Thromboembolic events represent well-recognised complications of neoplastic disease contributing, in a significant manner, to the morbidity and mortality from cancer. The close relationship between the activation of blood coagulation and tumor growth is known since 1865, when Armand Trousseau first described the clinical association between primary or idiopathic venous thromboembolism and an underlying occult malignancy. However, only in the last decades significant advances in this field have been achieved, both on the comprehension of the complex interactions between the tumor and the hemostatic system, and on the prophylaxis and therapy of the thromboembolic manifestations in cancer patients.

Hospice Uganda was established in Kampala, September 27, 1993. Since then the hospice has served over 3,000 patients with cancer, and referrals of patients with HIV/AIDS are increasing steadily. The concepts of hospice and palliative care are well accepted in Uganda, but the delivery of services has been severely constrained by limited resources. Possibilities for growth, both in service provision and education of health professionals, were improved significantly with the incorporation of palliative care objectives into the Ministry of Health 5-year strategic plan.

In an effort to identify selective ligands for the estrogen receptor subtype ERbeta, a series of aryl benzthiophenes was synthesized. In a radioligand binding assay and reporter gene assays in HeLa and SH-SY5Y cells, compounds were characterized as ERbeta-selective agonists. By targeting ERbeta in the brain, these compounds could lead to drugs able to separate the beneficial effects of estrogens on mood, learning, and memory from side effects such as the stimulation of endometrial and breast cancer.

Tumour architecture mimics many of the features of normal tissues, with a cellular hierarchy that regulates the balance between cell renewal and cell death. Although many tumours contain cells with the characteristics of stem cells, the identity of the normal cells that acquire the first genetic hits leading to initiation of carcinogenesis has remained elusive. Identification of the primary cell of origin of cancers and the mechanisms that influence cell-fate decisions will be crucial for the development of novel non-toxic therapies that influence tumour-cell behaviour.

Malignant spinal-cord compression (MSCC) is a common complication of cancer and has a substantial negative effect on quality of life and survival. Despite widespread availability of good diagnostic technology, studies indicate that most patients are diagnosed only after they become unable to walk. We review the epidemiology, pathophysiology, and clinical features of MSCC. Clinical trials have informed the optimum management of MSCC, and we review the role of corticosteroids, radiotherapy, and surgery in the management of patients. We also emphasise advances in radiation delivery and the results of a randomised trial that supported aggressive debulking in patients with MSCC.

Chronic pain is often difficult to treat effectively. We have exploited the high affinity of herpes simplex virus (HSV) for peripheral sensory neurons to create HSV-based vectors for the treatment of chronic pain. We have demonstrated that an HSV-based vector expressing proenkephalin reduces pain-related responses in rodent models of inflammatory pain, neuropathic pain, and pain resulting from cancer in bone. A human trial has been proposed.
