TITLE: [Preoperative ultrasonic diagnosis of metastasis to the cervical lymph nodes from otorhinolaryngeal neoplasms]
PUBDATE: 19851201
The results of a study on 30 patients with ear, nose and throat cancers are discussed; a comparison of clinical, ultrasound and histological findings for the preoperatory evaluation of the cervical lymph nodes has been carried out. Ultrasound technique is an excellent method of investigation for the staging "N" of such neoplasms; its contribution is very important for the following therapeutic program.

TITLE: Immunodiagnosis of colorectal cancer: clinical evaluation of Tennessee antigen (Tenagen).
PUBDATE: 19831101
Blood samples from 172 consecutive patients undergoing complete colon investigations to exclude colorectal cancer (CC) were examined for levels of Tennessee antigen, i.e., Tenagen (Tg), and carcinoembryonic antigen. Patients included 48 with CC, 34 with adenomatous polyps, 14 with colitis, and 76 with no organic mucosal disease. All patients were investigated by the one clinical and histopathology department. Laboratory estimations were performed blindly. Although there were some statistically significant differences among average values, the degree of overlap among the groups of patients precludes the practical application of Tg in the diagnosis of CC.

PUBDATE: 19840101
During the last decades, our ideas on the cancer-pregnancy association have been radically altered. Induced abortion, which has long been considered a prerequisite to successful treatment of cancer in such cases, is now performed only if requested by a fully informed mother or if embryotoxic chemotherapy is required. Many authors disagree that pregnancy has detrimental effects on cancer and believe that this only applies to breast cancers with axillary lymph mode involvement. Epidemiologists have shown that obstetrical factors are closely related to the risks of breast, endometrial and ovarian cancers. In some cancers, particularly in children and young adults, cure cannot be considered complete it has been obtained at the cost of infertility. To preserve fertility without reducing the prospects of cure is one of the major objectives of treatment in young cancerous patients.

PUBDATE: 19850201
Transection of the temporal branch of the facial nerve results in paralysis of the frontalis muscle with subsequent inability to wrinkle the forehead. In addition there is ptosis of the eyebrow with upper visual field defects. The intent of this article is to outline an orderly, rational approach to the management of such injuries based on our experience with seven patients whose nerve was damaged during Mohs surgery for cancer ablation. Repair options include various combinations of direct brow-plasty, indirect temporal or coronal browlifts, and adjunctive upper lid blepharoplasty.

PUBDATE: 19850601
Radioimmunoassays were employed to determine somatotrophin (STH) and somatostatin (SS) levels in patients with different types of tumors. In all patients with solid tumors and in 19 out of 25 patients with lympoblastoses SS concentration was elevated and ranged from 15 to 70 pg/ml. STH level in all cancer patients, with few exceptions, before and during treatment was within normal varying from 0.5 to 6 ng/ml. No distinct correlation between STH and SS levels was found. Proceeding from an analysis of the results obtained it is to be assumed that a study of SS behavior in cancer patients may hold promise in terms of the diagnosis and assessment of the efficacy of cancer therapy.

PUBDATE: 19850901
The report analyses the comparative efficacy of three different tumour markers (CEA, GICA and TPA) in providing indications for the postoperative monitoring of patients treated for cancer of the digestive tract. Assessment of effectiveness of chemotherapy produced different results according to the site of the tumour (stomach, colon or pancreas). In the case of cancers of the pancreatic head, the influence of jaundice on the markers was also evaluated, to discover which marker provided most information on the tumour investigated.

Of 8 patients, 7 patients achieved complete relief of nausea and emesis and 1 patient obtained partial elimination of symptoms. All these patients had consistent history of severe nausea and emesis before the methylpredisolone trial. At present, a potential effect of methylprednisolone on chemotherapy has not been known. In a case of testicular cancer the titer of tumor markers decreased straight on a table of logarithms.

Seven patients with superficially invasive vulvar cancer and lymph node metastases are reported. Histology of the primary tumor does not seem to correlate reliably with its metastatic potential in any individual case, though "spray," pseudoglandular, and small cell carcinomas may be particularly aggressive lesions. While wide local excision seems to be adequate treatment for the primary lesion, it is recommended that for lesions with greater than 1 mm of stromal invasion, bilateral inguinal-femoral lymphadenectomy should be performed for midline lesions, and at least ipsilateral inguinal-femoral lymphadenectomy should be performed for lateralized lesions.

PUBDATE: 19821201
In recent years several studies have been performed to improve the chemotherapy of metastatic gastric carcinoma. It is now established that combined chemotherapy for example with 5-Fluorouracil, adriamycin and mitomycin C or nitrosourea derivatives is superior to monotherapy with 5-Fluorouracil. With this combined chemotherapy, response rates of 30-40% can be achieved with prolonged medium survival time in responders. In locally advanced tumors the combined use of radiation and chemotherapy probably will be able to improve the results. Future studies are needed to show the role of adjuvant chemotherapy in gastric cancer patients after surgery.

PUBDATE: 19840801
Comparative studies of the anti-tumor effects of Adriamycin, 4'-Epi-Adriamycin, THP-Adriamycin and Mitoxantrone on rat mammary cancer induced by 7, 12-DMBA were performed. All 4 drugs were effective on the tumors, three administrations of Adriamycin, 4'-Epi-Adriamycin or Mitoxantrone at 1-week intervals showed remarkable tumor regression in each group. Mitoxantrone was not so effective in comparison with the other 3 drugs. Decrease in the body weight of the rats was most remarkably observed in the Mitoxantrone-treated group.

At present hyperthermia shows great promise when combined with other modalities. Local thermo-chemotherapy may also be a very attractive research field, but the combination of hyperthermia and anti-cancer drugs is still poorly understood. In this review, the background and the clinical reports on heat and chemotherapy which have been reported during the last year were analyzed. Finally, our clinical experiences with local hyperthermia combined with anti-cancer drugs for gastrointestinal cancer were also reported.

Two cases of systemic lupus erythematosus in association with primary gastric adenocarcinoma are presented. A pathological analysis of the two cases of gastric cancer does not support the hypothesis that lupus and its associated vasculitis is involved in the pathogenesis of gastric carcinoma. A critical review of the literature reveals numerous case reports suggesting the association of systemic lupus erythematosus with various malignancies; however, there is no definitive study which substantiates the association of gastric or other cancers with lupus. Future studies will be required to assess appropriately the possibility of an association between lupus and cancer.

Cases of endometrial carcinoma (104) from the Cancer Registry of Miyagi Prefecture, Japan, during the period from 1966--1977 have been reviewed histologically. The most impressive differences between tumors in this low-risk population and those reported in American women are higher frequencies of the mixed adenosquamous histology, myometrial vascular invasion, and lymphoid infiltration within and around the tumors in the Japanese cases. The possible significance of these observations, both clinical and epidemiologic, is discussed in detail.

Improvements in therapy for small cell carcinoma of the lung have been achieved with treatment regimens that result in significant toxicity. The workshop of the International Association for the Study of Lung Cancer focused on the following complications of chemotherapy and radiotherapy: leukopenia and resultant infections, esophagitis, pneumonitis and pulmonary fibrosis, cardiac toxicity, second malignancies, neurologic complications, and psychosocial effects. The data regarding the incidence and management of these complications are briefly reviewed, and recommendations for further studies are noted.
