Immunotoxins are hybrid molecules made up of an antibody and of the toxic subunit of a polypeptidic toxin. They act on the principle that the antibody binds the molecule to a cellular antigen which it specifically recognizes, so that the cytotoxic component only kills those cells that bear the antigen. Cell hybridizations obtained by fusion now produce monoclonal antibodies specific enough to bind the antigens present on certain malignant cells. In vitro experiments have demonstrated the high specific activity of immunotoxins against malignant cells, and this seems to be confirmed by in vivo experiments in mice. Such experimental results suggest that immunotoxins may eventually be used in the treatment of cancer as well as for other pharmacological purposes.

TITLE: A cancer-associated lactate dehydrogenase is expressed in normal retina.

An unusual isozyme of lactate dehydrogenase, lactate dehydrogenase k, is found in high concentrations in cultured cells transformed by the Kirsten murine sarcoma virus and in many human cancer tissues. In experiments described here high levels of a lactate dehydrogenase k activity were detected in extracts of normal rodent retina. This activity had the same key properties as the human tumor isozyme, namely, a highly cathodic electrophoretic mobility and inhibition of enzymatic activity by oxygen and 5',5'-dipurinenucleoside tetraphosphates. Expression of this activity in the retina may be related to the high aerobic glycolysis characteristic of the retina, a metabolic feature shared with many tumors.

Infection with Herpes simplex viruses (HSV) induces amplification of SV40 sequences in SV40-transformed Chinese hamster embryo cells (CO631). This is shown by in situ hybridization of the infected cells with cloned 32P-labelled SV40 DNA. The HSV-mediated synthesis of SV40 DNA is more pronounced than after treatment with chemical carcinogens. This initiator-like effect of HSV may, in concert with the previously reported mutagenic activity of this virus (Schlehofer and zur Hausen, 1982), point to a possible mechanism of HSV infection in human genital cancer.

On the basis of an analysis of results of studying 541 patients with lung cancer the authors consider that bronchoscopy can diagnose the central lung cancer practically in all the patients including the early stages of the disease before the development of changes in the lungs on roentgenograms. The histological verification of the peripheral lung cancer of the root and median zones is ensured in 77% of patients by bronchofibroscopy under local anesthesia with the transbronchial peripheral forcepts and brush biopsy under the roentgen and TV control. Bronchological diagnosis of cancer of the subpleural zone still remains less effective.

TITLE: Intramucosal carcinoma of the gastric antrum complicating Menetrier's disease.

A 48-year-old man presented with haematemesis and was found to have Menetrier's disease. Three and a half years later early gastric cancer was detected at routine follow-up endoscopy. This is the first report of the development of early gastric cancer during follow-up in a case of Menetrier's disease. It is probable that patients with Menetrier's disease are at increased risk of gastric cancer, and should have regular endoscopic follow-up with gastric biopsy.

One thousand seventy-eight patients diagnosed with primary breast cancer were examined for racial differences in histopathologic and clinical parameters. There were no observed differences in tumor histopathologic type or tumor endocrine status between races. There were no differences with respect to time to breast tumor recurrence observed between black and white patients. However, differences were observed in factors that contributed to tumor stage at diagnosis and to tumor grade. Survival differences observed in univariant analysis of blacks vs. whites were explainable by the presence of more severe skin involvement, tumor grade, and tumor size at diagnosis in the black patients.

Remarkable chromosome abnormalities were observed in bone marrow cells from a woman with chronic myelocytic leukemia and atypical tuberculosis due to Mycobacterium avium-intracellulare infection. Four chromosome breaks occurred at bands 1p13, 1q32, 11p15, and 22q11. These breaks resulted in a complex Philadelphia (Ph) translocation between chromosomes #1, #11, and #22 and in an inversion of chromosome #1. Oncogenes on these chromosomes include N-ras and c-sk on chromosome #1, c-H-ras on chromosome #11, and c-sis on chromosome #22. Complex chromosome rearrangements may facilitate multiple oncogene changes, thereby permitting several steps in cancer development to occur simultaneously.

The following studies were undertaken to determine the levels of serum carotene and vitamin A (retinol) in Egyptian patients with intestinal, urinary, hepatosplenic schistosomiasis, and bladder cancer. These studies have shown that both serum carotene and serum retinol levels are significantly reduced in patients with bladder cancer when compared to controls. These data, when viewed in light of previous animal studies noted, raise the question as to whether vitamin A administration can be beneficial in the prevention and/or treatment of human bladder cancer, particularly that associated with schistosomiasis.

A radionuclide study of liver, small intestine function and a radioimmunoassay to determine the level of insulin, C-peptide and gastrin in the blood were performed in 177 patients with cervical cancer before and during radiation therapy and in 77 healthy women. Combined radiotherapy of cervical cancer was shown to result in disorder of absorptive-excretory function of the liver, disturbed fat assimilation in the small intestine and a decrease in gastrin production. Hyperfunction of the pancreas endocrine apparatus was noted in the cervical cancer patients, and radiation therapy caused the suppression of insulin production.

TITLE: [Survival and factors related to the survival of recurrent breast cancer patients--a study with quantification theory]

Survival and factors related to survival were determined, using quantification theory, in 41 patients with recurrent breast cancer treated by a multidisciplinary approach. Average survival was 33 +/- 22 months (average +/- standard deviation), 50% survival was 32 months and 49% survived for more than 36 months. The important survival factors were the histological stage and effective therapy. First recurrence sites, menopausal status, number of organs involved in cancer recurrence and histologic type were not important factors to the survival. Using these 6 factors, we can distinguish the long survival group from the short survival group with 75% accuracy. The cumulative correlation ratio was 0.764.

Urinary levels of the glucuronides of androsterone, aetiocholanolone, 5 alpha-androstane-3 alpha, 17 beta-diol and 5 beta-androstane-3 alpha, 17 beta-diol, were determined in twenty-two-post menopausal women both in spring and in autumn--winter. In sixteen women dehydroepiandrosterone sulphate plasma levels were also measured. Plasma DHAS levels as well as urinary metabolite excretion values were significantly higher in autumn--winter than in spring. This seasonal variability should be taken into account in long-term studies involving adrenal hormone levels, excretion or metabolism. Moreover this seasonal variability might be of relevance for the cyclical growth rate of hormone dependent tumours such as breast cancer.

Individuals with cancer share many similar nutritional problems, regardless of the primary site of the malignant neoplasm. The presence of protein-calorie malnutrition is the most common secondary diagnosis in cancer and is frequently the cause of death. To date, the inability of health care professionals to halt or adequately control the severity of protein-calorie malnutrition in the individual with cancer indicates that the syndrome is more complex than inadequate ingestion of nutrients. The presence and severity of protein-calorie malnutrition in individuals with cancer cannot be attributed to one factor. The disease, treatment modalities, and complications of therapy all contribute to its development.

Histochemical and histopathologic changes have been described in mucosa adjacent to and overlying neoplasms and non-neoplastic conditions of the large bowel. These changes are considered by some investigators to be preneoplastic changes. We studied mucosal biopsy specimens obtained at colonoscopy from all sections of large bowel in 34 asymptomatic subjects. Every subject had a documented family history of colorectal cancer (Cancer Family Syndrome of Lynch); in addition, two had previous colorectal cancers and two had cancers diagnosed at the time of colonoscopy. Consistent interpretations of mucin species secretion were not obtained. Microscopic mucosal abnormalities were not detected. Mucin histochemical and histopathologic changes are not promising markers for colorectal cancer in the Cancer Family Syndrome of Lynch.
