We suggest an approach to patients with gastric polyps, based on a study of 73 patients seen at this institution over a 5-year period. There were 40 cases of adenomatous and 20 cases of hamartomatous polyps. Differentiation between these two groups of polyps was not possible on the basis of clinical presentation, laboratory data, radiological, or endoscopic appearance. Differentiation was possible only on histologic examination of a totally resected specimen. In this series, only the adenomatous polyps were associated with gastric cancers in five patients. Since the malignancy either preceded or followed the recognition of adenomatous polyps, it is important to establish an early definite histological diagnosis. Patients with known adenomatous polyps should be followed closely for the development of gastric carcinoma, even after the polyps have been removed.

Estradiol binding levels have previously been shown to be increased or decreased by the addition of of cGMP or cAMP respectively to cellular homogenates prepared from human endometrial tissue or from the endometrial cell line HEC 1. Similar cyclic nucleotide dependent effects have now been demonstrated in homogenates prepared from a second endometrial cell line, HEC 50, and from two breast cancer cell lines, CG-5 and T47D. It was previously shown that ATP and Mg2+ are necessary for the generation or inactivation of E2 binding sites. The present study demonstrates that the concentration of K+ and of dithiothreitol can also affect these reactions.

TITLE: Phase II study of doxifluridine in advanced squamous cell carcinoma of the head and neck.

We conducted a phase II trial with 5'-deoxy-5-fluoridine (doxifluridine) in advanced squamous cell carcinoma of the head and neck. The drug was given at the dose of 4 g/m2 daily X 5 every 3 weeks. Twenty eligible patients entered this trial, 12 being evaluable for response to doxifluridine. The majority of these patients received previous treatment for cancer. One complete and two partial remissions were observed (25%). Drug-induced toxicity consisted mainly of myelosuppression, mild nausea and vomiting, stomatitis and central nervous system side-effects. Other dosages or schedules for doxifluridine administration might be explored in poor-risk patients.

We present a retrospective 10 year analysis of 76 patients with urothelial tumours of the upper urinary tract. Patients older than 65 years, and female patients, presented with more advanced stages and had a worse prognosis. The grade and stage of the tumour has the greatest impact on survival. No conclusions can be drawn on the efficacy of various types of surgery, but simple nephrectomy gave a 5-year survival of 68%, with a stump recurrence rate of 15%. We observed that patients with urothelial tumours had 4 fold increased chance of developing gastrointestinal cancer.

An approach is described for predicting the statistical value of extending follow-up in a cohort study. A simple approximation to the expected number of new events of interest is given. The effect of these events on inferences for parameters such as a standardized mortality ratio is approached in two ways. The first concerns the probability of reversing the conclusion of a significance test. The second approach finds the plausible range of values for the standardized mortality ratio after further follow-up that are consistent with the currently available data. Each of these values is displayed together with the precision of the estimate. The methods are illustrated with results from the International Radiation Study of Cervical Cancer (IARC Scientific Publication No. 52, 1984).

Criteria of differentiation of breast tuberculosis were: incidence (Tbc: cancer = 1: 245), lacking in constant site of involvement, possible concomitant involvement of other organs, eg. lung tuberculosis, youth of the patient, multi-para and history of breast feeding, in isolated cases, co-existence of tuberculosis and cancer is not impossible. The authors stress meticulous care in evaluating the history, symptoms, signs and supplementary examinations. Needle biopsy and pathology are indicated.

To determine the area of surgical resection and the prognosis in patients with intrahepatic bile duct carcinoma, we studied the mode of spread of this cancer, using 27 autopsy and 4 lobectomized cases. In hilar carcinoma, there was lymphatic invasion to the extrahepatic bile duct before invation to the hepatic lobules. Scirrhous cancer spread more distantly toward the peripheral areas of the liver than non-scirrhous cancer. The occurrence of extrahepatic metastasis might involve the following factors; depth of infiltration, extent of venous invasion, intrahepatic metastasis, size of the main tumor, and number of hepatic segments invaded by the cancer.

From Jan. 1972 through Dec. 1984, 87 cases of endometrioid carcinoma of ovary were treated in our hospital, accounting for 23.5% of all cases of epithelial ovarian cancer admitted during the same period. Histologically they could be subdivided into 4 types, i. e. endometrioid carcinoma, clear cell carcinoma, adenocanthoma, and adenosquamous carcinoma. The 5-year survival rates for each type of tumors were 51.2, 0, 100 and 0% respectively. The 5- and 10-year survival rates were 90.1 and 90.1% for stage I, patients, 23.3 and 23.3% for stage II, 4.6 and 0% for stage III, and nil for stage IV. For improving the survival rate and reducing the incidence of recurrence, postoperative long term hormonal therapy should be considered.

A retrospective comparison between 28 stage II-IV ovarian cancer patients who received postoperative abdominopelvic irradiation and 15 such patients who received postoperative cis-platinum, Adriamycin, and Cytoxan chemotherapy was made. In both treatment groups there was a trend for a longer progression-free interval and 3-year survival in patients with residual tumor masses of less than 2 cm in diameter. In these patients there was also a trend favoring radiotherapy. However, all differences were not significant. Criteria for the choice of either modality of postoperative therapy of ovarian cancer patients remain to be determined. Nevertheless, the efficacy of both modes of treatment does not differ greatly.

The skin is a relatively rare site for metastatic carcinoma compared to other organs such as bone, lung, and liver. Once it is discovered, however, the underlying disease has usually become widespread. We describe two patients with recurrent renal cell carcinoma who presented with skin nodules six and ten years, respectively, after their initial cancers were diagnosed. Treatment options and prognostic factors are outlined and dermatologists are urged to consider neoplastic disease when confronted with patients presenting with acute, asymptomatic skin nodules.

TITLE: The malignant potential of colorectal Crohn's disease.

Six patients developed colorectal carcinoma in association with Crohn's disease. The cancer was diagnosed at a mean age of 46 years and a mean 19 years after the onset of Crohn's disease. Five carcinomas arose in the chronically inflamed segment of bowel, while a sixth arose in caecum previously bypassed for Crohn's ileitis. Four carcinomas were mucinous, four presented with fistula and four were treated by complete removal of the large intestine. Three patients have died of residual or recurrent cancer. The data support an aetiological link between Crohn's proctocolitis and carcinoma.

The ages at presentation of white patients with breast cancer were found to be significantly higher than those of blacks, Indians and coloureds; 73% of white women fell into the postmenopausal group, in marked contrast to only 35% of Indians, while blacks and coloureds had similar proportions of pre- and post-menopausal patients. A significantly higher incidence of poorly differentiated tumours was seen in Indian and black patients. Blacks showed a significant tendency to present with more advanced disease, while whites were generally diagnosed at a much earlier stage.

Over a ten-year period, 44 patients with known primary renal cell cancer underwent thoracotomy for pulmonary metastases. The median postthoracotomy survival for all patients was 33 months. The five-year survival was 27 percent. Postthoracotomy survival was significantly better in those patients with a disease free interval of greater than 24 months and patients with metastatic lesions greater than or equal to 3 cm. No difference in survival was detected in patients with one versus more than one lesion or in patients undergoing complete resection versus incomplete resection or biopsy only. Age, sex, grade of tumor, or location of the pulmonary metastasis had no influence on survival.

We tested 4'-epi-doxorubicin (epirubicin) in 15 patients with advanced squamous cell carcinoma of the head and neck which progressed after conventional therapy. The drug was administered at the dosage of 25 mg/m2 weekly. No patient achieved objective response. Toxicity was minimal. Epirubicin given at this dose and schedule revealed no activity in heavily pretreated patients with cancer of the head and neck.
