Although CA 125 is generally considered a serum marker of malignant tumors, it may be elevated in conditions characterized by peritoneal inflammation. We report a patient with known pulmonary sarcoidosis who presented with an adnexal mass and an elevated CA 125. At laparotomy, she was found to have miliary peritoneal disease simulating the appearance of metastatic cancer. Microscopic examination revealed a benign ovarian cyst and diffuse granulomatous disease similar to that present in the lung.

Ataxia-telangiectasia (AT) is a heterogeneous autosomal recessive disorder marked by cerebellar ataxia, oculocutaneous telangiectases, hypersensitivity to ionizing radiation, immunodeficiency, and cancer susceptibility. AT is also a spontaneous chromosomal breakage syndrome, notable for tissue-specific cytogenetic changes and telomeric fusions. Molecular characterization of rearrangements specific to T-lymphocytes suggests that a DNA repair/processing defect is potentially responsible for the diverse array of chromosomal abnormalities observed in a variety of AT cell types.

This paper presents a mixed model of carcinogenesis and derives the incidence functions and the first four cumulants of the number of tumors. The model is then fitted to some retinoblastoma data from the SEER (Surveillance Epidemiology End Results) project of the National Cancer Institute for the years 1973-1982. In all the cases presented, the estimated incidence functions appear to fit the observed incidence curves quite well, suggesting the usefulness of the model.

We report the use of ELT utilizing an Nd:YAG laser in a group of patients with cancer who represented a significant surgical risk. Effective palliation was achieved, with minimal morbidity, and the quality of life of most of our patients was preserved or enhanced, at a minimal physiologic cost. In carefully selected patients, ELT has proved to be a safe and highly effective procedure for the management of gastrointestinal cancers. It should be considered a valuable addition to the treatment armamentarium of the practicing clinician dealing with patients with advanced cancers or patients with significant medical conditions precluding surgical resection.

Neural blocking of phenol-glycerine was used for the treatment of cancer pain originated from the esophagus, stomach, liver, lung, rectum, ovary, nasopharynx, head and neck. According to the dose difference, 148 cases were divided into three groups of 7-10% 4-5 ml (30 cases), 15-20% 3-4 ml (70 cases), and 25% 2-2.5 ml (48 cases). The result observed among the three groups as good or fair was 46.7%, 80% and 89.6% respectively, however, complications occurred too within the rate of 13.3%, 34.3% and 14.6% respectively. It is indicated that 2.5 ml of 15-25% phenol-glycerine seems better.

Ehrlich's test, serum mucoproproteins, phosphohexoisomerase (PHI), and lactate dehydrogenase (LDH) levels were studied in patients with cancer and age matched controls. All parameters studied were found to be significantly increased in cancer. Evaluation of Ehrlich's test, PHI and LDH for their validity as screening tests revealed that Ehrlich's test is better suited as a screening test for cancer. A positive correlation observed in the study between serum mucoproteins and Ehrlich's test strengthens the postulation that mucoproteins are responsible for the colour development in Ehrlich's test.

Li-Fraumeni syndrome (LFS) is an autosomal dominant disorder that predisposes individuals to multiple forms of cancer including breast carcinoma, soft tissue sarcoma, osteosarcoma, leukemia and adrenocortical carcinoma. These diverse tumor types develop at unusually early ages. Analysis of the tumor suppressor gene p53 in family members with LFS have demonstrated that germline mutations in the p53 gene were present in most of the LFS family tested so far. Furthermore, germline p53 mutations were also found in cancer-prone individuals which were not indicative of the LFS.

TITLE: Hodgkin's disease following non-Hodgkin's lymphoma.

The risk of Hodgkin's disease following non-Hodgkin's lymphoma (NHL) was estimated using information collected by the National Cancer Institute's Surveillance, Epidemiology, and End Results Program. Histopathologic material representing both diagnoses was reviewed for 11 patients in whom Hodgkin's disease was reported after NHL. Diagnoses of both diseases were confirmed in nine patients. Comparisons with population rates indicated that patients with NHL are at an almost threefold risk of subsequently having Hodgkin's disease. Survival in this series was poor; seven of the nine patients died within 16 months after the diagnosis of Hodgkin's disease.

The results of 4 pathologically proven advanced lung cancer patients treated by CT guided percutaneous intubation of chemotherapeutic agent into the tumor are reported. Multidrug chemotherapy using mitomycin, cisplatin, cyclophosphamide or 5-Fu was given in a total of 11 treatments. All patients had symptomatic improvement with tumor reduction. There was no incidence of pneumothorax and other side reactions were mild. The authors believe that direct intervention chemotherapy of lung cancer is an effective and safe treatment of choice in advanced cases.

A 40-year-old mother and her 7-year-old son underwent colectomy for polyposis coli. Both colectomy specimens contained predominantly juvenile polyps. The mother's specimen also had numerous tubular adenomas and one 5-cm severely dysplastic villous adenoma without invasive carcinoma. Foci of adenomatous change in juvenile polyps were present in the son's specimen. The histologic features of both cases suggest a sequence of change from juvenile polyps to adenomatous polyps to cancer.

Plasminogen activator (PA) is a specific serine protease which catalyses conversion of the inactive plasminogen into the broad-spectrum protease, plasmin. PA exists in two structurally related forms known as tissue-type PA (tPA) and urokinase-type PA (uPA). Conversion of normal cells into a malignant state frequently leads to increased production of uPA. There is increasing evidence that uPA is directly involved in the process of metastasis. High levels of uPA in human breast cancers is a marker for poor prognosis. Finally, uPA may be a target for anti-metastatic agents, either by inhibition of its synthesis, inhibition of its activity or its binding to receptor.

Trans-sternal bilateral thoracotomy was performed to resect the right upper lobe and the left S1 + 2 + S3, and to complete lymphadenectomy in a 35-year-old female case of lung cancer in whom multiple lesions were suspected. Trans-sternal bilateral thoracotomy was considered to be useful for one-stage surgery in patients in whom bilateral lung cancer is suspected or confirmed, because it provides a sufficient surgical field enabling the resection of lung and lymph nodes. This may be the first case report of trans-sternal bilateral thoracotomy to treat multiple primary lung cancer.

From September 1980 to May 1987, fiberoptic gastroscopy was performed in 3538 patients. Of them, 356 (10%) had received operation for gastric cancer. A second primary cancer was found in 31 cases, recurrent gastric cancer in 18 and adenoma in 87. Among the 138 asymptomatic operated patients, a second primary cancer developed in 8, and 6 of them were in the early stage. In the 218 symptomatic operated patients, a second primary cancer developed in 23, and 10 of them were in the early stage. The results suggest that gastroscopic examinations at regular intervals are mandatory for asymptomatic postoperative patients.
