Both individual and community-based interventions have been effective in changing sun exposure knowledge and attitudes but generally have not been effective in changing behaviors. An integrative model of behavior change is described that has been successful in changing behavior across a wide range of health conditions. This model holds promise for developing a rational public health approach to skin cancer prevention based on sound behavioral science.

TITLE: Synthesis and biological properties of a 1-(6-aziridinylhexyl)-2-phenylindole, a potential fluorescence label for estrogen receptors.

2-Phenylindole derivative 8, linked to aziridine by a hexamethylene spacer group, was synthesized and tested. It binds to the estrogen receptor with an RBA-value of 4.0 (estradiol = 100). In vitro, it shows a selective cytostatic activity in hormone-dependent MCF-7 breast cancer cells (41% T/C at 10(-6) M). In vivo, it exerts a strong estrogenic effect. The binding to the estrogen receptor is largely reversible.

From 1980 to 1988 more than 1,549 organ donors were harvested, 125 of them (8%) died from cancer--116 brain tumors and 9 donors with extracerebral cancer. In 3 donors suffering from extracerebral cancer 5 kidneys were transplanted. All transplants functioned well without tumor signs up to 21 months after grafting. However, because of tumor recurrence in nearly 50% of the cases described in the literature generally organs from donors suffering from extracerebral cancer should not transplanted if possible.

The purpose of this literature review is to examine potential strategies for dealing with positive screening test results. If positive results are obtained, current data support the use of colonoscopy as the single most effective diagnostic strategy. We evaluated the relation between forced expiratory volume in 1 second (FEV1) and lung cancer incidence and mortality among members of the Kaiser Permanente Medical Care Program who took a multiphasic health checkup. FEV1 was inversely related to risk of lung cancer among former and current smokers, but not related among never-smokers. We observed a decreased risk of lung cancer mortality only in the higher quintiles of FEV1 in current smokers among men, but not in women. FEV1 appears to be associated with lung cancer as a physiologic marker for smoking-induced pulmonary damage.

Of the 140,000 cases of colorectal cancer anticipated this year, approximately 2% to 6% will occur in patients less than 40 years old. Data regarding colon cancer in this age group applies directly to the similarly aged active duty population. The problem is illustrated by two case histories. A review of the literature indicates that patients of this age group may have a worse prognosis, not due to age per se, but likely due to a combination of an advanced stage at diagnosis and a higher percentage of poorly differentiated or mucin-producing tumors. Early diagnosis remains essential for improved survival.

The immune response against murine IgG2a immunoglobulin was studied in 42 gastrointestinal cancer patients treated with mouse monoclonal antibodies. IgM, IgA, and all 4 IgG subclasses directed against mouse immunoglobulin were studied. Thirty of 42 patients developed an anti-mouse response of IgG isotype, often associated with IgM and IgA specific for mouse monoclonal antibodies circulating in serum. Patients first developed an IgM response, followed by IgA and IgG. IgG1 was found to be the most frequently expressed isotype, followed by IgG2 and IgG3. IgG4 was detected in only 6 patients.

A parametric family of weight vectors is identified which includes most shapes of weighting vectors that will be near optimal in many cancer prevention and screening trials. This family of weight vectors is used in an application of the proposed method to data from a breast cancer screening trial. A study comparing stomach cancer incidence rates between Puerto Rico and Puerto Rican migrants to New York City provided the foundation for this investigation.

A case of spontaneous regression of pleural and intrapulmonary metastases from renal cell cancer is reported with more than 60 cases reported since the original review of this clinical phenomenon in 1964. The metastatic disease sites are most often pulmonary, but extrapulmonary sites include liver, bone, and CNS regressions. The role of nephrectomy in "spontaneous regression" continues to be obscure, accounting for < 50% of documented cases. Immunologic mechanisms, although intellectually attractive, have not been identified.

Careful patient selection criteria can be used to identify patients who are appropriate for management of an adnexal masses via operative laparoscopy. Proper intraoperative assessment and liberal use of rapid frozen section are also important for optimal clinical outcome. Reported studies show that laparoscopic management of adnexal masses can be safely done. The short hospital stay and rapid return to normal activity combine to potentially reduce the overall cost of patient care. When cancer is unexpectedly found at the time of laparoscopic surgery for an adnexal mass, the surgeon should be prepared to proceed with staging laparotomy for appropriate treatment.
