In the light of the literature and 10 years personal experience, the following conclusions are drawn: 1. Screening for colon cancer with Hemoccult works and is practicable. 2. Asymptomatic and unknown symptomatic cancers are detected. 3. These cancers are predominantly in an early stage (70% at Dukes A and B). 4. Large adenomatous polyps, i.e. precancerous neoplasias, can also be detected. 5. One-time testing may miss 15-20% of cancers and 40-50% of adenomatous polyps (depending on their size!). 6. Repeated screening during the 2 years before diagnosis lowers these false-negative figures to less than 10% in carcinomas and less than 30% in large polyps. 7. Screening for colon cancer should be started at age 45 and thereafter repeated annually.

The influence of several factors for response to Chemotherapy and survival was analysed in 310 patients with inoperable lung cancer who were treated by single or combination chemotherapy. In small cell lung cancer, survival was strongly affected by the results of induction chemotherapy, and combination chemotherapy with three or four drugs was most effective. In non-small cell lung cancer, performance status and body weight loss on admission were important factors for response to chemotherapy and survival. Combinations did not improve the response rate in non-small cell lung cancer.

TITLE: Thyroid cancer in children and teenagers.

A study of thyroid cancer in patients younger than 20 years showed most tumors to be of the differentiated type. Histological types were pure papillary adenocarcinoma (9.5%), mixed papillary and follicular adenocarcinoma (77%), pure follicular adenocarcinoma (2.7%), and medullary carcinoma (10.8%). Of the patients with differentiated carcinoma, 84% had palpable cervical lymphadenopathy and 11% had pulmonary metastasis on admission. Patients with differentiated carcinoma were followed up for a mean of 15.4 years, and patients with medullary carcinoma for a mean of 7.3 years. No patient died as a result of papillary or follicular carcinoma, but two (25%) died of medullary carcinoma. Permanent hypoparathyroidism was found in 17% of patients after total thyroidectomy.

The membrane antigens of fifteen colorectal tumours were studied using a number of monoclonal antibodies and the immunoperoxidase technique on frozen sections. With this approach we could easily demonstrate differences in the membrane structure of the cancer cells within the tumour mass of given patients and also readily demonstrate differences between tumours that were indistinguishable by histological and other standard criteria. An unexpected finding was the patchy expression of HLA-DR antigens on cancer cells, in spite of the absence of HLA-DR on normal colorectal epithelium. These findings have interesting theoretical and clinical implications.

Morphological and immunocytochemical studies of pancreatic material from a patient with multiple endocrine neoplasia type-1 revealed hyperplasia of islets, multiple microadenomas, adenomas and cancers. Cellular composition of the hyperplastic islets was almost normal but the enlargement of the size and increase of the numbers were observed. Furthermore, neoformation of islets from ducts were found here and there. These hyperplastic islets, microadenomas, adenomas and cancers merged into each other and some of these microadenomas demonstrated malignant pictures. Most of the microadenomas produced multiple peptide hormones, on the other hand some of these revealed a single hormonal activity. These results show that the abnormality of the primordial cells of the pancreatic endocrine cells develops these lesion.

Twenty-three previously untreated patients with bronchioloalveolar cell lung cancer who had measurable disease and distant metastases (stage IIIM1, extensive) were treated with combination chemotherapy including 5-FU, vincristine, and mitomycin. Two of 23 patients (9%) achieved partial response lasting 5 and 6 months. Two patients (9%) died of sepsis while neutropenic. The current study does not justify the use of 5-FU, vincristine, and mitomycin combination chemotherapy in patients with metastatic bronchioloalveolar cell lung cancer.

The metastatic involvement of leptomeninges is an uncommon complication of systemic cancer, but there are evidences that its incidence is increasing, particularly for breast cancer. The two commonest primary sites are the breast and the lung and adenocarcinoma is the most frequent histologic type of carcinoma to metastasize to the leptomeninges. Clinical diagnosis is suggested by the simultaneous occurrence of symptoms and signs in more than one areas of CNS. Examination of cerebrospinal fluid is the most helpful diagnostic procedure. A combination of radiotherapy and intrathecal or intraventricular administration of Methotrexate seems to be the most successful treatment.

Since cancer registries have different recording practices, the incidence rates that they report must be compared with caution. Indexes of reliability of recording indicated that in 1971 the reported incidence of cervical cancer in Ontario was too high. In 1971 Ontario used a method of passive reporting of cancer cases: the Ontario Cancer Registry linked hospital reports, death certificates and reports from the Ontario Cancer Treatment and Research Foundation's treatment centres to produce a single record for each case. Pathological confirmation was requested for cases thus recorded by the registry. In 26% of cases a diagnosis other than cervical cancer was indicated. With these cases omitted, the incidence rate became 15.1/100 000, as opposed to the 20.5/100 000 reported by the registry.

During an eight-year period, serial plasma determinations were made on members of a Gardner's syndrome family in order to see if 1. levels rise when polyps first appear, and when cancer occurs; and 2. levels fall after colectomy with ileoproctostomy, ileostomy, or rectal mucosal replacement surgery. CEA analyses were carried out in the laboratory of Norman Zamcheck, M.D., using the Roche method. Small increases in CEA titer accompanied the appearance of polyps and small decreases accompanied their removal. Further serial studies are needed to determine whether changes in CEA titer can be indicators of polyp and/or cancer occurrence.

Among 83 consecutive, unselected female autopsy cases, 6 had been treated for invasive breast carcinoma (IBC) during lifetime. At autopsy, of these six women, two had IBC in the contralateral breast, and another two had in situ carcinoma. Among the 77 women without previous clinical breast cancer, one case of IBC and 14 cases of in situ carcinoma were found at autopsy. Thus, the total occurrence of primary malignant breast lesion was as high as 25.4%. Assuming that all IBCs evolve from in situ forms and that in situ carcinomas do not undergo total regression, our results seem to indicate that about one third of all in situ carcinomas develop into IBC.

TITLE: Activity of isotretinoin against squamous cell cancers and preneoplastic lesions.

We investigated the effect of the synthetic vitamin A derivative isotretinoin (13-cis-retinoic acid) on advanced cancers in 103 patients and on preneoplastic lesions in five patients. Six of 14 patients with squamous cell epithelial cancers had objective regressions of skin or subcutaneous metastases. Three of five patients with preneoplastic lesions had objective responses. The major dose-limiting toxic effects were reversible dermatitis, emotional lability, and headaches. We conclude that the growth of some squamous cell epithelial malignancies can be inhibited by isotretinoin and suggest that other retinoids should be evaluated as antitumor agents.

TITLE: Effective high-dose chemotherapy with autologous bone marrow infusion in resistant ovarian cancer.

Two patients with persistent minimal ovarian cancer after conventional polychemotherapy were treated with high doses of cyclophosphamide and VP 16-213 followed by autologous bone marrow infusion. Ten months afterward no clinical signs of tumor were apparent. In one patient the complete response was surgically documented. Toxicity included cardiac and pulmonary arrest during marrow infusion in one patient, but was otherwise manageable. This method of late intensification of chemotherapy in patients with persisting ovarian cancer merits further investigation.

A case-control study was performed on cancer of the nose, nasal cavities, middle ear and accessory sinuses from 1982 through 1984 in Hokkaido. Maxillary sinus cancer accounted for 84% of this cancer in men and 87% in women. Histologically, squamous cell cancer was observed in 91% of the men and 83% of the women. The age-standardized incidence rates of this cancer were 1.3 per 100,000 for men and 0.4 for women. These figures were compared with the age-standardized mortality rates of this cancer for 1975 and 1980 by sex.

Fifty patients with advanced, measurable, squamous-cell cancers of the head and neck received treatment with a combination of cisplatin, vinblastine, and bleomycin (PVB). The tumours in 25 patients (50%) responded to treatment (partially in 17 and completely in eight). The median duration of response was 18 weeks, and that of survival was 31 weeks. Toxicity was moderately severe; myelotoxicity occurred in 13 patients, and nausea and vomiting in 22 patients.

A 60-year-old man with nonresectable lung cancer underwent bronchoscopy which revealed a large squamous cell tumor mass narrowing the free airway to the left lung. A flexible quartz fiber connected to an argon ion laser was then inserted through a hollow channel of the fiberoptic bronchoscope. The laser was activated to heat the metal cap on the distal tip of the fiber to thermally dissolve the mass and relieve airway obstruction.
