Peptide hormones LH, FSH, TSH, e-peptide, prolactin, HCG, PTH, calcitonin and prostaglandin F2 alpha were evaluated in 25 patients with renal adenocancer. Prolactin elevation found in 45% was unrelated to the stage of the disease, while aberrant TSH and FSH were more sensitive in indicating distant metastasis. Prostaglandin F2 alpha, measured as a stable degradation product 13,14-DHK-PGF2 alpha, was frequently elevated and normalized following tumor nephrectomy within 12 month. Serum PTH was decreased in patients with tumor dissemination suggesting resorptive hypercalcemina, whereas none of the patient had circulating HCG levels. Therefore even patients without any paraneoplastic syndromes have a frequent change of peptide hormones directly or indirectly caused by renal cancer.

TITLE: The sequential analysis of cancer induction with chemicals.

A critical review of some of our current views on how chemicals induced cancer is presented. Emphasis is placed on pathologic processes, rather than on agents and on the many steps that lie between initial exposure to a carcinogen and the appearance of malignant neoplasia. The nature of some of the early steps in carcinogenesis is outlined including the early appearance during initiation of cells resistant to some of the inhibitory effect of carcinogens. This property is considered to be important in the development of cancer in at least one model of carcinogenesis. The essential characteristic of promotion, differential stimulation of proliferation of initiated cells, is discussed along with at least three possible mechanisms for such differential effects of promoting environments. The multistep hypothesis for cancer development is supported by observations of identifiable biochemical and biological properties of different cell populations at some steps. The implications of this for cancer diagnosis, prevention and control are briefly discussed.

TITLE: Skin cancer and water arsenic in Lane County, Oregon.

It was expected that the incidence of skin cancer would be related to the known high arsenic levels in water supplies of Lane County, Oregon. Incidence rates were based on all histopathologically confirmed cases among Lane County residents diagnosed during 1958-1971 and were compared to mean water arsenic levels measured during 1968-1974. Basal cell carcinoma had a mean annual incidence rate of 88 per 100,000, a 5:4::M:F sex ratio, and an increasing risk for both sexes in urban areas. Squamous cell carcinoma incidence was 50 per 100,000 and showed 2:1:M:F sex ratio but no urban predilection. Neither type of skin cancer was directly related to the arsenic levels as expected, although sporadic individuals with sufficient exposure could manifest the relationship. Fewer than expected drinking water sources were found to contain high arsenic content, so that the Lane County data do not necessarily contradict previous reports of positive correlation. The basal cell carcinoma pattern suggested that this disease might be potentiated by urban air pollutants.

TITLE: Detection of central nervous system metastasis with cerebrospinal fluid beta-2-microglobulin.

Beta-2-microglobulin determinations in cerebrospinal fluid (CSF) of patients with leukemia, lymphoma, and oat cell carcinoma were performed to evaluate the utility of this test in early detection of metastasis to the central nervous system (CNS). In the presence of CNS disease, significantly higher levels (P less than 0.001) were found than in normals or patients without CNS disease. Serial measurements were performed on two patients, and in all cases beta-2-microglobulin became elevated coincident with CNS relapse, and returned to normal as remission was achieved. Serial testing of CSF beta-2-microglobulin, in patients with cancers known to invade the CNS, should facilitate prompt treatment of CNS metastasis.

TITLE: Alpha 1-acid glycoprotein in gastric cancer juice.

Gastric juice was neutralized (nGJ) in vivo by 80 ml of a phosphate buffer containing radiolabelled vitamin B12 as dilution indicator. Unprocessed nGJ was analyzed in the double gel diffusion technique for the presence of serum proteins using monospecific antisera. Alpha1-Acid glycoprotein (AGP) was found in a high incidence (36 out of 38 subjects) in nGJ of gastric cancer patients. AGP was also observed less frequently in nGJ of patients with Billroth II resections (6/15), metaplasia (11/52), gastric ulcer (3/24), chronic atrophic gastritis (2/26) and chronic gastritis (3/63). AGP was absent in the control group (0/21), in patients with surface gastritis (0/38) and in subjects with normal acid secretion (0/45). Immunochemical studies demonstrated no identity of AGP with human "gastrointestinal tumor associated antigens." In 7 out of 17 AGP positive samples immunochemical differences between gastric and serum AGP were observed.

TITLE: Taste acuity and food attitudes of selected patients with cancer.

Patients with cancer may display taste abnormalities. In this study, abnormalities observed before treatment involved decreased salt and sweet sensitivity. For some thresholds, greater abnormalities were observed among men and women, among patients with colon cancer than among those with breast cancer, and among those with a greater extent of disease. Observations of slight increases in thresholds for sour detection and bitter recognition during the early treatment period and normalization of high sweet recognition thresholds after two weeks of treatment suggest that additional changes in taste acuity may accompany short-term treatment with 5'fluorouracil. However, absence of a correlation of taste changes with changes in food preferences points to the role of other factors in determining patients' food preferences during such treatment. Consideration of the incidence of taste abnormalities in these groups of patients with cancer and observation of altered food attitudes among patients receiving 5-fluorouracil does provide a basis for general recommendations for serving foods which will appeal to these patients.

The mortality experience of a nation-wide cohort of 293,958 veterans was analyzed by occupation and industry to generate hypotheses in occupational cancer. Results are presented on 107,563 deaths occurring between 1954 and 1970. Information on usual employment (occupation and industry) and smoking habits was available from questionnaires completed in 1954 and 1957. Complete enumeration of these results by occupation and industry is available as special National Cancer Institute monographs from the author (A. B.). This report presents the mortality experience for selected occupations. Excesses of lung cancer among shipyard workers, truck drivers, and plumbers are consistent with previous reports. Elevated risks for stomach cancer among carpenters and machinists may reflect exposure to dusts, abrasives, and cutting oils.

TITLE: Squamous-cell carcinoma of the anus in condyloma acuminatum. Successful treatment with preoperative chemotherapy and radiation.

A 40-year-old black male had unresectable giant condylomata acuminata of the anorectum with invasive, squamous-cell carcinoma. He was treated with intravenous 5-fluorouracil and mitomycin-C and with extended field radiation to the primary lesion. An abdominoperineal resection was performed, and the surgical specimen contained no residual cancer. Three years after diagnosis, the patient has no recurrent cancer. This case illustrates that otherwise unresectable giant condylomata acuminata with invasive carcinoma may be rendered operable with chemotherapy and radiation.

TITLE: Quantification of combined radiation therapy and chemotherapy effects on critical normal tissues.

In order to determine the modification of radiation effects on critical normal tissues which occurs with combinations of radiation and cancer chemotherapy, a review of laboratory and clinical data has been carried out. Information on 10 different normal tissues is available. It is clear that the antibiotic cancer chemotherapeutic agents are the most likely to enhance radiation injury, with increased levels reported in all tissues except the central nervous system. The second most common type of injury with combination therapy appears to occur with drugs causing injury to the normal tissue on their own, such as adriamycin in the heart and methotrexate in the central nervous system. Quantification of the dose-effect factor is only available on a limited number of tissues, and, primarily, in experimental animals. From these limited data, it is clear that dose-effect factors between 1.1 and 1.8 are seen, indicating that radiation doses must be reduced by 10-80% for the same level of injury when combined with chemotherapy. The augmentation of radiation damage by cancer chemotherapeutic agents is a serious problem in a wide range of tissues, but a problem which can be dealt with by accurate knowledge as to the dose-effect factor and appropriate modification of the radiation treatment.

Nine cases of suprarenal masses without endocrine abnormalities are presented: 2 adrenal pseudocysts, 2 adrenal myelolipomas, 2 neuroblastomas, 1 lymphoma metastatic to the adrenal glands, 1 teratoma, and 1 adrenal cortical carcinoma metastatic to the contralateral gland. The clinical and radiologic findings are discussed. Noninvasive and invasive procedures and their usefulness in diagnosing masses in the suprarenal area are reported. The importance of metastases to the adrenal glands in the differential diagnosis of suprarenal masses was investigated by reviewing the autopsies done from January through June, 1979, at The University of Texas System Cancer Center, M. D. Anderson Hospital and Tumor Institute, Houston, Texas. Adrenal metastases were found in 26 per cent of the cases studied. Breast and lung carcinomas and lymphomas were the most commonly found neoplasia. The relative incidence of microscopic and macroscopic metastases in all patients with tumor is discussed.

This study was undertaken to examine the frequency of histologic endometrial anomalies with different types of estrogen therapy, whether or not associated to progesterone treatment. 1002 endometrial biopsies were done on 745 women who had received: 1) cyclic treatment with low-dose estrogen; 2) cyclic treatment with high-dose estrogen; 3) sequential estrogen-progestogen treatment; and 4) subcutaneous implant of estradiol. Frequency of abnormalities was 7.0% in the first group, 14.8% in the second group, 1.2% in the third group, and 14.8% in the fourth group. Without drawing definitive conclusions from these results, it seems possible to affirm that the use of progestogen diminishes the risk of endometrial cancer during menopause.

TITLE: Radiologically occult in situ and incipient invasive epidermoid lung cancer: detection by sputum cytology in a survey of asymptomatic cigarette smokers.

Approximately 8000 cigarette-smoking men over the age of 45 have entered into a lung cancer detection program in New York City. Cytologic examinations of sputum were carried out on 4000 subjects and lung cancer was found by this technique in nine men with normal chest x-rays. Seven had in situ or incipient invasive epidermoid carcinoma confined to the bronchus. These seven cases were studied by detailed histologic examinations of the bronchial tree in the resected specimens through sixth generation subsegmental bronchi. It was concluded that: 1) invasive epidermoid carcinoma arises from carcinoma in situ of bronchial surface epithelium or an extension of that neoplastic epithelium in bronchial glands; 2) the site of origin is a segmental bronchus in most instances; and 3) each carcinoma should be considered as unifocal in origin even though there is a continuing risk of another primary lung cancer. It seems unlikely that squamous metaplasia or basal hyperplasia is an essential step in carcinogenesis; rather, we believe that carcinoma may arise in bronchial epithelium without regard to the presence or absence of basal hyperplasia or squamous metaplasia, which should be considered nonspecific reactions to injury that may or may not accompany carcinogenesis.

Hormone treatment, radiotherapy and anti-cancer chemotherapy are often used to treat prostate cancer. However, there is no effective method of treating hormone-independent prostate cancer. In this study, we attempted to establish a new treatment method for hormone-independent prostate cancer. We developed a new recombinant adenovirus vector containing a suicide gene and controlled by a tissue specific promoter, and examined the usefulness of gene therapy for hormone-independence and PSA expression in prostate cancer. We have also examined the usefulness of gene therapy involving an adenovirus and various tumor suppressor genes for human prostate cancer cells, which are under trial in the United States.

The extraordinary specificity of immune responses mediated by T cells against individual syngeneic tumors has led to the concept that many tumor antigens are 'unique'. The recent isolation of several T-cell-recognized unique antigens from various murine and human tumors has shown that the antigenic peptides are caused by somatic mutations and, thus, are truly tumor-specific. The following review summarizes current knowledge about these mutant tumor-specific antigens and their possible role in the development and progression of cancer. It also discusses some functional differences between mutant tumor-specific and shared tumor antigens, which generally represent unaltered peptides, also present on some normal cells.
