Early detection of cancer of the colon and rectum is fundamental because survival is linked to the pathologic stage. The population groups at high risk are reviewed, as well as the different screening techniques: search for occult blood in stool (Hemoccult), rectal palpation, rigid rectosigmoidoscopy, flexible fiberoptic sigmoidoscopy, radiology and determination of carcinoembryonic antigen. It is concluded that the Hemoccult test is the most useful technique for early diagnosis and it should be performed in individuals over 40 years and in the high risk population. The other diagnostic tests should be carried out if this test is positive.

In conclusion, a review of the epidemiology and pathology of latent prostatic cancer suggests this tumor to be the probable initial phase of a biological continuum that ends with metastatic cancer. Histological grade and tumor volume are highly correlated, as are histological grade and mitotic index. It has also been suggested that atypical intraductal hyperplasia is, in turn, a precursor of carcinoma. This concept gains support from the close association of the two conditions among younger (less than 60 years of age) patients and from their similar mitotic and labeling indices.

In thoracic surgery patients the etiology of cavities has drastically changed in the last 20 years. While at present only very few patients need surgery for cavernous tuberculosis, cavities of cancerous origin are becoming more and more frequent. During the period from 1977 to 1985 24 out of 225 bronchogenic carcinomas treated by resection were found to be cavernous (10.7%). Radiologically the degree of cavitation extended from a small marginal or central air collection to the formation of large, irregular, and thick-walled caverns. The excavation is the result of necrobiotic changes within the tumor. It arises when the tumor masses penetrate into the bronchial tree and are expectorated. However, prognosis of cavitated cancer remains exclusively dependent on the stage of the tumor. The cavitation itself seems not to influence it.

Clinico-laboratory examinations of 86 patients have shown that lung cancer develops against the background of activation of the thrombocytic link of hemostasis which manifested itself in thrombocytosis, greater degree of destruction of the lysosomal membranes, lower energy resources of the cells, their tendency to slow and little reversible aggregation. The authors consider that the indicators of the ADP-induced aggregation of thrombocytes may be used for prognosis of postoperative pyo-inflammatory and thromboembolic complications. HBO is good for correction of alterations in the thrombocytic link of hemostasis in lung cancer and its suppurative complications.

Fifteen consecutive patients undergoing extensive surgery for head and neck cancer involving the oral cavity and oropharynx were reconstructed using the technique of rigid internal fixation combined with various vascularized flaps. The follow-up ranged from 6 to 18 months. Modifications in various myocutaneous or vascularized free flaps combined with an analysis of the radiation physics necessary to make this procedure a viable alternative for immediate and delayed mandibular reconstruction will be discussed. Consistently satisfactory functional and cosmetic results have been achieved.

High levels of c-myc mRNA were observed in two human tumor cell lines, a giant cell carcinoma of the lung (C-Lu99) and a colon cancer (C1). The increased expression of c-myc in these cell lines, which was comparable with those in cell lines in which the c-myc gene is amplified, was not due to gene amplification. Run-on transcription revealed that the transcriptional rate of the c-myc gene was greatly increased in these cell lines.

The concept of a field of carcinogenesis as applied to the uterine cervix is not new. The question of the field origin of cervical cancer is but a part of the general question of tumorigenesis and is compounded by the occurrence of multiple tumors in one woman. Simultaneous double cervical cancers in cases of uterus didelphys confirm the field effects. Methods of further study of the histogenesis of cervical cancer need to be found. The application of chromosome banding might well shed light on the question. It might offer some promise for the selection of patients for treatment and serve as an indicator of prognosis, as has been found for urinary bladder tumors.

104 patients with head and neck cancer were entered into a prospective controlled trial of radiotherapy in hyperbaric oxygen (HBO). The trial compared 10 fractions of radiotherapy in HBO with 30 fractions of radiotherapy in air. Survival and local control rates were significantly higher in the HBO group. No difference in normal tissue effects were detected. The greatest advantage of HBO was seen in the less advanced tumors.

In order to reinforce the difficult closure of the bronchial stump, or esophageal anastomosis, a pedicle flap, taken from the latissimus dorsi muscle, was applied to 7 patients with tracheal repairs, and 11 patients with extensively dissected areas, at the time of esophageal cancer surgery. Utilizing this technique, the complications associated with extended esophagectomy could be minimized. Intrathoracic application of the latissimus dorsi muscle flap is a useful method of supporting extended esophagectomy for esophageal cancer.

A retrospective study of 484 patients with locally advanced cancer of the breast treated with irradiation alone revealed a partial or complete response in 64% of the patients, with a mean duration of 13 months. The 5-year survival was 21.9% for the entire group of patients; 32% for patients responding, and 5% for non-responding patients. The data suggests that the size of the tumor and the age of the patients influence the quality of response. The results indicate that radiotherapy adequately controls local disease in a significant number of patients but systemic treatment is needed for better disease control.

In this paper, correlations between ischaemic heart disease and several cancers associated with cigarette smoking are examined. Also, tobacco consumption data are correlated against ischaemic heart disease mortality incidence rates. Furthermore, correlations between ischaemic heart disease incidences and per capita consumption of fattening foods (meat, milk and eggs) are analyzed. In this work, results referring to Spain, and at an ecologic level, confirm conclusions of observational and experimental studies carried out in other countries, which points out a strong association between ischaemic heart disease, cigarette consumption and fattening diet.
