TITLE: Epidural administration of opiates by a new device.

Epidural administration of opiates as a long-term treatment of cancer pain, even for out-patients, is now well established. Most reports describe intermittent injections given several times a day, which may have technical and personal disadvantages. Continuous epidural infusion may be preferable. This report describes 16 patients who were treated with epidural opiates delivered by plastic infusion pumps. Pain relief was effective, the equipment was inexpensive and home treatment was easily accomplished.

TITLE: [Complete remission of stage III penile cancer by multidisciplinary treatment: report of a case]

A case of penile cancer with bilateral metastatic inguinal lymph nodes in a 68-year-old male is reported. We used combination chemotherapy with methotrexate, bleomycin, and cisplatin for 2 courses before ilioinguinal lymphadenectomy. The surgical specimen of ilioinguinal lymph nodes revealed no viable tumors. It is clear that this combination therapy was effective against metastatic penile cancer and the role of preoperative chemotherapy is becoming increasingly important in metastatic penile cancer. This chemotherapy and surgical approach deserves additional trial in patients with metastatic penile cancer.

Forty-two evaluable patients with advanced non-small cell lung cancer were treated with teniposide at doses ranging from 120 to 180 mg/m2 on Days 1, 3, and 5 every 3 weeks. Thirty-four patients had received no prior chemotherapy. Seven partial responses (16.6%) were obtained (21% in chemotherapy-unexposed patients). Marrow toxicity was the main side effect: life-threatening thrombocytopenia occurred in 9% of patients, and 54.5% experienced severe leukopenia. Teniposide, at the doses and schedule employed, has moderate activity in non-small cell lung cancer.

Patients with specific types of cancer have achieved substantial prolongation of survival after successful cancer chemotherapy. Furthermore, cancer chemotherapeutic agents are being widely used as immunosuppressive drugs in patients with benign conditions. Both of these patient populations are at risk for development of late complications of treatment. Though the late effects of chemotherapy are less well defined, they are insidious in onset and not manifested clinically until damage have become overt and irreversible. Many reports on the late effects of chemotherapy demonstrate the importance of assessing not only the therapeutic results of various treatment program but also of evaluating the long-term complications of therapy in order to maximize the benefit of the treatment. Thus, skillful and judicious application of chemotherapy produces a minimum of delayed consequences.

Twelve variants of uterine cavity structure were defined on analysis of 52 hysterocervicograms performed in patients with cancer of the uterine body. Using the method of linear programming, the most effective schemes for the location of radiation sources were proposed to ensure optimum dose distribution in intracavitary irradiation. Sources I and II were the main contributors to dose distribution. The use of sequential radiation sources was appropriate in considerable sizes of the uterine cavity. In a spherically shaped uterine cavity dose distribution could be ensured by using only one source, located in the center.

I discuss the impact of individual heterogeneity in survival analysis. It is well known that this phenomenon may distort what is observed. A general class of mixing (or frailty) distributions is applied, extending a model of Hougaard. The extension allows part of the population to be non-susceptible, and contains the traditional gamma distribution as a special case. I consider the mixing of both a constant and a Weibull individual rate, and also discuss the comparison of rates from two populations. A number of practical examples are mentioned. Finally, I analyse two data sets, the main one containing data from the Norwegian Cancer Registry on the survival of breast cancer patients. The statistical analysis is of necessity speculative, but may still provide some insight.

Based on our 14-year experience with breast-preserving treatment of breast cancer, we recommend limited surgical intervention for the removal of a small mammary carcinoma under the proviso that adequate radiotherapy is available and that the patient can be thoroughly followed up. We are convinced that the good cosmetic result and especially the excellent psychological reaction are particularly important in order that women lose their fear of an ablative operation. Only then, by performing an operation which is not excessive at the earliest possible time can the prognosis of this type of carcinoma, which is increasing be improved.

We present the results of 252 pelvic exenterations for primary and recurrent carcinoma of the cervix at the Hospital General de Mexico, a tertiary-care institution for the indigent. Emphasis is placed on the morbidity and mortality of the procedure in relation to patient selection. In underdeveloped countries, where early detection of cervical cancer is a rare event, pelvic exenteration must continue in the armamentarium of physicians; it can be associated with gains in the quality of life, with long-term survival, with effective rehabilitation, and possibly with cures.

Whether patients over age 40 should have a barium contrast enema (BCE) examination for possible identification of an unsuspected colon carcinoma before elective inguinal herniorrhaphy remains an unanswered question. We reviewed the medical records of all patients over age 40 who had inguinal herniorrhaphy at our institution between January 1980 and December 1984. Of 80 patients, 46 received a preoperative barium enema. One asymptomatic colon carcinoma (2.1%) was discovered. The reported incidence of asymptomatic colon cancer found by BCE in association with inguinal hernia ranges from 0 to 2.5%. We conclude that if there is a cause and effect relationship between asymptomatic colon cancer and inguinal hernia, the mechanism is as yet unknown.

Although such an approach is imperfect, the results support the belief that the screening programme has been largely unsuccessful.

Cancer may develop in connection with rare, genetic diseases or after exceptional, environmental exposures. Most commonly, however, cancer results from an unhappy coincidence of normal, genetic factors and ordinary, environmental conditions. Oncogenes are normal growth genes. Anti-oncogenes are normal regulatory genes which control growth genes. Cancer may result from changes (mutations) in an oncogene (such as in the Philadelphia chromosome), or from the lack of control of normal oncogenes. The latter may be due to loss of function of a suppressor gene (mutation in an anti-oncogene, such as in retinoblastoma) or through a change in the position of an oncogene (such as in Burkitt's lymphoma and in certain leukemias).
