The diagnostic approach in a patient with lung cancer should be oriented according to the mode of presentation of the disease. The first step includes history and clinical examination of the patient, and frontal and lateral chest X-ray, before histological typing of the tumor. The various diagnostic methods are selected according to localization of the mass in the thorax, and staging procedures are determined by the type of the tumor and the possible therapies available. The complexity of the problems surrounding patients with lung cancer mean that the general practitioner should seek the opinion of the chest specialist early rather than late in the pretherapeutic evaluation.

None of these agents has shown itself to be superior to bilateral orchiectomy, but their individual side effects profiles may make one more appealing than another. Total androgen ablation may offer a slight survival advantage over partial androgen ablation, but this must be weighed in terms of increased side effects and overall cost. Hormonal therapy remains an effective palliative but not curative means of treating advanced prostatic cancer.

One hundred and seven patients (57 patients with ulcerative colitis and 50 controls) were investigated for in vitro tetraploidy (IVT) in dermal fibroblast monolayer cultures. There was no difference in incidence of IVT between patients with ulcerative colitis and controls. We advance the hypothesis that the genetic background of the colorectal cancer type found in ulcerative colitis differs from that found in the colon cancer syndromes and in non-hereditary colorectal cancers. Colorectal cancer in ulcerative colitis is probably only dependent on the degree of inflammatory lesions of the colonic mucosa.

Cancer family history was studied for 43,117 subjects aged 35-79 at the occasion of stomach cancer mass screening and were followed up for 5 years. 376 cases of cancer were notified out of them by the linkage with records of population based cancer registry covering the prefecture. A group of 26 patients who were operated upon for idiopathic priapism between 1972 and 1986 were classified according to the date on which they attended hospital. During the same 15-year period 150 patients with penile cancer were treated and they served as controls. The data were analysed for circannual rhythmicity (365.25 days = 360 degrees and 0 degrees = 22 December. This revealed a significant circannual rhythmicity in the occurrence of idiopathic priapism, with the acrophase located on 2 August. There was a significantly higher incidence of idiopathic priapism between April and September.

TITLE: The current differential in black and white life expectancy.

The 1980 National Center for Health Statistics life tables for the U.S. black and white populations reveal a difference in life expectancy of 7 years between black and white males and 6 years between black and white females. Using cause-substituted life tables, we show that a number of causes of death contribute to the difference. The largest contributors are cardiovascular disease for both sexes and homicide and cancer for males.

This paper presents two advances in the evaluation of swallowing function which better define the swallowing processes in the head and neck cancer patient. Videofluoroscopy, improved by computer-assisted image analysis, provides more accurate information regarding oral and pharyngeal movement of structures. Radionuclide scintigraphy, a complementary technique, quantitates bolus flow (transit times) and the degree of aspiration. Several cases demonstrate the advantages of the methods and their usefulness in evaluating the effects of cancer treatment. The improved techniques also provide opportunity for designing new strategies of medical and surgical rehabilitation.

TITLE: [A case of advanced penile cancer treated with multimodal therapy--combination with tensor fascia lata myocutaneous flap]

A 40-year-old male with advanced penile cancer, whose left inguinal node was ulcerated at the time of initial presentation, underwent multimodal therapy. Four cycles of chemotherapy were given from September 20, 1984 to February 8, 1985. Partial penectomy and left ilioinguinal lymphadenectomy with removal of left groin ejaculation were performed on February 20, 1985, followed by right ilioinguinal lymphadenectomy on March 20, 1985. The skin defect of the left groin was covered with tensor fascia lata myocutaneous flap. The patient is alive with no evidence of disease 30 months after the surgery.

Literature data on photoaddition, photorecovery and photoaugmentation are discussed. The practical conclusion is that for estimates of the main risks of ultraviolet radiation to human skin, sunburn and skin cancer, any deviations from photoaddition are small. Risk estimates may be made on the basis of addition, and the errors possibly made by doing so are small compared to other uncertainties in such calculations. Deviations from photoaddition reported are interesting from the viewpoint of studying mechanisms. Apparent controversies cannot be resolved on the basis of presently available data. Confusion can certainly be reduced, and there are indications that more subtle techniques of investigation are needed to resolve the remaining problems.

Caudal epidural administration of morphine was used to provide pain relief to a 5-month-old infant with neuroblastoma and massive abdominal distension. Intermittent injections were performed on 6 occasions over 1 week and resulted in prolonged analgesia (17-20 h) and improved depth of respiration on each occasion. Epidural opioid analgesia should be regarded as technically feasible for relief of cancer pain in patients of all ages.

The results of gamma-beam therapy of esophageal cancer leave much to be desired. However betatron therapy improves them. To compare the results of esophageal cancer therapy 4 therapeutic modalities were employed: the 1st group--chemoradiotherapy (methotrexate at a weekly dose of 45 mg), the 2nd group--chemoradiotherapy (methotrexate at a dose of 10 mg alternating with prospidin at a dose of 200 mg), the 3rd group--radiotherapy (inhibitory radiation), the 4th group--radiotherapy (gamma-beam therapy). Higher efficacy of chemoradiotherapy as compared to radiotherapy only was established.

Between 1977 and 1986, 325 patients with a diagnosis of ovarian carcinoma, peritoneal cancer, or malignant mesothelioma were reviewed with identification of 23 patients (7%) having peritoneal papillary cancer. Only 2 patients had disease confined to the pelvis and the rest had widespread abdominal disease. Various combination chemotherapeutic agents were used with over a 65% response rate to first-line chemotherapy. Twenty of the 23 patients received cisplatin combination chemotherapy with an overall response rate of 65%.

These enzymes oxidize drugs and carcinogens as well as endogenous chemicals such as steroids and eicosanoids. As Peter Guengerich explains, activities of individual cytochrome P-450 enzymes vary among individuals as a result of both genetic and environmental influences; in some cases, the mechanisms are known. Such variation can have major influences in determining drug toxicity, inborn errors of steroid metabolism, and possibly cancer risk.

A case of minute gastric cancer which was preoperatively diagnosed, measuring 3 X 3 mm in diameter is reported. The X-ray examination revealed a faint barium collection surrounded by the mucosal folds. The endoscopic findings showed a small ulcerated lesion with a reddish and irregular margin and convergent folds. Radiological characteristics of the minute gastric cancer and its diagnostic approach are discussed.
