Immunosuppressed persons are at greater risk of developing malignancies. In human immunodeficiency virus (HIV) immunosuppression the most common oral cancers are Kaposi's sarcoma and non-Hodgkin's lymphoma. Squamous cell carcinoma has also been reported to be associated with HIV disease. Kaposi's sarcoma is the most frequent neoplastic disease in acquired immunodeficiency syndrome and is by far the most common in the head and neck area. This article reviews the prevalence, clinical features, and management of these diseases in HIV infection.

The methodology is applied to data involving DNA adduct counts and smoking variables among ex-smokers with lung cancer. A more detailed investigation is made of the parametric relationship between the number of adducts and years since quitting while retaining a smooth relationship between adducts and the other covariates.

As the human genome sequencing expands and the molecular bases of diseases are better understood, the tools that allow gene packaging and transfer are available. TITLE: Bladder sparing surgery for locally advanced female urethral cancer.

Cystectomy remains one of the standard treatments for women with locally advanced (stage C) urethral cancer, or stage A or B lesions that have failed radiation therapy. Often, the urethral neoplasm does not invade the bladder. We treated 5 women with stage C urethral cancer by wide local excision with or without concomitant radiation therapy, while preserving the bladder. Local control was achieved in all patients with followup ranging from 4 to 75 months (median 42). We believe that bladder sparing treatment strategies should be considered for selected stage C tumors when the bladder is not invaded by the neoplasm.

Ras proteins send signals through multiple effector pathways. The Raf/MEK/MAPK and PI 3' kinase pathways are well-validated Ras effectors in human cancers, but many other candidate pathways could be equally important. RalGDS is such a candidate: in a new paper from Chris Marshall's group, an important role for RalGDS in Ras transformation in vivo has been established for the first time. Mice lacking RalGDS are defective in tumor formation, possibly because of increased apoptosis in Ras-driven tumors. The hunt for a clear role for RalGDS activation in human cancer is on.

Brain metastases from primary cervical carcinoma are uncommon. Only 41 cases have been reported so far in the English literature. We have seen 2 cases over the last 2 years among 481 patients with cervical cancer. Brain metastases developed 7 and 35 months after the primary diagnosis. One patient with solitary metastasis had no evidence of disease elsewhere and is free of disease after surgery and radiation therapy 7 months after the brain metastasis. Another patient with multiple brain metastases and cervical disease is alive 2 months after palliative radiation therapy.

The concept of aggression was long relegated to the shadow of the libido in psychoanalytic theory, placed among the negative affects by some theoreticians, denied the role of a primary drive among leading writers in clinical psychology, brought to the fore as an adaptive force in ethology, and proved to be a decisive factor in the development of psychosomatic ailments. The second part of the paper reviews experiments using projective techniques, showing that highly creative subjects as opposed to ones low in creativity seem inclined to accept their aggressive impulses. A total denial of these impulses has been typical of women with breast cancer.

The treatment of superficial bladder cancer is aimed at eradicating existing disease, inhibiting tumor recurrence, and preventing progression to invasion or metastasis. Transurethral resection is the primary modality, with intravesical therapy used in some cases to kill residual overt or occult carcinoma and premalignant lesions. Among the agents tested extensively, BCG appears to be more effective than mitomycin C, doxorubicin, or thiotepa, especially against carcinoma in situ. A number of questions remain to be answered about the management of this disease.

The advantages of this method relate to the asymmetric three-dimensional growth of basal cell carcinoma (BCC). A device is described here based on published knowledge about the growth of BCC. The model demonstrates the precise way in which Mohs surgery varies from other surgical approaches to yield higher cure rates in specific circumstances. This device may be helpful in educating residents and medical students about Mohs surgery and in preparing patients for this procedure.

The authors present the case of a 17-year-old girl with tuberous sclerosis (TS) who presented with symptoms of intussusception. Although endoscopically diagnosed with multiple colonic polyps, presumed to be hamartomas, and an invasive rectal adenocarcinoma, postoperative pathology findings confirmed the rectal cancer and showed multiple colonic adenomas. Multiple colonic adenomatous polyps in a young girl with tuberous sclerosis is extremely rare. Furthermore, we believe that this is the first report of an invasive adenocarcinoma of the large intestine occurring in a patient with TS.

Ricin is a very toxic RIP II type lectin. As no effective antidote is known, its toxicity could be misused in terrorist actions. The mechanism of the toxic action is currently studied for the preparation of selective immunotoxins, which could be used in the therapy of various cancer diseases or HIV infection. There are promising in vitro results, but some adverse in vivo effects limit the use of these preparations in real therapy.

TITLE: Effects of temperature on polyunsaturation in cytostatic lipids of Haslea ostrearia.

Unusual chemicals produced by the-'blue oyster' diatom, Haslea ostrearia, include the water-soluble blue pigment marennine and numerous polyunsaturated sesterterpene oils or haslenes. Aqueous extracts of the alga exhibit in vitro and in vivo activities against human lung cancer cells and anti-HIV effects. Here we report that three haslenes also demonstrate in vitro cytostatic action against a human lung cancer cell line. The most active haslene is the most unsaturated and unsaturation in the haslenes increases with increasing algal growth temperature.
