At the 17th Scientific Meeting of the International Society of Hypertension (Amsterdam, June 7-11, 1998), clinicians and scientists from all over the world exchanged the results of latest research on the pathophysiology, prevention and management of hypertension. Progress in two key areas of basic research was presented at the meeting: ion channels and experimental genetics. In the clinical arena, a number of large, long-term studies were presented on topics that included risk factors of cardiovascular events in male hypertensives; antihypertensive therapy and the risk of cancer; and antihypertensive therapy in dementia. A population-wide approach was stressed as essential in a session on diet intervention and hypertension in the 21st century.

Regular mammography screening combined with timely and appropriate treatment can reduce mortality from breast cancer by 30% in women aged 50-69 years and 16% in women aged 40-49 years. A physician's recommendation has been strongly associated with a patient having a mammogram. This report analyzes data collected during 1997 in North Carolina as part of the Behavioral Risk Factor Surveillance System (BRFSS), which indicated that 23% of women aged > or =40 years who had had a routine physical examination during the 2 years preceding the survey did not recall having a discussion about mammography with a health-care provider.

Papillary thyroid carcinoma (PTC) is the most common epithelial thyroid tumor and comprises approximately 80% of all thyroid cancers. In this article, the authors discuss the data showing that total thyroidectomy is the treatment of choice of clinically significant PTC, and review an algorithm for the management of lymph node metastases. Although the prognosis for patients with PTC is generally good, appropriate surgical management (total thyroidectomy plus 131I and life-long TSH suppression) can further reduce recurrence and cancer death rates significantly.

Quercetin is a flavonoid molecule ubiquitous in nature. A number of its actions make it a potential anti-cancer agent, including cell cycle regulation, interaction with type II estrogen binding sites, and tyrosine kinase inhibition. Quercetin appears to be associated with little toxicity when administered orally or intravenously. Much in vitro and some preliminary animal and human data indicate quercetin inhibits tumor growth. More research is needed to elucidate the absorption of oral doses and the magnitude of the anti-cancer effect.

Several hundred carotenoid research studies have been published since 1996, when two major intervention trials showed a lack of protective effect of beta-carotene supplements against lung cancer. Recent epidemiologic studies continue to show an association between high dietary intake of beta-carotene and lower risk of lung cancer. New research is attempting to clarify the apparently contradictory results of intervention and epidemiologic studies. Promising areas of investigation include characterizing biologic activities of carotenoids and gaining further insight into whether they may serve primarily as markers for a healthy lifestyle or diet.

Various indices of PSA have been developed in an attempt to refine its sensitivity and improve its clinical value. These include the ratio of serum PSA level and prostate volume, the rate of change of the PSA level with time, age-referenced PSA, and the proportion of free PSA in serum relative to total PSA (free to total PSA ratio). The free to total PSA ratio is lower in patients with prostate cancer than in those with elevated PSA levels due to benign prostatic hyperplasia.

Cancers metastatic to the eye are the most common intraocular malignancy, the choroid being the most common site of affection. Breast and lung cancers are the two most common malignancies metastatic to the eye. Patients usually present with diminished visual acuity of field loss. Management of these cases is very gratifying with a large majority of patients showing resolution and maintenance of useful vision in the involved eye. This communication presents our experience in management of a case of choroidal metastasis from a breast primary. A combination of radiotherapy and chemotherapy lead to complete resolution of the lesion with no recurrences in a follow-up of over 6 months.

Irinotecan is the only accepted second-line treatment for colorectal cancer in the USA. Doses are, however, frequently limited by associated late-onset diarrhoea. Thalidomide has antiangiogenic and immunomodulatory properties and is being investigated as an antineoplastic. We did a pilot study of combination therapy with thalidomide and irinotecan for metastatic colorectal cancer. In an interim analysis of nine patients, thalidomide had almost eliminated the dose-limiting gastrointestinal toxic effects of irinotecan, especially diarrhoea and nausea (each p<0.0001), and eight of nine patients were able to complete the chemotherapy course.

Uterine cancers comprise 2 distinct pathological entities: cancer of the cervix and cancer of the corpus. They differ both in descriptive and etiologic epidemiology. Cervical cancer is more frequent in the developing world, is decreasing in terms of incidence and even more mortality in many countries and has essentially a viral etiology, being linked to human papilloma viruses. Cancer of the uterine corpus is more frequent in the developed world, is rather stable in terms of incidence and mostly has a hormonal etiology.
