Twelve years' experience with neutron beam therapy in Britain, the USA, Europe and Japan shows that local control is achievable in late-stage epidermoid cancer somewhat more frequently than with conventional radiotherapy. Tumours reputed to be radioresistant (salivary gland, bladder, rectosigmoid, melanoma, bone and soft-tissue sarcomas) have proved to be particularly responsive to neutrons. Pilot studies in brain and pancreatic tumours suggest promising new approaches to management of cancer in these sites. The availability of neutron therapy in the clinical environment opens new prospects for irradiation of 'radioresistant' tumours, permits more conservative cancer surgery, expands the use of elective chemotherapy and provides a wider range of options for cancer patients.

TITLE: Definitive irradiation of T1-T4N0 larynx cancer.

The use of definitive irradiation in T1-T4N0 larynx cancer was reviewed in 236 patients at Rush-Presbyterian-St. Luke's Medical Center in Chicago. Recurrence-free survival for all stages was 86% at five years. Ultimate recurrence-free survival, including successful surgical salvage, was 91% at five years. The data described herein and the literature reviewed support the recommendation for definitive radiation therapy for T1 and T2N0 larynx cancer of any site. T3 lesions may also be reasonably treated with radiation therapy initially, reserving surgery for salvage. However, more data are needed to accurately identify the T3 larynx cancer patients most suited for radiation therapy.

TITLE: Efficacy and side effects of the regional arterial infusion of anti-cancer drugs combined with direct hemoperfusion in malignancies of the urinary tract.

Regional arterial infusion of high doses of anti-cancer drugs combined with direct hemoperfusion was done in 12 patients with malignancies of the urinary tract. The perfuser contained 130 gm. activated charcoal, which was coated with cellulose. General toxicity caused by escape of the drug from the tumor area was prevented by adsorption within activated charcoal. The capacity of activated charcoal to adsorb doxorubicin hydrochloride was excellent, as determined by measurements of plasma concentration. With this method, objective tumor reduction was noted in 84 per cent of the patients with prostatic or bladder carcinoma, although results were disappointing in patients with renal cell carcinoma.

TITLE: A spiral-grooved endoesophageal tube for management of malignant esophageal obstruction.

Dysphagia is common in patients with cancer of the esophagus or cardia. The rate of resectability of the lesion is low, and the majority of patients require palliation to relieve the dysphagia. Esophageal intubation is a simple, safe method at present. However, migration of the tube is a frequently reported complication. To prevent dislodgment of the tube, an esophageal tube with spiral grooves and a distal ring has been devised and used in 334 patients in the Veterans General Hospital, Taipei, Taiwan. There have been no operative deaths. The postoperative complication rate was 15%; migration of the tube comprised only 2.7% of that figure.

TITLE: Evidence of breast cancer mortality reduction: aggressive screening in women under age 50.

Five year follow-up data for breast cancer screening of 10,531 self-selected women are presented. The data are compared with the Health Insurance Plan of New York population and it appears that the magnitude of the mortality reduction in these two populations is similar. However, contrary to the New York findings, the data demonstrate that the benefit to screening this population was maximal in women under age 50. No breast cancer deaths have been recorded in 5 years of follow-up in this younger population (6,030 individuals). Interpretation of the data is that aggressive screening for breast cancer can favorably alter breast cancer mortality, particularly in younger women.

The mothers of 1731 children who died of cancer in the United Kingdom between 1972 and 1981, together with the mothers of 1731 matched controls, were asked about exposure to diagnostic ultrasound during the relevant pregnancy. The claims of 103 case mothers and 103 control mothers were verified by hospital records. Exposure to ultrasound in utero did not differ between cancer and control children. It is concluded that ultrasound is safe as regards the risk of cancer and leukaemia. The observed difference between cases and controls exposed during the earlier years of ultrasound use may be due to the selective application of this technique to abnormal pregnancies at that time.

Cis-diamminedichloroplatinum (CDDP) was administered i.v., at the dosage of 20 mg/m2 for 5 consecutive days and recycled every 3 weeks, to 11 patients with bidimensionally measurable metastatic lesions from bladder cancer previously untreated with antineoplastic drugs. Eight patients (6 men and 2 women) were evaluated for clinical response and 9 for toxicity. No complete regression was observed, and partial regression was obtained only in 3 patients (37.5%). Severe leukopenia and thrombocytopenia occurred only occasionally; moderate nausea and vomiting were observed in 5 cases. Nephrotoxicity was noted in 4 of the 9 patients evaluable for toxicity, and 2 of them, with ureteral obstruction, died of renal failure.

The use of geographic data to interrelate mortality rates and environmental variables is an important approach to determining reasons for the variations in disease frequency. An alternative methodology to standard regression analysis was recently suggested, which is particularly useful when there are a large number of geographic areas and relatively few deaths. An extension of this technique is described, which includes a modified regression approach using areas aggregated according to their level of the environmental factor. An example is given relating nasal cancer mortality to the distribution of industry in England and Wales. The known hazards in the furniture and leather industries are indicated and other potential risk industries, including tailoring, are suggested.

TITLE: Phase II evaluation of bleomycin in patients with advanced epithelial ovarian cancer.

Twenty-four patients received bleomycin twice weekly im. Treatment was given for 3 weeks initially, and if there was no evidence of disease progression treatment was given for another 3 weeks. Four patients had partial responses of 2, 4, 4, and 5 months' duration. Three patients had stable disease which did not progress for at least 4 months. There were ten patients with ascites; three of these showed a significant reduction in the amount of ascites present during treatment. There was a partial reduction of tumor in one of these patients. Toxicity was minimal with this regimen, and bleomycin deserves further study in the treatment of ovarian cancer.

In a study of 144 patients with cystic dysplasia of the breast diagnostic and therapeutic accuracy of palpation, mammography, aspiration cytology and pneumocystography was investigated. Only in 9.7% of the cases a surgical biopsy was necessary. In more than 90% of the cases biopsy was not performed; instead of this follow up with palpation, mammography and aspiration cytology up to 11 months was done. No breast cancer was overlooked, three of four proliferating dysplasias were detected. In 21% a local cystic recidive occurred which again was punctured. In contrary to surgical biopsy follow up of cystic dysplasias is not complicated by scars and their diagnostic problems. No complications were seen.

TITLE: Immunocytochemical localization of testosterone in human hepatocellular carcinoma.

A study of 10 hepatoma patients was carried out for the detection and localization of oestrogen, progesterone, testosterone, and their corresponding binding sites in liver tumour cells using immunoperoxidase and immunofluorescent techniques on histopathological sections and cultured monolayers. None of the hepatomas was positive for progesterone and its receptor; one was positive for oestrogen and its receptor; five were positive for testosterone and its receptor. Oestrogen, testosterone and their respective receptors localized predominantly to the cytoplasm of hepatoma cells. The results obtained in this investigation reveal an association between testosterone and human liver cancer which may be useful in the selection and monitoring of liver cancer patients to future hormonal therapy.

A patient with an advanced adenocarcinoma of the ovary developed spontaneous central nervous system (CNS) bleeding as initial evidence of brain involvement by a tumor. The rarity of this phenomenon in patients with ovarian tumors is emphasized and the incidence and course of brain metastases in ovarian cancer are reviewed. Similar to previous reports, our patient had extra abdominal systemic disease at the time CNS involvement was diagnosed. Although good short-term results were achieved with whole-brain irradiation, the prognosis for these patients remains dismal. The incidence of brain metastases may increase in patients with advanced ovarian malignancies given the possibility of achieving long-term survival in a significant percentage of women with widespread disease being currently treated with intensive systemic chemotherapy.

Serum carcinoembryonic antigen (CEA) and salivary CEA levels were determined for 18 patients with squamous oral carcinoma, 12 patients with oral leukoplakia, and 14 healthy volunteers. The determination of the CEA-serum levels has no diagnostic significance. Salivary CEA levels are not correlated with CEA-serum values, with cigarette consumption and clinical stage. Salivary CEA determination provides no additional information for the primary diagnosis. Serial determinations may have adjunctive value in monitoring oral cancer.

Sulbactam/cefoperazone (SBT/CPZ), a new developed antibiotic, was clinically studied. The following results were obtained. Total of 5 cases comprising 1 with endometritis, 3 with Bartholin's abscess, 1 with pyometra (due to corpus cancer) were treated with SBT/CPZ at a dose of 1 g twice daily for 4-5 days by intravenous injection or intravenous drip infusion. The clinical response was good in all cases. A case with pyometra did not respond to the therapy in bacteriologically. Side effects and abnormal laboratory findings due to the drug were not noted.

Preliminary data on the use of a combined surgical-polychemical-immunoradiotherapy protocol in the treatment of 13 T2N2M0 lung cancer patients are reported. The results are compared with those obtained in a 13 patient control group, homogenous by histotype, stage and surgical treatment who were given no other therapy. At 24 months the average survival rate in the first group was 61.5% compared to 15.3% in the second. It was significant that survival at 24 months after pneumonectomy rose from 16.6% to 50% and even more significantly survival after lobectomy rose from 14.2% to 71.4%.
