Ras proteins send signals through multiple effector pathways. The Raf/MEK/MAPK and PI 3' kinase pathways are well-validated Ras effectors in human cancers, but many other candidate pathways could be equally important. RalGDS is such a candidate: in a new paper from Chris Marshall's group, an important role for RalGDS in Ras transformation in vivo has been established for the first time. Mice lacking RalGDS are defective in tumor formation, possibly because of increased apoptosis in Ras-driven tumors. The hunt for a clear role for RalGDS activation in human cancer is on.

Ricin is a very toxic RIP II type lectin. As no effective antidote is known, its toxicity could be misused in terrorist actions. The mechanism of the toxic action is currently studied for the preparation of selective immunotoxins, which could be used in the therapy of various cancer diseases or HIV infection. There are promising in vitro results, but some adverse in vivo effects limit the use of these preparations in real therapy.

Colorectal cancer (CRC) is a common cancer in the more developed Western countries. This paper aims to give a review of the established or suspected and potentially modifiable risk factors for CRC. Increased BMI, physical inactivity, tobacco smoking and high alcohol consumption are strongly implicated factors. The precise dietary patterns associated with increased risk are less clear. Use of NSAIDs as well as postmenopausal hormones seems to be associated with a substantially reduced risk of CRC. Altogether, the evidence indicates that primary prevention of CRC is feasible.

We evaluated whether social deprivation affected decision-making for breast cancer surgery. Of 3419 patients, 53.6% had mastectomy and this was predicted by deprivation, age, tumour size and hospital, all of which retained significance on multivariate analysis, except deprivation. Pathological characteristics and surgical decision-making determined choice of operation not deprivation.

In this paper, we provide a working classification for circulating biomarkers according to their potential clinical application. We hope this classification will provide a framework to which the results of future studies can be added. We also discuss the promises and pitfalls in the optional use of biomarkers in oncology.

A 73-year-old woman underwent a pelvic exenteration for transitional cell carcinoma of the bladder and radiation-induced rectovaginal fistula. The patient had undergone radical hysterectomy and radiotherapy for cervical cancer 30 years earlier. Fifteen months after the operation, she suffered from ureteroaortic fistula, which was controlled by intraluminal ureteral occlusion using Gianturco coils. During 53 months of follow up, she has been free of hemorrhagic episodes.

The question of liver transplantation for hepatobiliary malignancy has continued to generate controversial discussion. As shown by single-center studies and large databases, there is a clear indication for total hepatectomy and liver replacement under the premises of appropriate selection of suitable patients as well as of favorable type and stage of tumors. Future improvement of tumor-free patient survival can be expected from better understanding of tumor biology, including prevention and earlier detection of cancer, and effective multimodality treatment strategies.

During treatment, an apparent toxic skin reaction occurs before dermal quiescence ensues. We report a case of a typical flare of AK in a woman treated with capecitabine for advanced breast cancer.
