Therapeutic options for the patient with systemic breast cancer were long limited to either chemotherapy and hormonal therapy. While these therapies provide palliation for many patients with advanced disease, and occasionally cure the patient with micrometastatic disease, they are not optimal either with regard to toxicity or efficacy. This paper reviews the past of systemic therapy (chemotherapy and hormonal therapy), then discusses developing treatment strategies. These strategies include the recently introduced anti-HER-2 antibody trastuzumab, novel agents targeting other aspects of tumor growth mechanisms, as well as agents blocking angiogenesis.

A 76-year-old man had undergone omentopexy for empyema. He was found to have gastric cancer and revealed Borrmann III by gastroendoscopy intraoperative period. Segmental gastrectomy was performed without dissection of lymph nodes, because his general condition was poor. After operation, the patient showed septic shock, ARDS and hepato-renal failure but his condition was getting improved by conservative therapy. After 5 years later, he was examined gastro-endoscopy, abdominal and chest CT and revealed no recurrence sign.

This article provides an overview of the problem of genetic susceptibility to childhood cancer with a particular emphasis on problems with ascertaining inherited cancer risk and the role of tumor-suppressor gene mutations in cancer predispositions. The association between neurofibromatosis type 1 and childhood leukemia is used to illustrate some of the issues faced by molecular biologists and genetic epidemiologists in identifying and analyzing at-risk individuals. The problem of incomplete penetrance in cancer susceptibility is presented and potential models are discussed. The article concludes with a number of tentative conclusions from existing data and speculations for future studies.

A 46-year-old man was diagnosed as Stage IV lung cancer with neck lymph node and bilateral adrenal metastases. He was treated with seven courses of combined chemotherapy (CDDP + etoposide), until he showed extreme hypercalcemia and acute renal failure. The bisphosphonate, pamidronate with elcatonin and prednisolone dramatically lowered his serum calcium level and normalized his renal function. This is a representative case of humoral hypercalcemia of malignancy whose serum calcium could be safely and effectively controlled with pamidronate.

However, our case shows no round shadow indicating an enlarged lymph node, suggesting that the mediastinal adenocarcinoma originated from mediastinal tissue.
