TITLE: Mortality from prostate cancer in Italy: 1950-1979. Cross-sectional rates and cohort analysis.

This paper studies prostate cancer mortality in Italy from 1950-1979 to evaluate its importance in relation to total cancers, examine the time trend for forecasting future mortality trends, and makes an attempt to interpretate mortality by analyzing demographic and risk factors. In the Authors' opinion findings from cross-sectional rates, cohort analysis and Devesa-Schneiderman method, indicate that in Italy even if the age-adjusted mortality rate has nearly doubled over the period, as in many other countries, mortality can be expected to remain constant or decrease in the future, since the data to hand show a decreasing trend in some age cohorts.

The possible expression of Hanganutziu-Deicher (H-D)-type heterophile antigen on the cell surface of several types of human cancer tissues was investigated by the membrane immunofluorescence test with anti-H-D serum. Anti-H-D serum of high titer was obtained from chickens immunized with H-D antigen-active glycosphingolipid, N-glycolylneuraminyl-lactosylceramide (equine hematoside). H-D antigen was demonstrated in 3 to 6 gastric cancer tissues, 2 of 3 lung cancer tissues, and 2 of 4 breast tissues examined, but not in 5 colorectal cancer tissues examined.

It is increasingly recognised that recessive mutations play an important role in the pathogenesis of many forms of malignancy. Some of the affected loci may prove to be recessively-activated proto-oncogenes, but others are now known to be tumorigenic solely by virtue of their loss or inactivation and therefore form a distinct and novel family of tumour genes. Preliminary evidence suggests that such genes are likely to be functionally heterogeneous and to encode molecules involved in the inhibition of cellular proliferation and/or the induction of differentiation. Their further study is likely to illuminate fundamental mechanisms of normal cellular growth and differentiation as well as having important implications for the pathogenesis and management of cancer.

In breast cancer a decrease in triiodothyronine concentration was revealed with the normal level of thyroxine that may be associated with the disturbed peripheral metabolism into triiodothyronine. The lowering of thyroid functional activity is noted during radiation therapy of patients at menopause. Administration of thyroidin during radiation therapy to elderly patients with the III stage of the disease compensated for hormone deficiency producing a sparing effect on the thyroid, and influenced the therapeutic results favorably. A high level of the somatotropic hormone was observed in patients of the reproductive age and at menopause, the level of insulin was increased in all the patients.

Pancreatic cancer is a frequent gastrointestinal malignancy with demonstrable increases in blacks. Predisposing factors are nonspecific but statistically relevant. Early signs and symptoms are vague and the duration prior to medical or surgical evaluation is typically more than four months. The overt clinical picture represents advanced stages of pancreatic cancer that are nonresectable. Positive results or prolonged survivals in excess of five years for curative surgical procedures are meager. The best approaches toward cancer of the pancreas are improvements in education, prevention, early diagnosis, palliative surgical procedures with adjuvant medical measures, and radical surgery in selective cases and under specific circumstances.

Histological examination revealed various features: Anaplastic transitional cell carcinoma and glandular formation scattered with portions resembling cancer pearls. Sarcomatoid-like area at ureteral margin of the mass and carcinoma-in-situ at renal pelvic mucosa were also seen. Final interpretation was grade III, transitional cell carcinoma, with squamous and glandular differentiation, spindling, and pleomorphic areas.

TITLE: [High-dose CDDP therapy using a balloon-occluded arterial infusion (BOAI) in recurrent ovarian cancer]

An intraarterial high-dose CDDP, using a balloon catheter, has been infused into five patients with a recurrent ovarian cancer. The dose of CDDP was 150 mg-200 mg/body. Two patients experienced a partial tumor regression, whereas three showed no change. With regard to the toxicity, the NAG index (NAG activity/urinary creatinine) rose markedly in all patients, but toxicity was almost similar to that seen from a systemic administration. No significant catheter complication occurred.

14 male patients with cancer of the colon were enterally fed for 2 days preoperatively, on the day of the operation and 5 days postoperatively with a high-caloric oligopeptide diet without the additional enteral administration of calories. All the anastomoses (5 rectal and 5 sigmoid colon resections as well as 4 hemicolectomies ) healed without problems; no complications occurred . The nutrition was well tolerated without exception and the jejunal tube tolerated without discomfort. The short-lived function proteins fell during postoperative metabolism. However, the means for total protein, albumin, prealbumin, transferrin and the immunoglobulins (IgM, IgG, IgA und IgD) were still within the lower part of the normal range. Only retinol-binding protein fell to values up to 30% below normal.

Thirteen human malignant tumors can be cured in a high percentage by combined modality treatment (surgery, radiotherapy, chemotherapy) even if patients suffer from advanced disease. In other neoplasms accounting for about 40% of all tumors, intensive treatment leads to good palliation and significant prolongation of survival time. An important prerequisite for this success is a comprehensive pretherapeutic diagnostic work-up that reveals the extent of disease. Side effects after intensive therapy cannot be completely avoided, but can be alleviated. The complex interrelationship of interdisciplinary therapeutic measures in the context of combination modality treatment makes it necessary that patients with potentially curable neoplasms be treated at cancer centres.

The paper is concerned with a study of the concentration of thyrotropic hormone (TTH), thyroxine, triidothyronine, thyroxine-binding globulin (TBG), thyroglobulin (TG) and carcinoembryonic antigen (CEA) in the blood of 69 patients with benign and malignant thyroid tumors. It has been established that the determination of TTH and TBG level can be used as an additional criterion to define the nature of a tumor process in the thyroid gland. A study of TTH and TG levels is of diagnostic importance in patients with recurrences and metastases of thyroidal cancer to the lungs. A study of the level of CEA in the blood is an important parameter in the control over the efficacy of thyroid cancer treatment.

Hydatid disease due to Taenia echinococcus involves bone in about 1% of all cases. The spine is involved in about 50% of cases. Neural compression is common in the form of paraplegia or nerve root compression. The prognosis with spinal involvement is generally regarded as very poor and often likened to that of spinal cancer. Four cases of spinal hydatid with neural involvement are presented. Three were treated by anterior spinal decompression and all four were treated with mebendazole. The results show good neural recovery and it is concluded that the prognosis is not as dismal as it was formerly thought to be.

Nitrosamines and precursor secondary amines were assayed in foods from families in four villages of the esophageal cancer high incidence area of Linxian, Henan Province, People's Republic of China. Amines (as tosylamides) and nitrosamines were readily detected at p.p.m. and p.p.b. levels, respectively, in all samples. In this small preliminary survey (25 families, four villages), however, there were no strong correlation between the levels of the carcinogenic nitrosamines or the precursor secondary amines with the incidence of esophageal cancer in the individual families. The success of the analytical procedures suggests that a more extensive study is warranted.

The South Pacific Commission Cancer Registry has been operational since 1977, and reasonably complete cancer incidence rates are available for New Caledonia, Fiji, Micronesia, the Cook Islands, and Niue. In addition, less complete reporting is available from American Samoa, Papua New Guinea, and French Polynesia. Cancers of the lung, oral cavity, esophagus, liver, and cervix are potentially preventable but prevalent in many Pacific island countries. Unusually low rates of many cancers were observed in Fiji, the most notable being lung and colon-rectum. Thyroid cancer rates are elevated in some but not all female populations. These and other variations in cancer by site are important areas for further research.
