TITLE: Breast cancer detection in an institution. Is mammography detrimental?

The poor results of the National Breast Screening Study of Canada regarding the screening of women aged 40-49 have been interpreted to suggest that mammography might be detrimental. Our comparison of women who had mammography and had their cancers detected by mammography with women who did not have mammography and had palpable cancers suggest that this is not the case. There is no detrimental effect; rather, the use of mammography results in an improvement in survival. The poor results of the National Breast Screening Study likely are due to the unbalanced allocation of women with advanced cancers to the screened group, the poor quality of the mammography in the trial, and an insufficient sample size.

Inflammatory breast cancer is a rare disease characterized by a frequently fatal outcome despite combined modality approach used. Its precise definition is mainly clinical. A positive skin biopsy does not seem to be mandatory to establish the diagnosis. Induction chemotherapy applied before loco regional treatment has improved the 5 year overall survival. This chemotherapy is followed by a loco regional treatment consisting of either surgery or radiotherapy. The use of adjuvant chemotherapy is classical but its real value has never been assessed by appropriate randomized trials. Therapeutic improvements will perhaps come from more intensive induction chemotherapy regimen or from intensification of adjuvant treatment. Adequate randomized trials are needed to progress with this disease.

It is difficult to predict the tumour invasion into the mandible by oral cancer pre-operatively, and consequently the decision to preserve or sacrifice the mandible is largely individualistic. The present study of 44 cases analyses the reliability of pre-operative parameters to assess mandibular involvement. Clinical, radiological and scintigraphic features have been compared with the detailed histology of the bone. This study confirms the usefulness of superior marginal resections in lesions which are close to but not involving the mandible, as well as for superficial lesions which are actually seen to involve the mandible but their CT and bone scans are negative for tumour invasion.

TITLE: [Initial results of combined surgical-medical therapy for metastatic cancer of the stomach]

Fifty patients with advanced gastric cancer underwent chemotherapy in accordance with the FAMTX protocol. Complete tumor remission was achieved in 6 patients and partial remission in 11 patients. In this group 9 patients (tumor stage: T3-4, N2-3, M chi-1) underwent a second-look operation after chemotherapy either to confirm the complete remission or to resect the remaining tumor mass. It is concluded that even in advanced stages of gastric carcinomas a more aggressive approach involving a combination of surgical and chemotherapeutic treatment improves the survival time of these patients.

TITLE: [A case of external supravesical hernia]

The first report of external supravesical hernia was made by Astley Cooper in 1804, but it is so rare that we could not find any case reported in Japan. Here we report a case of external supravesical hernia in a male who had bladder cancer. This hernia was reported to be caused by fragility induced by operation or radiotherapy. It can be confused with inguinal direct hernia because it is not considered in the differential diagnosis. Findings such as a palpable hernia sack and the orifice can enable correct diagnosis. Surgical treatment is the only treatment of choice for this disorder.

Two cases of prostatic cancer with single bone metastasis in the tibia are discussed. The intense uniform involvement of a solitary limb bone with high perfusion and blood pool activity in the 3-phase bone scan and the positive white blood cell scan (observed in one case) were not typical for a metastatic bone lesion. Conventional radiomorphology--lamellar, periosteal reaction, disseminated medullar sclerosis, no localized lesion--also led to other differential considerations such as osteomyelitis and malignant lymphoma, which could not be specified by CT and MRI. Even if there is no typical morphology in scintigraphic and radiologic imaging, biopsy should be performed to exclude bone metastasis in prostatic cancer.

Pure squamous carcinoma is a rare pancreas cancer histological variety. Moreover the association of malign humoral hypercalcemia and neoplasia of the pancreas exocrine portion is extremely rare. The case of a patient with pure squamous pancreas carcinoma and hypercalcemia is discussed. The absence of bone metastasis allowed to classify it within the malign humoral hypercalcemia syndrome. It is the first case described in the spanish scientific literature and the third in the international literature of an epidermoid pancreas carcinoma associated to malign humoral hypercalcemia.

Microtubules are among the most strategic subcellular targets of anticancer agents. Recently, new antimicrotubule agents have been introduced to clinical cancer chemotherapy. Navelbine is a new derivative of vinca alkaloids, which especially has an activity against NSCLC. Unlike classical anti-microtubules agents like colchitine and vinca alkaloids, which induce depolymerization of microtubules, taxol has an unique mechanism of action which induces tubulin polymerization. Taxotere, an analogue of taxol is slightly more active as a promoter of tubulin polymerization than Taxol. E7010 is a novel sulfonamide that inhibits tubulin polymerization. Results of preclinical and clinical studies of these agents are reviewed.

The relevance of cytologic procedures in the diagnosis of malignant pleural effusions is evaluated. Pleural fluid sample obtained from 48 patients with malignant disease (39 metastatic tumors, 9 mesotheliomas), were studied to search for cancer cells. The results were: 32 (66.6%) patients had a cytologic diagnosis of malignant pleural effusions; 16 malignant specimens were not confirm by pleural fluid cytologic analysis. In our experience the cytologic diagnosis of malignant pleural effusions are more frequent in metastatic disease, especially in lung cancer.

A 48-year-old male was referred to our division, on the suspicion of lung cancer with bone metastasis. Chest radiography showed a mass shadow and bone scintigram demonstrated multiple scattered regions of increased uptake. However bronchial and bone biopsy specimens revealed granuloma with epithelial cells and giant cells. A diagnosis of tuberculous osteomyelitis was made, and antituberculous therapy was begun. Chest radiography and bone scintigrams one year later showed marked improvement.
