This paper studies prostate cancer mortality in Italy from 1950-1979 to evaluate its importance in relation to total cancers, examine the time trend for forecasting future mortality trends, and makes an attempt to interpretate mortality by analyzing demographic and risk factors. In the Authors' opinion findings from cross-sectional rates, cohort analysis and Devesa-Schneiderman method, indicate that in Italy even if the age-adjusted mortality rate has nearly doubled over the period, as in many other countries, mortality can be expected to remain constant or decrease in the future, since the data to hand show a decreasing trend in some age cohorts.

The possible expression of Hanganutziu-Deicher (H-D)-type heterophile antigen on the cell surface of several types of human cancer tissues was investigated by the membrane immunofluorescence test with anti-H-D serum. Anti-H-D serum of high titer was obtained from chickens immunized with H-D antigen-active glycosphingolipid, N-glycolylneuraminyl-lactosylceramide (equine hematoside). H-D antigen was demonstrated in 3 to 6 gastric cancer tissues, 2 of 3 lung cancer tissues, and 2 of 4 breast tissues examined, but not in 5 colorectal cancer tissues examined.

It is increasingly recognised that recessive mutations play an important role in the pathogenesis of many forms of malignancy. Some of the affected loci may prove to be recessively-activated proto-oncogenes, but others are now known to be tumorigenic solely by virtue of their loss or inactivation and therefore form a distinct and novel family of tumour genes. Preliminary evidence suggests that such genes are likely to be functionally heterogeneous and to encode molecules involved in the inhibition of cellular proliferation and/or the induction of differentiation. Their further study is likely to illuminate fundamental mechanisms of normal cellular growth and differentiation as well as having important implications for the pathogenesis and management of cancer.

Cell kinetics of cancers have been described in books, texts and other reports, but the correlation with action mechanism of antineoplastic agents has rarely been mentioned in the literature. The action mechanism of the antineoplastic agents such as interferon, ACNU and cisplatin was analyzed with use of propidium iodide and BrdU double staining by flow cytometer. Interferon showed S phase accumulation, ACNU and cisplatin blocked the stage of G(2)M phase. Flow cytometry was useful for the analysis of cell kinetics.

TITLE: Dynamic studies on portal haemodynamics by scintiphotosplenoportography: flow patterns of portal circulation.

Scintiphotosplenoportography (SSP) was performed in 190 instances in 161 patients. No significant complications, such as severe pain or splenic haemorrhage, were encountered in any of the patients studied. Flow patterns of SSP were classified into nine groups according to the direction of collateral flow. Hepatopetal collateral flow was frequently observed in pancreatic cancer. In portal hypertension, cephalic collateral flow was more frequently observed than caudal flow. In detecting oesophageal varices, both SSP and endoscopy were performed in 81 patients. In four out of 46 patients with cephalic flow, oesophageal varices could not be observed by endoscopy. Conversely, in three out of 44 patients with oesophageal varices, SSP failed to show cephalic flow. The combined use of SSP and endoscopy would therefore provide more useful information in the management of patients with portal hypertension.

Of 176 patients with minimal breast cancer, 138 had intraductal carcinoma in situ, 21 minimally invasive carcinoma, and 17 lobular carcinoma in situ. Various modalities of treatment were used including radical, modified radical, and simple mastectomy with and without radiation therapy. Long-term postoperative follow-up was available in all but five patients and ranged from one year to twenty-one years. Actuarial analysis projected a twenty year survival of 93.2 per cent for the entire group. Analysis of survival figures based on each of the several treatment modalities showed no definite advantage of one form of treatment over another. The data suggest that minimal breast cancer is a prognostically favorable diagnosis, provided invasive carcinoma is not present or does not develop in the opposite breast. It is also indicated that breast cancer is potentially a bilateral disease and that follow-up and treatment of the opposite breast must be of major concern in the care of these patients.

In breast cancer a decrease in triiodothyronine concentration was revealed with the normal level of thyroxine that may be associated with the disturbed peripheral metabolism into triiodothyronine. The lowering of thyroid functional activity is noted during radiation therapy of patients at menopause. Administration of thyroidin during radiation therapy to elderly patients with the III stage of the disease compensated for hormone deficiency producing a sparing effect on the thyroid, and influenced the therapeutic results favorably. A high level of the somatotropic hormone was observed in patients of the reproductive age and at menopause, the level of insulin was increased in all the patients.

TITLE: Precise selection of breast cancer patients with bone metastasis for endocrine ablation.

To enhance the precision of selection of breast cancer patients with bone metastasis for endocrine ablation, 85 patients underwent a Levodopa test for 4 consecutives days. Assessment of the result of the test was made on the basis of thorough clinical evaluation, x-ray examination, and results of serum and skin tests as well as cancer receptor studies when tissue was available. Results of 23 surgical procedures were as follows: bilateral oophorectomy elicited good clinical response in five patients with positive preoperative Levodopa tests; one negative clinical response followed a negative Levodopa test; bilateral adrenaloophorectomy in nine patients elicited good response in seven who had positive Levodopa tests and negative responses in two patients who had negative Levodopa tests; bilateral adrenalectomy in eight patients elicited good response in five patients with positive Levodopa tests and negative responses in three who had negative Levodopa tests. There were no deaths and there was only one complication after operation. Levodopa testing appears to be an effective means for selection of patients with bone pain from recurrent breast cancer who will benefit from endocrine ablation. When the Levodopa test was negative for bone pain relief, endocrine ablation was of no benefit to the patient.

TITLE: Bladder cancer producing granulocyte colony-stimulating factor: a case report.

A rare case of bladder cancer producing granulocyte colony-stimulating factor (G-CSF) is reported. A 63-year-old man with a progressive, grade 3 transitional cell bladder carcinoma, showed marked leukocytosis (maximum 181,800/mm3) and an elevated serum G-CSF to 131.7 pg/mL. After radical cystectomy and neoadjuvant chemotherapy, the tumor recurred locally. Immunohistochemical examination with monoclonal antibody specific for G-CSF revealed positive staining in the local recurrent tumor. To our knowledge, there have been only seven cases reported of bladder cancer producing G-CSF.

TITLE: Validity of clinical examination and mammography as screening tests for breast cancer.

This study is aiming to determine the validity and observer variability of clinical examination and mammography as screening tests for breast cancer. Women over the age of forty are given two independent clinical examinations of the breasts, and mammograms are taken and read independently by two radiologists. This paper presents the results of screening the first 1215 women to be enrolled in the study. At their first screening attendance, 231 women (19%) were referred for surgical opinion, 119 (9-8%) underwent biopsy, and cancer was diagnosed in 17 (1-4%). 2 further cancers were diagnosed in the ensuing six months among women who had been negative on initial screening, representing a false-negative rate of 2 out of 19 (11%). Clinical examination resulted in 189 referrals (15-6%), 90 biopsies (7-4%), and detected 11 cancers; corresponding figures for mammography were 76 referrals (6-3%), 55 biopsies ((4-5%), and 14 cancers. Observer variability was greater for clinical examination than for mammography. These early results suggest that as a screening test mammography compares favourably with clinical examination, but both tests are necessary if many false negatives are to be avoided.

The development of colorectal cancer is the most serious long-term problem faced by patients with extensive ulcerative colitis and Crohn's colitis, with an incidence 20-fold higher and an average age of onset 20 years younger than colorectal cancer in the general population. This report summarizes the epidemiology, pathology, and recent advances in the molecular pathogenesis of colorectal cancer in inflammatory bowel disease and of its acknowledged precursor, mucosal dysplasia. It also reviews the rationale behind the use of endoscopic surveillance for dysplasia as a means of reducing cancer mortality, as well as some of the issues involved in its effective implementation.

TITLE: Radioiodine uptake by squamous-cell carcinoma of the lung.

A patient with a history of total thyroidectomy for papillary carcinoma showed a solitary hot spot in the chest on post-therapy radioiodine imaging. Subsequent evaluation demonstrated that this lesion was a primary squamous-cell lung cancer. Our case illustrates a rare but important differential diagnosis from lung metastasis of thyroid cancer in 131I scintigraphy. Alternatively, this "aberrant uptake" might be a rather common phenomenon for lung neoplasms, although the underlying mechanism is not yet known. Future experimental studies might lead to a new application of radioiodine imaging in pulmonary oncology.
