We present a retrospective 10 year analysis of 76 patients with urothelial tumours of the upper urinary tract. Patients older than 65 years, and female patients, presented with more advanced stages and had a worse prognosis. The grade and stage of the tumour has the greatest impact on survival. No conclusions can be drawn on the efficacy of various types of surgery, but simple nephrectomy gave a 5-year survival of 68%, with a stump recurrence rate of 15%. We observed that patients with urothelial tumours had 4 fold increased chance of developing gastrointestinal cancer.

The authors report 15 patients with tuberculosis of the breast initially misdiagnosed as breast cancers. Criteria of differentiation of breast tuberculosis were: incidence (Tbc: cancer = 1: 245), lacking in constant site of involvement, possible concomitant involvement of other organs, eg. lung tuberculosis, youth of the patient, multi-para and history of breast feeding, in isolated cases, co-existence of tuberculosis and cancer is not impossible. The authors stress meticulous care in evaluating the history, symptoms, signs and supplementary examinations. Needle biopsy and pathology are indicated.

From Jan. 1972 through Dec. 1984, 87 cases of endometrioid carcinoma of ovary were treated in our hospital, accounting for 23.5% of all cases of epithelial ovarian cancer admitted during the same period. Histologically they could be subdivided into 4 types, i. e. endometrioid carcinoma, clear cell carcinoma, adenocanthoma, and adenosquamous carcinoma. The 5-year survival rates for each type of tumors were 51.2, 0, 100 and 0% respectively. The 5- and 10-year survival rates were 90.1 and 90.1% for stage I, patients, 23.3 and 23.3% for stage II, 4.6 and 0% for stage III, and nil for stage IV. For improving the survival rate and reducing the incidence of recurrence, postoperative long term hormonal therapy should be considered.

The skin is a relatively rare site for metastatic carcinoma compared to other organs such as bone, lung, and liver. Once it is discovered, however, the underlying disease has usually become widespread. We describe two patients with recurrent renal cell carcinoma who presented with skin nodules six and ten years, respectively, after their initial cancers were diagnosed. Treatment options and prognostic factors are outlined and dermatologists are urged to consider neoplastic disease when confronted with patients presenting with acute, asymptomatic skin nodules.

TITLE: The malignant potential of colorectal Crohn's disease.

Six patients developed colorectal carcinoma in association with Crohn's disease. The cancer was diagnosed at a mean age of 46 years and a mean 19 years after the onset of Crohn's disease. Five carcinomas arose in the chronically inflamed segment of bowel, while a sixth arose in caecum previously bypassed for Crohn's ileitis. Four carcinomas were mucinous, four presented with fistula and four were treated by complete removal of the large intestine. Three patients have died of residual or recurrent cancer. The data support an aetiological link between Crohn's proctocolitis and carcinoma.

We tested 4'-epi-doxorubicin (epirubicin) in 15 patients with advanced squamous cell carcinoma of the head and neck which progressed after conventional therapy. The drug was administered at the dosage of 25 mg/m2 weekly. No patient achieved objective response. Toxicity was minimal. Epirubicin given at this dose and schedule revealed no activity in heavily pretreated patients with cancer of the head and neck.

Cowden disease (multiple harmartoma syndrome) is of particular interest because the diagnostic mucocutaneous findings are coupled with a striking association of thyroid cancer, breast cancer, and polyposis of the gastrointestinal tract. We report a case of Cowden disease with particular emphasis on the natural history of the case. Although our case is the second reported with colon carcinoma, the absence of adenomatous polyps in other reports of Cowden disease and the stability of the gastrointestinal polyps in our patient over a seven-year period lead us to conclude that the risk of gastrointestinal malignancy is very low.

Breast reconstruction has been playing an increasing role in breast cancer therapy at The University of Texas M. D. Anderson Cancer Center. Our goal is not only to correct cosmetic deformity but also to allow mastectomy patients to wear normal clothing and function as normally as possible in society. Reconstruction is now available to most women who request it. The most commonly used techniques, in our institution, are tissue expansion, use of the latissimus dorsi flap, and use of the transverse rectus abdominis musculocutaneous (TRAM) flap. Each has advantages and disadvantages, with the choice of method depending on individual circumstances. In most cases, successful breast reconstruction can be achieved.

Two hundred and nineteen patients underwent gastrectomy for cancer in Nottingham University Hospital between January 1978 and December 1985. Twenty of these patients had early gastric cancer (EGC). Barium meal was performed in 15 patients and upper gastrointestinal endoscopy in 15. In all cases, barium meal failed to define the early nature of this disease. Only two lesions were thought to be EGC endoscopically and at laparotomy three were considered to be EGC. As neither radiologist, endoscopist nor surgeon can reliably identify EGC, all patients with gastric cancer in the absence of proven metastases should undergo gastrectomy.

Fulminant, necrotizing colitis is a frequent, and generally fatal, complication of severe granulocytopenia, occurring during the treatment of hematological malignancies. In these cases, the patient complains of severe peritonitis, including nausea, vomiting, abdominal pain, diarrhea or melena, and a high temperature. Here, a rare case of anticancer chemotherapy-induced diffuse necrotizing enterocolitis throughout the entire intestinal tract is presented, which developed in a patient who did not have a hematologic malignancy but who had colon cancer, the only clinical symptom of which was watery stools, without any evidence of peritoneal irritation. Full attention should be paid to progressive diarrhea in patients with malignancies during anticancer chemotherapy.

The presence of ceruloplasmin in follicular adenomas (15 cases) and thyroid carcinomas (39) and two Hürthle cell tumours was investigated by immunohistochemical methods. Follicular adenomas were all negative whereas Hürthle cell tumours, follicular and papillary carcinomas exhibited various degrees of cytoplasmic positivity for ceruloplasmin. Highly atypical multinucleated cells and incorporated follicular structures observed in anaplastic carcinomas were positively stained; the spindle cell variety of these cancers was always negative for ceruloplasmin. Medullary carcinomas and normal thyroid tissue were constantly unstained. The distribution pattern of ceruloplasmin is the same as that of lactoferrin, a marker recently proposed for the differential diagnosis between follicular adenomas and thyroid carcinomas; these two glycoproteins are structurally related.

The skin is a major interface between the body and the environment and possesses membrane-bound cytochrome P-450-dependent enzyme activity that is capable of both detoxifying endogenous and exogenous substrates and enhancing the toxic effects of selected substances. The use of chemical inhibitors of enzyme activation by P-450-dependent enzymes represents one possible approach to controlling toxic responses in target tissue such as the skin. Our data indicate that certain polyphenols and imidazoles are potent inhibitors of epidermal carcinogen metabolism, and of the DNA binding and the carcinogenicity of PAHs such as BP. The use of such agents may offer a novel approach to the prevention of environmentally induced cancer.

Based on experimental data suggesting synergy between cisplatin and cytosine arabinoside, 17 patients with recurrent squamous cancer of the head and neck were treated with this combination. The response rate was 18% with no complete responses, and the partial responses were of brief duration. There were moderate hematologic and gastrointestinal toxicities and two therapy-related deaths. The study was stopped early because of low response. When compared to results reported for cisplatin alone, the combination of cisplatin and cytosine arabinoside offered no clear advantage.

Hypercalcemia in association with skeletal metastases is common; hypocalcemia in this clinical setting is unexpected, though it has also been described, most commonly with primary lesions of the breast or prostate. In a subset of hypocalcemic patients with breast cancer, there is an inappropriate endocrinologic response as evidenced by a relative hypoparathyroidism and an elevation in the serum level of calcitonin. Patients with thyroid cancer benefit from treatment with exogenous thyroid hormone for two reasons: it provides adequate levels of thyroid hormone to peripheral tissues, and it reduces the level of thyrotropin, which may be an important growth factor in patients with differentiated malignant neoplasms. The use of radioactive iodine for thyroid cancer is highly controversial. Its most appropriate applications are in follicular cancers, in older patients, and in distant functioning metastases. Its value in papillary cancer is questionable, particularly in young patients. There is a great need for effective basic and clinical research on the natural course of differentiated thyroid cancer and the effects of specific therapies.
