Immunosuppressed persons are at greater risk of developing malignancies. In human immunodeficiency virus (HIV) immunosuppression the most common oral cancers are Kaposi's sarcoma and non-Hodgkin's lymphoma. Squamous cell carcinoma has also been reported to be associated with HIV disease. Kaposi's sarcoma is the most frequent neoplastic disease in acquired immunodeficiency syndrome and is by far the most common in the head and neck area. This article reviews the prevalence, clinical features, and management of these diseases in HIV infection.

TITLE: Bladder sparing surgery for locally advanced female urethral cancer.

Cystectomy remains one of the standard treatments for women with locally advanced (stage C) urethral cancer, or stage A or B lesions that have failed radiation therapy. Often, the urethral neoplasm does not invade the bladder. We treated 5 women with stage C urethral cancer by wide local excision with or without concomitant radiation therapy, while preserving the bladder. Local control was achieved in all patients with followup ranging from 4 to 75 months (median 42). We believe that bladder sparing treatment strategies should be considered for selected stage C tumors when the bladder is not invaded by the neoplasm.

Brain metastases from primary cervical carcinoma are uncommon. Only 41 cases have been reported so far in the English literature. We have seen 2 cases over the last 2 years among 481 patients with cervical cancer. Brain metastases developed 7 and 35 months after the primary diagnosis. One patient with solitary metastasis had no evidence of disease elsewhere and is free of disease after surgery and radiation therapy 7 months after the brain metastasis. Another patient with multiple brain metastases and cervical disease is alive 2 months after palliative radiation therapy.

The treatment of superficial bladder cancer is aimed at eradicating existing disease, inhibiting tumor recurrence, and preventing progression to invasion or metastasis. Transurethral resection is the primary modality, with intravesical therapy used in some cases to kill residual overt or occult carcinoma and premalignant lesions. Among the agents tested extensively, BCG appears to be more effective than mitomycin C, doxorubicin, or thiotepa, especially against carcinoma in situ. A number of questions remain to be answered about the management of this disease.

The advantages of this method relate to the asymmetric three-dimensional growth of basal cell carcinoma (BCC). A device is described here based on published knowledge about the growth of BCC. The model demonstrates the precise way in which Mohs surgery varies from other surgical approaches to yield higher cure rates in specific circumstances. This device may be helpful in educating residents and medical students about Mohs surgery and in preparing patients for this procedure.

12 cases of human lung cancer tissue were analyzed for the presence of transforming growth factor-alpha with RIA. Histologically 9 were squamous cell carcinoma and 3 were adenocarcinoma. Except one squamous carcinoma, TGF-alpha could be detected in 11 cases. TGF-alpha level was from 0.26 ng to 1.26 ng per gram tissue. There was no significant difference in TGF-alpha level between squamous cell carcinoma and adenocarcinoma. The relationship between TGF-alpha and human lung cancer remains for further study.

In direct G-banded preparations of 150 cases of cervical cancer, double minutes (dmin) were observed in 43% (64 cancers) of the patients, including a case of carcinoma-in-situ (CIS). The incidence of dmin varied from 1 to over 20 per cell. The karyotypic findings in the tumors containing dmin did not reveal any similarities and the prognosis of the patients with dmin was significantly worse compared to those without dmin. The presence of dmin in cytogenetic preparations of primary tumors may indicate a role in the maintenance and/or progression of malignancy, particularly as dmin have been suggested to represent amplified genes.

Interleukin-4, a cytokine with pleiotropic effects, shares some common properties with interleukin-2. Availability of recombinant interleukin-4 has led to a better knowledge of this cytokine's functions, and also makes clinical trials possible. This brief review discusses the in vitro and in vivo data that support a potential use of interleukin-4 in cancer immunotherapy.

The relationship between cancer and abnormalities of blood coagulation has been recognized for well over a century. Deep venous thrombosis of the lower extremities is most common, but pulmonary embolism, upper extremity vein thrombosis, disseminated intravascular coagulation, and other, more unusual, clinical events may occur. Chemotherapy also has been linked to thrombotic episodes. Anticoagulant therapy is appropriate in many patients with venous or arterial thrombosis. Prophylaxis of venous thrombosis is particularly important in hospitalized cancer patients who are at bed rest or undergoing surgery.

TITLE: [The role of the second-look operation in modern therapy of ovarian carcinoma]

The authors evaluate the results of second look operations of ovarian cancer in a group of 34 women. The second look operation was preceded by intraperitoneal and intravenous chemotherapy. This therapeutic protocol contributed to improvement of therapeutic results. In 50% of thus treated women it proved possible to remove the residual tumour and the survival time was prolonged in 75% of women by 1-3 years. The advantages of this therapeutic procedure are according to the authors in particular reduction of the risk of super-radical primary operations and facilitation of the decision whether to terminate treatment or proceed with it.

Murine monoclonal antibodies to human small cell lung cancer (SCLC) have been developed and partially characterized. Primary hybridoma clones were screened in the indirect immunofluorescence assay (IFA) on alive H417 cells. Then five clones (IgG1, IgG2a, IgG3 and IgM) non-reactive with normal human bone marrow cells and positively reactive with SCLC tumors were selected. The H417.3 antibody is directed against 47-50kD surface antigens of H417 cells. The antibodies are supposed to be applied for the immunodetection of SCLC metastases to bone marrow and immunotoxin preparations.
