA and UVB prior to and after a subsequent chemotherapy treatment showed a UVB-induced recall of erythema, as well as a phototoxicity-like response. Skin biopsies from test sites were compatible with phototoxic dermatitis. The occurrence of both erythema recall and the phototoxicity-like reaction suggests that more than one mechanism is involved in this side effect.

Correlation between c-myc gene expression and phosphatidylinositol (PI) metabolism was studied using seven human kidney cancer cell lines. We found that the exceptional incorporation of 32P into CDP-diacylglycerol (CDP-DG) was observed in only two cell lines, in which c-myc expression was increased among seven cell lines tested. In these two cell lines, PI was also labeled. The other five cell lines indicated the accumulation of radioactive PI, whereas CDP-DG was unlabeled.

Pelvic lymphocysts occur infrequently after radical surgery for cervical cancer. The problem is one of recognition. In a retrospective analysis of patients seen over 10 years in this department, the incidence was 25.3%. All were diagnosed by bimanual, clinical examination only. The only factor that appears to relate to the formation of lymphocysts is the presence of poorly differentiated tumors; 70.8% of patients developing lymphocysts had poorly differentiated tumors. All the lymphocysts in this series resolved spontaneously and did not produce any major complications.

Radiation-induced osteoradionecrosis of the ribs and brachial plexus lesions following irradiation of breast cancer patients are well known phenomena and have been reported several times in the literature. Damage to the shoulder joint after irradiation is uncommon. The case of a 75-year-old woman with breast cancer is described; she first developed a brachial plexopathy and then necrosis of the head of humerus after therapy with combined ortho-voltage and high-voltage radiation. The differential diagnosis and possible pathogenetic mechanisms are discussed.

Primary liver cancer has been induced experimentally in female Wistar rats by diaminoazobenzene intoxication or subcapsular liver implantation of Novikoff cell suspensions. The ultrasonic B-scan image of these malignant tumours are described via statistical texture parameters in respect of image brightness, microtexture and macrotexture. A discrimination of the tumour texture profiles is possible with an accuracy of about 80%. There are correlations between ultrasonic image parameters and histomorphological tissue components but there is no specific link between the histological tumour-type and these B-scan data.

Thirty-seven patients with advanced ovarian cancer were treated in a prospective, randomised trial comparing chlorambucil with a combination of cyclophosphamide and cis-diamminedichloroplatinum (cis DDP). Treatment with the combination was associated with an increased overall response rate (69% versus 23%) (p = 0.04). A response was associated with a longer median survival but no survival advantage was demonstrated for patients who received cyclophosphamide/cis DDP. A larger study is needed to evaluate fully the potential of regimens containing cis DDP in the management of advanced ovarian cancer.

Studies are reviewed which relate to five basic content areas of research concerning depression and cancer: significant loss experience, emotional inhibition, hopelessness, psychiatric assessment of depression, and test measurement of depression. Methodological issues within each area are addressed as they relate to the potential validity and generalizability of findings. In general, there is no support for increased loss experience in cancer patients and mild support for increased emotional inhibition and hopelessness in these individuals. Traditional psychiatric assessment approaches are not seen as providing solid evidence for a depression/cancer relationship due to reliability and design issues, while psychometric assessment provides mild support for a prospective relationship between depression and later cancer. Issues relating to approaches to research, means of measurement of depression, design issues, and questions of conceptualization of depression are discussed.

TITLE: Radiotherapy: what are patients' needs?

Although there is a substantial body of research into the impact of cancer therapies such as surgery and chemotherapy, little is known about the experiences of cancer patients receiving radiotherapy. Serial interviews with 39 patients undergoing radical radiotherapy to the oral cavity revealed that misunderstandings and groundless fears about this form of treatment were widespread. A majority were unprepared for the severity and duration of their side-effects. These results suggest that written information for patients about radiotherapy and its side-effects should be made available routinely. There is also scope for nutritional support. Finally, as a supplement to out-patient attendance, informal contact with the hospital in the immediate post-treatment phase would provide invaluable reassurance to patients.
