The sensitivity of cytology for detection of cervical intraepithelial neoplasia (CIN) is effectively augmented by cervicography with the disadvantage of lower specificity. However, the combination of cytology and cervicography can decrease the number of recalls, biopsies, and unnecessary treatments, which reduces cost. Down-staging of invasive cancer by visual inspection seems a cost-effective alternative to the introduction of cytology in countries with limited health facilities. Other methods such as speculoscopy, polarprobe, laser-induced fluorescence, computerized digital imaging colposcopy, or computer imaging have a potential to be used for future screening or triaging; however, so far, insufficient data are available to evaluate the validity of these techniques.

The genes responsible for each of the inherited syndromes of intestinal polyposis and colorectal cancer are now known. This knowledge has led to genetic testing for syndrome diagnosis and for determining which persons in an affected family actually have the condition. Genetic testing has also allowed a more precise clinical characterization of each of the syndromes and their subtypes. Optimal application of genetic testing, proper cancer prevention, and management of nonmalignant features of each of the syndromes are the next goals. This report summarizes each of the syndromes, their genetics, and management and provides an approach for genetic testing.

A case of calcific tendinitis of the gluteus medius is presented. This report describes a patient with a history of breast cancer who had the combination of amorphous calcifications in the gluteus medius tendon and the MR finding of conspicuous bone marrow edema in the adjacent greater trochanter, prompting concern for metastatic disease. We present images from radiography, bone scanning, CT, and MR imaging. The unusual combination of findings in these studies should be considered conclusive for calcific tendinitis, and should not be confused with malignancy.

Prostate cancer is a frequent concern of the clinician caring for older male patients. The certainty of arriving at the correct diagnosis is related to the presenting patient's risk for prostate cancer, the results of the digital rectal examination, and the value of the serum prostate-specific antigen (PSA). A case report of a patient with acute urinary retention and an elevated PSA is presented. Possible explanations for the elevated PSA are discussed. The clinician's intuitive thought process is compared with an analytic approach using calculated probabilities. Several factors that complicate the estimation of the likelihood of prostate cancer are discussed.

To clarify the range of potential etiologies that may contribute to organic brain syndrome in patients receiving systemic opioids for cancer pain, we describe 15 patients who presented this complication. In 11 cases, concomitant conditions were found that could contribute to the onset of organic brain syndrome. These data illustrate that multiple causes often play a role in the development of mental status changes in advanced cancer. Opioids are seldom the only causal factor implicated.

We report a NSCLC patient effectively treated with anti-cancer agents on an outpatient basis despite chemotherapy with carboplatin and docetaxel, he relapsed after achieving a transient response. Combination chemotherapy with gemcitabine and vinorelbine administered every three weeks in an outpatient clinic caused the lung cancer volume to reduce remarkably. This regimen was well tolerated and suitable for outpatients. Its usefulness as a second line chemotherapy in the treatment of NSCLC should be investigated.

About 40 percent of newly diagnosed breast cancers occur in women older than 65. Hormonotherapy plays an important role as adjuvant treatment of primary cancers in post-menopausal women because of their great hormone-dependence. The drug of choice is tamoxifen which is very well tolerated. Its beneficial effect appears to be related to the duration of administration. It also seems to be particularly promising in the initial treatment (neoadjuvant) of elderly women and potentially beneficial in the breast cancers said to be of good prognosis when the axillary nodes are not involved.

A 16-year-old girl with acute lymphocytic leukemia in hematologic remission and no evidence of active central nervous system disease had a unilateral hypopyon as her sole site of relapse. She underwent diagnostic paracentesis and was treated with topical corticosteroids, chemotherapy, and radiotherapy to the central nervous system. Ocular involvement of the iris and anterior chamber as the site of primary disease or a relapse is a rare event. The anterior chamber serves as a sanctuary for cancer cells, and treatment protocols, including better anterior chamber prophylaxis, are warranted.

Clinical usefulness of cytokines in cancer treatment has been described. Cytokines presently used in clinical practice are IFNs, IL-2, and hematopoietic growth factors. IFNs and IL-2 used with an expectation of direct antitumor effect of these cytokines have been proven to be specifically effective against certain types of tumors. Hematopoietic growth factors have been used as the adjuvant drugs in cancer treatment and proven to be effective against neutropenia and associated infections after chemotherapy and bone marrow transplantation. Future development of new cytokines and trials on the combination among cytokines and cytokines with chemotherapeutic agents, will promote the usefulness of cytokines in cancer treatment.
