TITLE: Recessive mechanisms of malignancy.
PUBDATE: 19880801
It is increasingly recognised that recessive mutations play an important role in the pathogenesis of many forms of malignancy. Some of the affected loci may prove to be recessively-activated proto-oncogenes, but others are now known to be tumorigenic solely by virtue of their loss or inactivation and therefore form a distinct and novel family of tumour genes. Preliminary evidence suggests that such genes are likely to be functionally heterogeneous and to encode molecules involved in the inhibition of cellular proliferation and/or the induction of differentiation. Their further study is likely to illuminate fundamental mechanisms of normal cellular growth and differentiation as well as having important implications for the pathogenesis and management of cancer.

After first analyzing the basic pathophysiological aspects and the mechanism of action of aminoglutethimide, the authors report on their experience with the use of this steroid biosynthesis inhibitor in a group of geriatric patients with advanced cancer of the breast. After illustrating the results of their study, the authors conclude by stressing that the use of this drug appears to improve the quality of life of the patients with a significant increase in rates of complete or partial remission of the neoplastic disease.

Since 1981, cancer has been No. 1 cause of death in Japan. A "Comprehensive 10-Year Strategy for Cancer Control" is a national project that was strongly promoted by the former Prime Minister Mr. Yasuhiro Nakasone in 1983. The program comprises project research studies and functions supporting them. These include the education and utilization of young researchers (research resident), international cooperation and research resources (cell and gene) bank. The aim of studies is clarification of the mechanism of human carcinogenesis within 10 years and the results will be applied to prevention, diagnosis and therapy of cancers.

PUBDATE: 19890201
Pancreatic cancer is a frequent gastrointestinal malignancy with demonstrable increases in blacks. Predisposing factors are nonspecific but statistically relevant. Early signs and symptoms are vague and the duration prior to medical or surgical evaluation is typically more than four months. The overt clinical picture represents advanced stages of pancreatic cancer that are nonresectable. Positive results or prolonged survivals in excess of five years for curative surgical procedures are meager. The best approaches toward cancer of the pancreas are improvements in education, prevention, early diagnosis, palliative surgical procedures with adjuvant medical measures, and radical surgery in selective cases and under specific circumstances.

PUBDATE: 19880801
An intraarterial high-dose CDDP, using a balloon catheter, has been infused into five patients with a recurrent ovarian cancer. The dose of CDDP was 150 mg-200 mg/body. Two patients experienced a partial tumor regression, whereas three showed no change. With regard to the toxicity, the NAG index (NAG activity/urinary creatinine) rose markedly in all patients, but toxicity was almost similar to that seen from a systemic administration. No significant catheter complication occurred.

Hydatid disease due to Taenia echinococcus involves bone in about 1% of all cases. The spine is involved in about 50% of cases. Neural compression is common in the form of paraplegia or nerve root compression. The prognosis with spinal involvement is generally regarded as very poor and often likened to that of spinal cancer. Four cases of spinal hydatid with neural involvement are presented. Three were treated by anterior spinal decompression and all four were treated with mebendazole. The results show good neural recovery and it is concluded that the prognosis is not as dismal as it was formerly thought to be.

PUBDATE: 19860501
Nitrosamines and precursor secondary amines were assayed in foods from families in four villages of the esophageal cancer high incidence area of Linxian, Henan Province, People's Republic of China. Amines (as tosylamides) and nitrosamines were readily detected at p.p.m. and p.p.b. levels, respectively, in all samples. In this small preliminary survey (25 families, four villages), however, there were no strong correlation between the levels of the carcinogenic nitrosamines or the precursor secondary amines with the incidence of esophageal cancer in the individual families. The success of the analytical procedures suggests that a more extensive study is warranted.

Twenty-nine patients with advanced epithelial ovarian cancer were included in a phase-II trial of cyclic hormonal treatment. The schedule consisted of tamoxifen 10 mg X 3 daily for 14 days followed by medroxyprogesterone acetate 400 mg X 2 daily for 14 days. No objective response was recorded in 17 evaluable patients and cyclic hormonal treatment appears to be ineffective in advanced ovarian cancer.

PUBDATE: 19890601
Disseminated superficial actinic porokeratosis is an autosomal dominant condition that requires sun exposure for full expression. It affects only sun-exposed areas, with relative sparing of the face. In sun-damaged skin of Australians disseminated superficial actinic porokeratosis is commonly mistaken for solar keratosis. Twenty-nine subjects with disseminated superficial actinic porokeratosis were involved in an extensive questionnaire and clinical study. The distribution of lesions was charted on all subjects, with a mean count of 268 lesions per subject. We found no evidence that skin cancer had arisen in disseminated superficial actinic porokeratosis lesions. A review of the etiologic and clinical features of disseminated superficial actinic porokeratosis also is presented.

As a serious side effect of immunosuppressive therapy following organ transplantation, the incidence of malignancies shows a significant threefold increase compared with the age-matched general population. Under treatment with conventional immunosuppressive drugs (azathioprine, steroids and antilymphocyte globulin) patients are supposed to be susceptible to skin and lip cancers (40%) whereas the recently introduced new immunosuppressive drug cyclosporine A shows a comparable incidence of non-Hodgkin lymphomas (41%) and the patients seem to develop more potentially life threatening tumours of internal organs. In the present paper we report on a case of laryngeal carcinoma following immunosuppressive treatment with cyclosporine A 10 months after heart transplantation.

Early cancer of the breast includes those categories of the so-called early breast cancer (TNM Stage I), minimal breast cancer, noninvasive breast cancer and T0 breast cancer. In the breast, it seems extremely rare that the lesion is genuine border-line of malignancy. A papillary or lobular lesion is usually worrisome in differential diagnosis, either benign or malignant. Sclerosing adenosis is most likely to be overdiagnosed as malignant. Tubular carcinoma, low papillary carcinoma and clinging carcinoma are most likely to be underdiagnosed as benign. To avoid overdiagnosis and/or underdiagnosis of malignancy, it is essential to learn exactly every histologic feature of various lesions.

Factors which influence survival in patients with colorectal cancers were analyzed. From the anatomical and clinical points of view, large bowel cancer should be evaluated separately as colon and rectal cancer. The factors were stages of the disease, depth of tumor infiltration, morphological type of tumor appearance, histological type of tumor, venous invasion of cancer cell, lymphatic invasion of cancer cells, lymphnode metastasis, degree of tumor resection, distant metastasis and dissemination in abdominal cavity.

We had recently experienced two cases of tuberculous peritonitis. One was suspected of ovarian cancer but finally diagnosed as tuberculosis by the exploratory laparotomy. The other was found out casually at the operation of benign ovarian tumor. Both cases accompanied with pulmonary tuberculosis. Tuberculous peritonitis should be considered in the differential diagnosis in patients with abdominal disorders. Remarkable elevation of serum CA-125 related to ovarian cancer was found in both cases. It was suggested that level of serum CA-125 might be useful for diagnosis and monitoring of the disease.

A retrospective study was carried out on the type of habitual diet consumed in the Niterói (Rio de Janeiro) city. Two groups of subjects were integrated: the Experimental Group, formed by patients with neoplasms of the large intestine and rectum, and the Control Group, formed by individuals with no gastrointestinal disorders. The information obtained through direct inquiry, strongly suggests that cancer of the colon and rectum may be related to the high proportion of fat and low proportion of dietary fiber found in the diets of the patients studied.

PUBDATE: 19890801
The surgical treatment of rectum cancer in 165 patients with the apparatus AKA-2 for compressive anastomoses was analyzed. The oncological adequacy of proximal resections of the rectum at the distal level of the section not less than 3.5-5 cm from the visually seen margin of the tumor depending on its growth type was shown. Low anterior resection of the rectum in patients with considerable size of the tumors is not oncologically justified.
