TITLE: Positron Emission Tomography (PET): an update on applications in breast cancer.

Positron Emission Tomography (PET) is an imaging method which detects alterations in tumor physiology and displays them in an anatomically precise manner. Historically, the method has been a research tool, but it is being increasingly applied to clinical imaging problems. PET, using the glucose analog 18-F-fluoro-2-deoxy-D-glucose (FDG), has been applied most widely and shows promise in detecting primary cancers, characterizing breast masses, staging for axillary metastases, evaluating for systemic metastases, and following response to therapy. This review summarizes the current status of this technique, which is in rapid evolution as new tracers and new imaging cameras become available.

Malignant spinal-cord compression (MSCC) is a common complication of cancer and has a substantial negative effect on quality of life and survival. Despite widespread availability of good diagnostic technology, studies indicate that most patients are diagnosed only after they become unable to walk. We review the epidemiology, pathophysiology, and clinical features of MSCC. Clinical trials have informed the optimum management of MSCC, and we review the role of corticosteroids, radiotherapy, and surgery in the management of patients. We also emphasise advances in radiation delivery and the results of a randomised trial that supported aggressive debulking in patients with MSCC.

UFT, a combination of uracil and tegafur, is a second-generation anticancer agent. UFT has been used in Japan, other Asian countries, South America, and Russia. Recently, UFT has been extensively studied for colorectal, pancreatic, and various types of cancer in North America and Europe, especially with leukovorin. We report a case of a scleroderma-like reaction induced by long-term administration of UFT. This is the first report of UFT-induced scleroderma-like reaction.

The implantation of cadaveric fat for breast augmentation, which has been performed up to the Seventies, is nowadays considered obsolete. Since complications frequently arise many years later, we are, however, occasionally confronted with the problematic consequences of the technique. The present case report describes the clinical, mammographic and for the first time to our knowledge MR-mammographic findings of a mammoplasty with cadaveric fat in a 57-year-old patient with a history of breast cancer.

Epstein-Barr virus (EBV) targeted therapeutic strategies for viral associated malignant diseases have received only perfunctory consideration, first, because latent herpesviruses have been intractable to antiviral chemotherapy and, second, because the role EBV has in maintenance of the malignant cell phenotype has been uncertain. Two patients with EBV related primary central nervous system lymphoma (PCNSL) in the setting of advanced AIDS, were treated with low dose hydroxyurea based on in vitro anti-EBV activity. The responses obtained here suggest the promise of antiviral approaches in select cancers.

TITLE: A novel antineuronal antibody in a motor neuron syndrome associated with breast cancer.

A 72-year-old woman developed a lower motor neuron syndrome (MNS) 4 months before the appearance of breast cancer. Monoparesis progressed to quadriparesis despite high-dose IV immunoglobulins, plasma exchange, and azathioprine, and high-dose IV methylprednisolone. The patient improved only after the removal of the tumor. MRI demonstrated hyperintensities in the cervical spinal cord. The patient had antibodies that reacted with axonal initial segments and nodes of Ranvier. The findings suggest that in this patient lower MNS may be a paraneoplastic condition associated with breast cancer.

The two HPV-encoded oncoproteins, E6 and E7, can independently induce chromosomal abnormalities. We summarize the current state of knowledge concerning HPV-induced genomic instability and discuss its significance in the context of human carcinogenesis.
