Malignant spinal-cord compression (MSCC) is a common complication of cancer and has a substantial negative effect on quality of life and survival. Despite widespread availability of good diagnostic technology, studies indicate that most patients are diagnosed only after they become unable to walk. We review the epidemiology, pathophysiology, and clinical features of MSCC. Clinical trials have informed the optimum management of MSCC, and we review the role of corticosteroids, radiotherapy, and surgery in the management of patients. We also emphasise advances in radiation delivery and the results of a randomised trial that supported aggressive debulking in patients with MSCC.

The pharmacological inhibition of DNA methyltransferases provides novel opportunities for the therapy of human cancers. Several Phase III trials of DNA methyltransferase inhibitors have been completed successfully, but the efficacy of these compounds appears to be compromised by their lack of specificity. To address this issue, much work has been focused on defining the functional characteristics of human DNA methyltransferases. This knowledge will be essential for the design of a new generation of pharmacological inhibitors that target epigenetic processes.

Hydrogen peroxide is a widely available disinfectant that has been reported to cause colitis. We report a case of a 67-year-old man who presented with an acute proctitis caused by a self-inflicted 3% hydrogen peroxide enema. The patient's intention was to cure himself of a recently diagnosed prostate cancer, because the waiting list for oncological consultation was deemed too long. The pathogenesis of hydrogen peroxide mucosal injury and a review of the literature is discussed.

Malignant melanomas in situ can usually be treated in consultation with a specialist (C). Larger lesions may require referral. Based on best available evidence, surgical excision is the first-line treatment for most nonmelanoma skin cancers, with cure rates as high as 98% with proper margins. Consider Mohs surgery for larger lesions, sclerosing lesions with morpheaform histology, or for cosmetically sensitive areas. With properly selected lesions, curettage/electrodesiccation and cryosurgery have cure rates comparable to that of surgical excision.
