Radiotherapy has played an important role in the curative and palliative treatment of cancer and should continue to do so in the future. As the radiotherapist is concerned with the overall management of the cancer patient, he should be a well trained clinician. Notable advances and improvements in apparatus have taken place recently. Better treatment planning, new forms of after loading techniques, neutron therapy, hyperthermia and other interesting developments are also discussed. With these advances, problems of radiological protection are reviewed and recommendations made from time to time by the International Commission and Radiological Protection to ensure the safety of both staff and patients.

Parabiosis of paired NEDH rats and subsequent irradiation of one produced multiple stresses, particularly altered hormonal milieu. Benign mammary tumors developed in 8.6% of the control pairs, 12.7% of the shielded partners, and 19.7% of the irradiated rats. Mammary cancers developed in 5% of control pairs, 14.2% of the shielded partners, and 11.9% of the irradiated rats. With increased amounts of prolactin, the rate was 50% in shielded partner, 10% in the irradiated rats. Mesenchymal sarcoma develop at the anastomosis in parabiont rats. Their incidence rate in single controls is 3.1%, in parabiont control pairs 12.5%, in pairs with one partner irradiated 16.6%, and with both irradiated 23.1%. The incidence rate in other parts was increased threefold over controls in the irradiated partners. Radiation appears to be the dominant factor in both mammary and anastomotic tumors.

TITLE: Estradiol receptors in subpopulations of breast cancer cells isolated from human primary tumors.

Cancer cells isolated from biopsy specimens from seven women with primary breast cancer were analyzed for estradiol receptors. The cells were separated into subpopulations using the techniques of velocity centrifugation, buoyant density centrifugation, and density gradient electrophoresis. It was found that only large breast cancer cells (with diameters between 10.1-18.5 micrometers) concentrate tritiated estradiol. The total population of cancer cells has diameters between 5.6-18.5 micrometers, and the proportion of large cancer cells from different tumors varies from 8-59% of the total population. It is suggested that clinical regression following endocrine ablation is related to the proportion of large cancer cells and to the concentration of estradiol receptors in these cells.

TITLE: Auriculotemporal syndrome after combined modality therapy for cancer.

Auriculotemporal syndrome frequently follows trauma to the auriculotemporal nerve, a branch of the mandibular nerve. Classic symptoms are gustatory sweating and hyperemia, with pain occurring in less than 10% of cases. We have treated six patients for a variant of auriculotemporal syndrome following combined modality therapy for head and neck cancers. Unique was the presence of pain and the absence of gustatory sweating in all cases. The combination of radical surgery, chemotherapy, and irradiation obscured the "textbook" presentation, leading to initial misdiagnosis. Long-term relief (one year) from this atypical presentation of auriculotemporal syndrome was afforded by a simple mandibular nerve block, a technic that is of no sustained therapeutic benefit in the classic cases.

The nutritional state of patients with malignant disease must be assessed at the beginning of treatment. Nutritional supplements, given either orally or intravenously, enable patients to tolerate larger doses of chemotherapy and radiotherapy and the clinical response may be enhanced by nutritional support. Crystalline L aminoacids and glucose are the main nutrients used intravenously and fat emulsions are required in prolonged parenteral nutrition. Hypophosphataemia related to glucose administration and infection related to access to the circulation are the main complications, both of which are preventable. Total parenteral nutrition alone will not improve the results of cancer treatment significantly but its careful use in association with surgery, radiation therapy and chemotherapy can be most valuable.

Substantially different conclusions have been reached by several studies about relationships between mammary cancer and obesity. We studied retrospectively 106 consecutive patients who underwent mastectomy for breast cancer. We found no association between obesity per se and the frequency or time of recurrence. Our sample did reaffirm that recurrence of breast cancer is related to tumor size and nodal status. Obesity was similarly related to tumor size and nodal status. However, we could not confirm prior findings that obese patients with pathologically similar breast tumors had a poor prognosis.

Gastric cancer remains difficult to cure except in its superficial spreading stage, that is when it is mucosal or submucosal without any lymph node involvement. Its detection rate should reach 20% of all cancers. Three anatomical types are described: the malignant polyp, the muco-erosive cancer and the ulcero-cancer. The most efficient radiological procedure is fibroscopy rather than radiology: it permits biopsy and brush cytology, two procedures which yield 95% of positive diagnosis. The most frequent precancerous lesion seems to be chronic gastritis and especially gland dysplasia. Early detection may be made easier by defining a poor risk population: gastric ulcer, polyadenoma, partial gastrectomy going back to more than fifteen years, and dyspeptic patients in whom biopsy discovers atrophic gastritis. Such a population should be regularly followed up.

TITLE: Morphogenesis of peritoneal metastasis in human gastric cancer.

A comparative light microscopic and scanning electron microscopic study of the morphogenesis of peritoneal metastasis in 34 human gastric cancers was performed. Prior to adhesion of gastric cancer cells to the peritoneum, the mesothelial cells became hemispherical and exfoliated from the peritoneum, and gastric cancer cells adhered to the naked areas of the submesothelial connective tissue. A flat metastatic tumor was formed by cancer cell proliferation in the shallow region of the peritoneum. Subsequently, after the infiltration of cancer cells into the connective and adipose tissue, the formation of a large tumor mass was observed. There was a correlation between the surface and histological structure of the metastatic tumors. In poorly differentiated cancer, the cells were isolated while in differentiated cancer, they formed nodules with indistinguishable cell boundaries.

TITLE: Activity of indicine N-oxide in refractory acute leukemia.

Indicine N-oxide, the first pyrrolizidine alkaloid N-oxide to be studied in the treatment of cancer in humans, was administered to ten patients: four children and two adolescents with refractory acute lymphocytic leukemia and four adults with refractory acute nonlymphocytic leukemia (three acute myelocytic, one myelomonocytic). Two patients, a 4-year-old boy with acute lymphocytic leukemia and a 22-year-old man with acute myelocytic leukemia, achieved complete remission lasting 3 and 5+ months, respectively. Another 15-year-old male with acute lymphocytic leukemia had a partial remission for four months. Toxicities included bone marrow suppression, mild anorexia and nausea, and transient elevation of liver enzymes. Jaundice and liver failure, presumably induced by drug, occurred in two patients.

Estrogen receptor assays were performed on tumor specimens from 6 male patients with documented breast cancer. Five out of six (83%) males had positive ER assays with values ranging from 23-384 femtomoles per mg cytosol protein. A review of the literature showed that 32 out of 38 patients (84%) reported were positive for estrogen receptor. Four out of five ER positive males responded to orchiectomy. Estrogen receptors are present in a high percentage of males with breast cancer and may predict response to treatment, similar to female breast cancer.

TITLE: A cancer registry for the community radiation center.

A common problem for cancer treatment centers is maintenance and utilization of the cancer registry. Computerization of the large amount of patient information involved is desirable. Organization and implementation of a patient registry using a PC-12 treatment planning computer is described. This application is reasonable and effective for community radiation centers, and retains the interactive features that make these dedicated mini-computers so useful. Details of organizing and running this type of registry are included. Examples of the patient data from a radiation therapy department are given and the possibilities for analysis of the information discussed. An important part of the registry is the one-line summary of individual cases, termed the "patient status graph," that condenses patient follow-up history for quick review. The importance of a pertinent and practical registry for all cancer centers is discussed.

This article describes results of postmastectomy rehabilitation program as operating at Mt. Sinai Hospital of Cleveland, a general community facility. Common physical and emotional problems following mastectomy are documented, as well as barriers to effective rehabilitation--particularly the fact that such programs are usually associated with specialized oncology settings and not seen as feasible in the community hospital. An analysis of outcomes of a program operated at both Mt. Sinai and Memorial Sloan-Kettering Cancer Center demonstrates that comparable and positive patient outcomes can be achieved in terms of range of motion, resumption of prior activities, and emotional stress. Recommendations are made regarding ways to overcome common problems in implementing such programs in an inexpensive and effective manner.

Combined serum and bone marrow radioimmunoassays for prostatic acid phosphatase provide a unique means for the early diagnosis and more accurate clinical staging of prostatic cancer. The combined screening technique appears to be helpful, particularly in providing a clinical assessment of the presence or absence of early, subclinical lymphatic and bone marrow metastases. Low titer elevations of bone marrow prostatic acid phosphatase by radioimmunoassay have been observed commonly in clinically understaged C prostatic cancer with normal technetium bone scans, indicating the presence of unrecognized stage D disease with bone metastases. The combined screening method also is of distinct clinical value in the early diagnosis of prostatic cancer and in monitoring the effects of specific therapy. In therapeutically responsive patients marked suppression of serum and bone marrow prostatic acid phosphatase is observed regularly with the radioimmunochemical method under study.

When hyperthyroidism results from active thyroidal secretion, both serum hormonal levels and the radioactive iodine uptake (RAI) are elevated. Discordant results (increased serum hormone levels and a low RAI) are found either in the usual forms of hyperthyroidism when large quantities of iodide are ingested, or in atypical forms of hyperthyroidism, including spontaneously resolving hyperthyroidism of subacute thyroiditis, thyrotoxicosis factitia, toxic struma ovarii, and functioning metastatic thyroid cancer. Two patients are described who exemplify pitfalls in the interpretation of thyroid function test results in atypical hyperthyroidism. The following guidelines are helpful in avoiding these pitfalls: (1) An RAI should be performed in all thyrotoxic patients to confirm the presence of thyroidal hypersecretion, rather than atypical hyperthyroidism. (2) A positive response to thyroid-stimulating hormone stimulation is an important finding differentiating the other atypical forms of hyperthyroidism listed from subacute thyroiditis.

A comparison study was made of staining for immunoglobulin by immunofluorescence and immunoperoxidase in 23 lymph node biopsies. Cryostat sections were stained for immunoglobulins using a direct technique while paraffin-embedded sections were stained for immunoglobulins using the peroxidase-antiperoxidase (PAP) technique. Of 23 cases studied, including seven reactive hyperplasias and 16 lymphoid cancers, there was agreement between the two techniques in 18 cases. Nine of 14 non-Hodgkin's lymphomas showed agreement of results. All reactive hyperplasias showed complete agreement and had a distinctive pattern by both immunofluorescence and immunoperoxidase. Fixation, reagent specificity, and sensitivity of the methods may account for some of the discrepancies noted. Immunoperoxidase staining may be a useful diagnostic aid, particularly when immunofluorescent staining is not possible.
