TITLE: Detection of central nervous system metastasis with cerebrospinal fluid beta-2-microglobulin.

Beta-2-microglobulin determinations in cerebrospinal fluid (CSF) of patients with leukemia, lymphoma, and oat cell carcinoma were performed to evaluate the utility of this test in early detection of metastasis to the central nervous system (CNS). In the presence of CNS disease, significantly higher levels (P less than 0.001) were found than in normals or patients without CNS disease. Serial measurements were performed on two patients, and in all cases beta-2-microglobulin became elevated coincident with CNS relapse, and returned to normal as remission was achieved. Serial testing of CSF beta-2-microglobulin, in patients with cancers known to invade the CNS, should facilitate prompt treatment of CNS metastasis.

The mortality experience of a nation-wide cohort of 293,958 veterans was analyzed by occupation and industry to generate hypotheses in occupational cancer. Results are presented on 107,563 deaths occurring between 1954 and 1970. Information on usual employment (occupation and industry) and smoking habits was available from questionnaires completed in 1954 and 1957. Complete enumeration of these results by occupation and industry is available as special National Cancer Institute monographs from the author (A. B.). This report presents the mortality experience for selected occupations. Excesses of lung cancer among shipyard workers, truck drivers, and plumbers are consistent with previous reports. Elevated risks for stomach cancer among carpenters and machinists may reflect exposure to dusts, abrasives, and cutting oils.

A 40-year-old black male had unresectable giant condylomata acuminata of the anorectum with invasive, squamous-cell carcinoma. He was treated with intravenous 5-fluorouracil and mitomycin-C and with extended field radiation to the primary lesion. An abdominoperineal resection was performed, and the surgical specimen contained no residual cancer. Three years after diagnosis, the patient has no recurrent cancer. This case illustrates that otherwise unresectable giant condylomata acuminata with invasive carcinoma may be rendered operable with chemotherapy and radiation.

The authors describe strains of human cancer of the stomach, colon, cervix and corpus uteri as well as uterine chorionepitheliomas transplanted into nude mice and rats. The histological appearance of these strains is in full accordance with that of primary tumors. Electrophoresis of lactate dehydrogenase isozymes indicates that the strains mainly consist of human cells. The strains are transplanted in nude animals at various time intervals. The strains of human gastric cancer are transplanted from animal to animal in a month and those of corpus uteri in a week.

For cancer therapy, especially radiation therapy, it is very important to determinate the invaded region. Since computed tomography (CT) is introduced into medical radiography, it has become well known that it has a highly diagnostic value not only for the intracranial lesions, but also for extracranial pathological processes. In this paper, 4 cases (pinealoma, rhabdomyosarcoma of the ear, nasopharynx tumor and Histiocytosis X of the orbit) are presented. In each case, pre-therapeutic and posttherapeutic CT images are shown. CT is also very useful during and after therapy in patients with malignancy of the head and neck.

The results of these trials, and some trials still in progress, are summarized, and the current understanding of the role of systemic adjuvant therapy for breast cancer is reviewed.

Pneumatosis intestinalis is an unusual, but potentially serious, postoperative complication. The differential diagnosis includes life-threatening conditions, such as intestinal necrosis, that may require immediate surgical intervention. The authors describe a case of pneumatosis intestinalis occurring postoperatively in a seriously ill patient with advanced ovarian cancer. The cause of pneumatosis intestinalis was attributed to enteral feeding by catheter jejunostomy, and a successful outcome is reported after conservative management. As the use of enteral feeding through catheter jejunostomies will likely increase, this cause of pneumatosis intestinalis must be recognized.

TITLE: [Treatment of organic impotence by implantation of Jonas prosthesis]

We carried out operative insertion of Jonas penile prosthesis in 10 cases with organic impotence. Erection had been incomplete due to radical surgery for bladder or rectal cancer in 5, pelvic fracture in 1, diabetes in 1, Peyronie's disease in 1 and unknown in 2 of the operated cases. Nine of the 10 cases were able to have sexual intercourse at 5-16 weeks after the operation, but the prosthesis was taken off in the remaining one due to long-lasting pain in the penile and perineal region. We have confirmed that the implantation of Jonas prosthesis is a safe and useful procedure for organic impotence.

TITLE: Effective high-dose chemotherapy with autologous bone marrow infusion in resistant ovarian cancer.

Two patients with persistent minimal ovarian cancer after conventional polychemotherapy were treated with high doses of cyclophosphamide and VP 16-213 followed by autologous bone marrow infusion. Ten months afterward no clinical signs of tumor were apparent. In one patient the complete response was surgically documented. Toxicity included cardiac and pulmonary arrest during marrow infusion in one patient, but was otherwise manageable. This method of late intensification of chemotherapy in patients with persisting ovarian cancer merits further investigation.

The use of cisplatin may be associated with severe nausea and vomiting. Two separate, randomized, double-blind trials, comparing the anti-emetic effect of chlorpromazine with placebo and chlorpromazine with droperidol, were conducted in patients receiving cisplatin for ovarian cancer. Chlorpromazine was statistically superior to placebo in the control of nausea and vomiting in those patients treated with chlorpromazine had significantly less nausea than with droperidol, but there were no other significant differences between chlorpromazine and droperidol. Toxicities of chlorpromazine and droperidol were similar. Chlorpromazine shows useful activity against cisplatin nausea and vomiting.

A prevalence screening program with colonoscopy was undertaken in four kindreds manifesting the cancer family syndrome. Forty-five percent of counseled patients underwent colonoscopy, providing 42 asymptomatic individuals. Seven patients (17%) were found to have adenomatous or villous polyps; two of these were malignant and one showed epithelial atypia. Three of the seven patients with polyps had multiple lesions. Only two patients had rectosigmoid polyps. From these limited data, colonoscopy is recommended as a useful screening procedure in individuals at high risk for colorectal cancer.

Newer aspects of therapeutic potentials of cannabis and cannabinoids are reviewed. The major active constituent of cannabis sativa, delta-9-tetrahydrocannabinol and synthetic cannabinoids are evaluated in several clinical trials on their antiemetic efficacy in cancer chemotherapy induced vomiting. 80% of patients refractory to standard antiemetic treatment could be improved with the synthetic cannabinoid levonantradol. Other therapeutic effects, which are presently investigated in clinical trials are analgesia, antispasticity, anticonvulsion and the reduction of intraocular pressure in glaucoma. The future goal of cannabinoid research is the separation between specific pharmacologic activities and undesirable psychotropic effects.

Prognostic factors of uterine cervical cancer, endometrial cancer and ovarian cancer were discussed. Among various prognostic factors, clinical stage had the most meaningful label. The prognosis for adenocarcinoma of the uterine cervix is poorer than for epidermoid carcinoma. In endometrial cancer, poorly differentiated adenocarcinoma showed a worse prognosis than well-differentiated adenocarcinoma. One of the most vexing problems in ovarian cancer is that, at the time when a diagnosis of ovarian cancer is made, about 60% of these cases have already spread intraperitoneally and remain no longer suitable for complete resection.

Despite the lack of absolute confirmation from a properly controlled clinical trial, there is now sufficient evidence to permit the working assumption that screening mammography beginning at age 40 will play a substantial role in controlling breast cancer. An analysis of available data indicates that the benefits of mammographic screening far exceed potential risk, and that earlier detection of cancer will actually add years to life rather than simply permit an earlier diagnosis. American radiologists are now challenged to provide screening mammography in an easily accessible and inexpensive form, so that it is effectively available to all women over age 40.
