Epidural administration of opiates as a long-term treatment of cancer pain, even for out-patients, is now well established. Most reports describe intermittent injections given several times a day, which may have technical and personal disadvantages. Continuous epidural infusion may be preferable. This report describes 16 patients who were treated with epidural opiates delivered by plastic infusion pumps. Pain relief was effective, the equipment was inexpensive and home treatment was easily accomplished.

Intravenous urography with total body opacification, and tomography as required, often give the most information toward evaluating abdominal masses in children. Ultrasonography is a noninvasive procedure which defines normal structures and differentiates cystic and solid tumors. The combination of these studies gives sufficient information about renal tumors to plan for possible surgery. Arteriography is not necessary for the diagnosis of Wilms' tumor, nor its surgical or medical management. Pseudotumor of the kidney is due to focal cortical hyperplasia. It can be diagnosed by nephrotomography, renal arteriography or renal scanning. The latter method is most accurate and has the lowest morbidity. Aortography is advisable in the evaluation of a patient with pheochromocytoma in an attempt to locate multiple tumors. Determining the extent of abdominal neuroblastoma by angiography and lymphangiography does not appear to influence the mode of therapy, not the survival rate; therefore, invasive diagnostic procedures do not appear to be indicated in neuroblastoma. Angiography is necessary in the evaluation of liver cancer. If one lobe is determined to be free of disease, lobectomy is a possible cure. Splenic cysts and choledochal cysts can be diagnosed by noninvasive methods such as ultrasonography or radioisotope scanning. Arteriography and percutaneous opacification are not necessary to make these diagnoses.

A total retropubic prostatectomy is an exact, clearly defined operation, which, in our hands, has been extensively standardized: it lasts an average of 1 1/2 h, results in relatively little blood loss-in only 20% of the cases is one or more units of whole blood utilized-and is accompanied by complications that are kept within tolerable boundaries. Complete enuresis can be avoided by using a particular stitching technique for the outlet of the bladder. A considerable incontinence occurred in only one patient, who during the day was temporarily forced to wear a Cunningham clamp. In the postoperative stationary phase, mortality was 2%. A radical prostatectomy makes complete recovery from cancer of the prostate possible without mutilation and without aggravating functional injuries. It has, therefore, a secure place in the surgical therapy of cancer.

Rats and hamsters were exposed to 0.1 ppm bis(chloromethyl)ether (BCME) six hours per day, five days per week throughout their lifetime. Additional groups of rats were given 10, 20, 40, 60, 80, and 100 exposures to 0.1 ppm BCME and then held until death. Forty cancers originating in the respiratory tract were found in the 200 rats involved in these studies. These included 14 cancers of the lung and 26 cancers of the nasal cavity. They occurred in dose-related fashion. A single undifferentiated carcinoma of the lung was seen in a hamster.

The overall structure, previously determined by X-ray analysis of an N-bromoacetyl derivative (Anguili, R., Foresti, E., Riva Di Sanserverino, L., Isaacs, N.W., Kennard, O., Motherwell, W.D.S., Wampler, D.L. and Arcamone, F. (1971) Nat. New Biol. 234, 78-80) has been confirmed, although substantial conformational differences are observed. The conformation described here is very similar to that found for the related drug carminomycin I (Wani, M.C., Taylor, H.L., Wall, M.E., McPhaill, A.T. and Onan, K.D. (1975) J. Am. Chem. Soc. 97, 5955-5956; Pettit, G.R., Einck, J.J., Herald, C.L., Ode, R.H. Von Dreele, R.B., Brown, P., Brazhnikova, M.G. and Gause, G.F. (1975) J. Am. Chem. Soc. 97, 7387-7388); it is suggested that this represents a significantly stable molecular conformation; an intramolecular C(7)...O(9) hydrogen bond is invoked to account for this. This conformation is likely to be at least close to that of daunomycin when bound to DNA.

Bovine leukosis describes lymphatic cancers of cattle. The most common form of this disease occurs in adult animals and is caused by bovine leukemia virus. Infection is widespread in the United States, especially in dairy cattle, but the virus produces tumors in only a small percentage of infected animals. Nevertheless, bovine leukemia virus has been receiving attention from the dairy industry because of its importance in health certification of cattle or semen intended for export. Another source of concern is whether bovine leukemia virus poses any risk to human health. These problems are discussed in the light of recent technological advances in tumor virus research and specifically regarding our current understanding of the biology of bovine leukemia virus.

A case of penile cancer with bilateral metastatic inguinal lymph nodes in a 68-year-old male is reported. We used combination chemotherapy with methotrexate, bleomycin, and cisplatin for 2 courses before ilioinguinal lymphadenectomy. The surgical specimen of ilioinguinal lymph nodes revealed no viable tumors. It is clear that this combination therapy was effective against metastatic penile cancer and the role of preoperative chemotherapy is becoming increasingly important in metastatic penile cancer. This chemotherapy and surgical approach deserves additional trial in patients with metastatic penile cancer.

Parabiosis of paired NEDH rats and subsequent irradiation of one produced multiple stresses, particularly altered hormonal milieu. Benign mammary tumors developed in 8.6% of the control pairs, 12.7% of the shielded partners, and 19.7% of the irradiated rats. Mammary cancers developed in 5% of control pairs, 14.2% of the shielded partners, and 11.9% of the irradiated rats. With increased amounts of prolactin, the rate was 50% in shielded partner, 10% in the irradiated rats. Mesenchymal sarcoma develop at the anastomosis in parabiont rats. Their incidence rate in single controls is 3.1%, in parabiont control pairs 12.5%, in pairs with one partner irradiated 16.6%, and with both irradiated 23.1%. The incidence rate in other parts was increased threefold over controls in the irradiated partners. Radiation appears to be the dominant factor in both mammary and anastomotic tumors.

Forty-two evaluable patients with advanced non-small cell lung cancer were treated with teniposide at doses ranging from 120 to 180 mg/m2 on Days 1, 3, and 5 every 3 weeks. Thirty-four patients had received no prior chemotherapy. Seven partial responses (16.6%) were obtained (21% in chemotherapy-unexposed patients). Marrow toxicity was the main side effect: life-threatening thrombocytopenia occurred in 9% of patients, and 54.5% experienced severe leukopenia. Teniposide, at the doses and schedule employed, has moderate activity in non-small cell lung cancer.

TITLE: Estradiol receptors in subpopulations of breast cancer cells isolated from human primary tumors.

Cancer cells isolated from biopsy specimens from seven women with primary breast cancer were analyzed for estradiol receptors. The cells were separated into subpopulations using the techniques of velocity centrifugation, buoyant density centrifugation, and density gradient electrophoresis. It was found that only large breast cancer cells (with diameters between 10.1-18.5 micrometers) concentrate tritiated estradiol. The total population of cancer cells has diameters between 5.6-18.5 micrometers, and the proportion of large cancer cells from different tumors varies from 8-59% of the total population. It is suggested that clinical regression following endocrine ablation is related to the proportion of large cancer cells and to the concentration of estradiol receptors in these cells.

Patients with specific types of cancer have achieved substantial prolongation of survival after successful cancer chemotherapy. Furthermore, cancer chemotherapeutic agents are being widely used as immunosuppressive drugs in patients with benign conditions. Both of these patient populations are at risk for development of late complications of treatment. Though the late effects of chemotherapy are less well defined, they are insidious in onset and not manifested clinically until damage have become overt and irreversible. Many reports on the late effects of chemotherapy demonstrate the importance of assessing not only the therapeutic results of various treatment program but also of evaluating the long-term complications of therapy in order to maximize the benefit of the treatment. Thus, skillful and judicious application of chemotherapy produces a minimum of delayed consequences.

TITLE: Auriculotemporal syndrome after combined modality therapy for cancer.

Auriculotemporal syndrome frequently follows trauma to the auriculotemporal nerve, a branch of the mandibular nerve. Classic symptoms are gustatory sweating and hyperemia, with pain occurring in less than 10% of cases. We have treated six patients for a variant of auriculotemporal syndrome following combined modality therapy for head and neck cancers. Unique was the presence of pain and the absence of gustatory sweating in all cases. The combination of radical surgery, chemotherapy, and irradiation obscured the "textbook" presentation, leading to initial misdiagnosis. Long-term relief (one year) from this atypical presentation of auriculotemporal syndrome was afforded by a simple mandibular nerve block, a technic that is of no sustained therapeutic benefit in the classic cases.

There are abundant experimental and clinical data to associate radiation exposure and thyroid cancer. The incidence of thyroid cancer increases with increasing doses of thyroidal radiation from 6.5 rads to 1,500 rads, but higher doses tend to destroy the gland and are associated with hypothyroidism rather than cancer. The peak occurrence of thyroid tumors is between five and 30 years after exposure, but a person may develop thyroid tumors as long as 50 years after irradiation. Multiple thyroidal lesions or disorders, including adenomas, thyroditis, and hypothyroidism as well as malignant neoplasms, can occur after radiation exposure. It is imperative, therefore, to study carefully all patients with a history of radiation exposure to the gland and to observe them carefully for life. Suggestions are made for the management of thyroid disease in these patients.

TITLE: Gastric malignancy: ten year experience at a community teaching hospital.

A ten-year review of gastric malignancy was undertaken and several factors examined. The degree of organ involvement played a significant role in determination of survival. Surgical resection, either palliative or curative, was associated with a more favorable short-term survival and bypass alone appeared to add nothing to patient survival time. The general lack of symptoms in early gastric cancer appears to be a majority determinant of the advanced state of malignancy in these patients. Early screening may represent a partial solution to this dilemma.
