TITLE: Squamous-cell carcinoma of the anus in condyloma acuminatum. Successful treatment with preoperative chemotherapy and radiation.

A 40-year-old black male had unresectable giant condylomata acuminata of the anorectum with invasive, squamous-cell carcinoma. He was treated with intravenous 5-fluorouracil and mitomycin-C and with extended field radiation to the primary lesion. An abdominoperineal resection was performed, and the surgical specimen contained no residual cancer. Three years after diagnosis, the patient has no recurrent cancer. This case illustrates that otherwise unresectable giant condylomata acuminata with invasive carcinoma may be rendered operable with chemotherapy and radiation.

After more than 100 reported randomized trials of systemic adjuvant therapy in breast cancer, it is clear that this therapy will prolong the time to recurrence and the survival time significantly, at least in some patient subsets. But there is less than complete agreement as to which subsets, which therapies, how much benefit can be achieved, and when (if ever) the risk/benefit ratio may not favor this approach. The results of these trials, and some trials still in progress, are summarized, and the current understanding of the role of systemic adjuvant therapy for breast cancer is reviewed.

We carried out operative insertion of Jonas penile prosthesis in 10 cases with organic impotence. Erection had been incomplete due to radical surgery for bladder or rectal cancer in 5, pelvic fracture in 1, diabetes in 1, Peyronie's disease in 1 and unknown in 2 of the operated cases. Nine of the 10 cases were able to have sexual intercourse at 5-16 weeks after the operation, but the prosthesis was taken off in the remaining one due to long-lasting pain in the penile and perineal region. We have confirmed that the implantation of Jonas prosthesis is a safe and useful procedure for organic impotence.

Prognostic factors of uterine cervical cancer, endometrial cancer and ovarian cancer were discussed. Among various prognostic factors, clinical stage had the most meaningful label. The prognosis for adenocarcinoma of the uterine cervix is poorer than for epidermoid carcinoma. In endometrial cancer, poorly differentiated adenocarcinoma showed a worse prognosis than well-differentiated adenocarcinoma. One of the most vexing problems in ovarian cancer is that, at the time when a diagnosis of ovarian cancer is made, about 60% of these cases have already spread intraperitoneally and remain no longer suitable for complete resection.

A large scale cohort study on cancer conducted in Japan for 265118 adults aged 40 and above from 1965 to 1982 revealed a striking risk reducing effect of daily consumption of green-yellow vegetables especially in cigarette smokers. Such effect was observed clearly for cancer of all sites, cancers of mouth and pharynx, esophagus, stomach, liver, larynx, lung, urinary bladder, and uterine cervix. Beta-carotene, vitamin C, minerals, such as calcium and iron, and dietary fibre rich in green-yellow vegetables must be operating in reducing risk of these cancers. Significance of these findings in preventing cancer in man was discussed.

The morphologic appearance of osseous metastasis may rarely suggest a specific site of origin. A subperiosteal location has recently been reported to be typical for metastatic lung carcinoma. We report three cases of such metastasis which destroyed the cortex so that it appears saucer shaped. The primary tumor was a bronchogenic carcinoma in every case. We conclude that in the cancer-age group cortical metastasis is a highly characteristic feature of lung carcinoma.

Radiation-induced osteoradionecrosis of the ribs and brachial plexus lesions following irradiation of breast cancer patients are well known phenomena and have been reported several times in the literature. Damage to the shoulder joint after irradiation is uncommon. The case of a 75-year-old woman with breast cancer is described; she first developed a brachial plexopathy and then necrosis of the head of humerus after therapy with combined ortho-voltage and high-voltage radiation. The differential diagnosis and possible pathogenetic mechanisms are discussed.

A rare autopsy case of clear cell carcinoma of the pancreas developing in a 71-year-old Japanese male is described. He complained of epigastralgia and back pain and was clinically diagnosed as having primary pancreatic cancer. After death due to disseminated metastasis and cachexy, autopsy revealed pancreatic clear cell carcinoma metastasizing to various organs including the lungs (lymphoangiosis carcinomatosa). The tumor was almost entirely composed of clear cells with cytoplasm weakly positive for PAS and alcian blue, and negative for Sudan III stains. Histological differential diagnosis from the clear cell carcinoma of other organs, especially that of kidney, is also mentioned.

The diagnosis of cancer is made on histologic examination by recognizing the characteristics of the malignant phenotype, i.e. abnormal cells in abnormal groupings, often in abnormal locations. Histophysiologic gradient culture reconstitutes conditions that meet the spatial imperatives of tissues in nature. A variety of carcinomas arise in the genitourinary system involving both glandular and stratified epithelium. To be considered here are the contrasting polarizations of proliferation of normal and neoplastic rat urothelium, the continuity of sheets of epithelium in nature, the poorly understood stable and unstable interepithelial boundaries, and the formation of organoid endocrinelike tissue in histophysiologic gradient culture of normal human amnion epithelium.

The neuropeptides vasopressin, bradykinin, cholecystokinin, galanin, neurotensin and gastrin-releasing peptide stimulate rapid, transient increases in cytosolic Ca2+ in small cell lung cancer cell lines at nanomolar concentrations. Responsiveness to individual peptides is heterogeneous among the diverse cell lines, but the ability to respond to regulatory peptides is a general phenomenon. Peptide responses demonstrate homologous desensitisation and are blocked by ligand-specific antagonists, indicating that they are mediated by distinct receptors. Many neuropeptides are also secreted by small cell lung cancer. Here we suggest that multiple autocrine and paracrine interactions regulate its growth.

We report three cases of acute leukaemia with t(4;11) (q21;q23), one of them of undifferentiated and the other two of lymphoid phenotype, occurring after adjuvant radiochemotherapy for breast cancer (two cases) or occupational exposure to radiation (one case). Although the myeloid phenotype and characteristic chromosomal anomalies usually observed in secondary leukaemia were lacking, our observations raise the possibility of causal relationship between exposure to carcinogens and the occurrence of a leukaemia with t(4;11).

In this review of the literature, it is emphasized that the published results about the transthoracic pulmonary needle biopsy (TPNB) cannot be readily evaluated due to the inconsistency of the reference tests, the methods and the investigated subjects. Gold-standards for evaluation of lung tumors have been described and are recalled. The limits of the TPNB are pointed out in the diagnosis of solitary pulmonary nodules and benign lesions. In a bayesian approach, it is shown that the TPNB is only useful in confirming an a-priori diagnosis of cancer.

A number of workers exposed to asbestos dust for more than three months were medically examined for lung diseases caused by asbestos. A follow-up examination four years later revealed that the lung capacity of these workers was significantly lower than in the normal population. 9% of them showed changes in the lungs caused by asbestos dust, but no case of cancer was discovered. There was no development of significance in these changes during the four years.

TITLE: The accuracy of local death certificates in cancer of the lung and stomach.

The study analysed the accuracy of local death certificates (LDC) for patients with lung cancer (424 males, 36 females) and stomach cancer (99 males, 62 females) who died between 1979 and 1984. All the diagnoses were confirmed histologically. For a subgroup of lung cancer (males), we also evaluated the national death certificate (NDC) and the correspondence of LDC vs. NDC. The reliability of LDC was quite poor. This paper examines the different reasons for the underevaluation of cancer in LDC and the variables which modify the accuracy of cause of death on the death certificate.
